    Notes: Summary The internal carotid artery system in swine has a special anatomic configuration similar to a brain “arterial-arterial malformation”. The internal carotid artery breaks up into a multitude of fine channels (rete mirabile) situated at the base of the skull on the side of the hypophysis. This anatomic arterial model was used to analyze acute and chronic angiographic and histological changes after occlusion of the rete mirabile with I) avitene, II) avitene, and 50% ethanol, III) avitene, 30% ethanol and Polyvinyl alcohol, IV) avitene 50% ethanol and Polyvinyl alcohol, V) IBCA and VI) silk. Histopathological changes observed in the rete mirabile six weeks following occlusion demonstrated that a mixture of avitene, 30% ethanol and Polyvinyl alcohol and IBCA produced the best anatomic results. Embolization with avitene, PVA and ethanol induced a more bland histological reaction than the one observed with IBCA. Preliminary clinical experience with this mixture is reassuring in those cases in which the AVM was surgically resected. The partially thrombosed AVM was easily depressed and compressed by the neurosurgeon allowing for satisfactory hemostasis in and around the nidus of the AVM.

    Description / Table of Contents: Résumé Cet article rapporte le cas d'une femme dont le tableau clinique, atypique, comportait des signes évoquant une encéphalite et d'autres une tumeur cérébrale. L'angiographie cérébrale objectivait une masse cérébrale unilatérale et une dilatation des veines médullaires profondes, habituellemant interprêtée comme un signe de néoplasme. Le diagnostic d'astrocytome fut porté à l'examen de la biopsie cérébrale. Par la suite, une rémission clinique spontanée et une régréssion des signes angiographiques furent constatées, ce qui a une réévaluation de l'examen microscopique et au diagnostic final d'encéphalite. La pathogenie des anomalies angiographiques cérébrales observée en cas d'encéphalite est passée en revue. La dilatation transitoire des veines médullaires profondes est probablement due à l'hyperhémia. La répétition a distance de l'angiographie est recommandée lorsque le diagntic d'encéphalite est suspecté.
    Abstract: Zussamenfassung Dieser Bericht betrifft den Fall einer Frau deren Erkrankung teilweise als Enzdphalitis, teilweise als Hirntumor angesehen wurde. Zerebrale Angiographie zeigte eine einseitige zerebrale Masse mit hervorstehenden tiefen medullaren Venen, gewöhnlich als Beweis einer bösartigen Geschwulst angenommen. Biopsie wurde als Astrozytom gelesen. Spontane Remission und Zurückgehen der angiographischen Befunde führten zu einer Revision der mikroskopischen Diagnose als Enzephalitis. Die Pathogenese der abnormalen zerebralen angiographischen Befunde wird diskutiert. Das vorübergehende Auftreten der tiefen medullaren Venen ist wahrscheinlich durch die Hyperämie bedingt. Nachfolgende Angiographie ist empfohlen, wenn ein Verdacht auf Enzephalitis besteht.
    Notes: Summary This case report concerns a woman who presented an atypical clinical pattern with some features of encephalitis and other features of brain tumor. Cerebral angiography showed a unilateral cerebral mass with prominence of deep medullary veins, usually interpreted as evidence of a neoplasm. Brain biopsy was interpreted as astrocytoma. Subsequent spontaneous clinical remission and regression of angiographic findings led to a reappraisal of the microscopic interpretation with a final diagnosis of encephalitis. The pathogenesis of cerebral angiographic abnormalities observed with encephalitis is reviewed. The transient prominence of deep medullary veins is probably due to hyperemia. Followup angiography is recommended when encaphalitis is suspected.

    Notes: Summary This report concerns a group of 49 patients who had been diagnosed as having gliomas on the basis of CT scans plus angiograms and for whom histologic proof was available. Ninety percent were found to be gliomas, 4% were metastases, and histology was unclear in the rest. Combining the CT criteria of inhomogeneous enhancement and surrounding edema with one or more of three angiographic signs (early venous drainage, stain, abnormal vessels) allowed a more confident diagnosis of glioblastoma.

    Notes: Summary Magnetic resonance imaging (MRI) studies were performed on 27 patients with vascular lesions of the central nervous system before and after embolization with either IBCA, polyvinyl alcohol foam particles, Avitene (microfibrillar collagen) or balloons. Thirteen pial brain arteriovenous malformations (AVMs), 3 brain AV fistulas, 2 giant aneurysms, 5 dural AVMs, 1 vertebro-vertebral fistula and 3 meningiomas were studied. The pre-embolization MR demonstrated the nidus and venous drainage of all pial AVMs. MR failed to detect 3 out of 5 dural AVMs using only spin echo sequences. A draining vein alone was seen in the remaining two cases. MR was superior to CT in detecting contiguous parenchymal changes such as atrophy, reversible ischemia, and mass effect in the pre-embolization studies. Following embolization, MR demonstrated partial or complete obliteration of the vascular nidus in all 13 pial AVMs. The embolized area was seen as an area of increased signal consistent with thrombus where previously there had been signal void. Ischemic or edematous changes in the brain parenchyma following embolization were seen on MR more easily than on CT scans. MR was accurate in the assessment of aneurysm patency, degree of thrombosis and balloon position in both giant aneurysms, and AV fistulae. These MR findings had an impact on patient management. MR will be an increasingly useful tool in the diagnosis and management of a number of neurovascular diseases requiring endovascular intervention.

    Notes: Summary A patient with subarachnoid hermorrhage secondary to a basilar artery aneurysm developed severe bilateral middle and anterior cerebral artery vasospasm with extensive neurologic deficits. Microangioplasty of the middle cerebral artery segments bilaterally led to reversal of the neurologic deficits, allowing early operative treatment of the aneurysm in a previously inoperable patient.

    Description / Table of Contents: Résumé Vingt-neuf cas de ménigiomes pré-sellaires ont été revus pour evaluer la valeur de chacun des tests radiographiques dans la détection de ces tumeurs. La dimension de la tumeur a été comparée aux résultats des tests. Les radiographies du crâne et les scintigraphies cérébrales étaient souvent normales, en particulier lorsque les tumeurs mesuraient moins de 2 cm de haut. La pneumoencephalographie et l'arteriographie carotidienne étaient anormaux dans tous les cas étudiés sauf pour un méningiome clinoidien antérieur visible seulement sur les tomographies de la selle turcique. Dans 75% des cas l'artériographie opacifia la tumeur, permettant d'analyser sa vascularisation. Ce pourcentage elevé de détection est à verser au crédit des pneumoencephalo-tomographies et des soustractions stéréoscopiques des arteriographies sélectives interne et externe.
    Abstract: Zusammenfassung In 29 Fällen von präsellar gelagerten Meningeomen wurden die verschiedenen radiologischen Untersuchungen vom Standpunkt ihrer Fähigkeit, die korrekte Diagnose zu stellen, untersucht. Die Berechnung der Tumorgröße wurde für die verschiedenen Untersuchungsmethoden verglichen. Schädelaufnahmen und Szintigramme gaben in einer erheblichen Anzahl von Patienten, besonders in jenen, in den der Tumor weniger als 2 cm über das Planum sphenoideum ragte, falsche negative Resultate. Pneumographie and Carotis Arteriographie waren dagegen in allen Fällen, bis auf einen einzigen, positiv. Bei diesem Patienten konnte ein Meningeom des Processus alae parvae nur mit Hilfe von Polytomographie der Sella diagnostiziert werden. Eine „Anfärbung” des Tumors wurde in 75% der Arteriogramme erkannt. Die im Arteriogramm demonstrierte Blutversorgung der Tumoren wurde weiterhin studiert. Tomographie in Kombination mit der pneumographischen Untersuchung und stereoskopische Subtraktion der selektiven Carotis interna and externa Arteriographie werden als Erklürung der hohen diagnostischen Genauigkeit dieser Untersuchungsmethoden anerkannt.
    Notes: Summary Twenty-nine cases of presellar meningiomas were reviewed to determine the value of each of several radiological tests in the detection of these tumors. Tumor size was correlated with the test results. Skull films and brain scans were false negative in a significant number, particularly in the group of tumors of less than 2 cm in height. Pneumoencephalograms and carotid angiograms were positive in all cases studied except for one anterior clinoid meningioma detected only by sellar polytomography. Tumor stain was noted on 75% of angiograms and tumor vascular supply was analyzed. These high rates of detection were believed to be due to the use of stereoscopic subtractions of selective internal and external carotid angiograms and the use of tomography during pneumoencephalography.
