    Description / Table of Contents: Résumé Le traitement de l'ischémie du membre inférieur due à l'athéromatose chronique ne peut être planifié qu'après étude fonctionnelle quantitative par des méthodes non agressives et appréciation des risques respectifs de l'évolution naturelle de l'affection et de l'évolution postopératoire. Cuand la claudication intermittente est prononcée, la chirurgie restauratrice est indiquée. Le court-circuit aorto-fémoral est préférable à la thrombo-endartériectomie, sa mortalité étant inférieure (2 à 3%) et la perméabilité à long terme étant meilleure (82% après 5 ans et 72% après 10 ans). En fait, la thrombo-endartériectomie doit être réservée aux lésions segmentaires de l'artère iliaque primitive. Le court-circuit extra-corporel donne les mêmes résultats mais il doit être réservé aux cas où l'abord direct est contre-indiqué. La chirurgie d'exérèse au-dessous du ligament inguinal aboutit à des résultats satisfaisants quand la veine saphène interne peut être employée. Hay a peu de différence dans le taux de perméabilité selon le type de prothèse utilisée. Il dépend du flux artériel postopératoire et s'abaisse de 76 à 42% après 5 ans lorsque des lésions périphériques sont présentes. Dans les cas de douleurs de décubitus insupportables ou de problèmes trophiques, la chirurgie restauratrice est indispensable. La revascularisation de l'artère fémorale profonde permet de sauver le membre inférieur. Il est possible d'éviter l'amputation de la jambe dès qu'une seule artère distale est fonctionnelle. L'indication de l'amputation d'emblée dépend de l'âge du malade et de sa condition générale.
    Abstract: Resumen El plan de tratamiento en la isquemia ateromatosa crónica de las extremidades inferiores sólo puede ser establecido después de cuantificar los problemas funcionales mediante técnicas no invasivas y después de comparar los riesgos de la progresión natural de la enfermedad con el riesgo y el posible resultado de la intervención quirúrgica. Cuando la claudicación se encuentra en un estado avanzado, la cirugía reconstructiva está indicada. La derivación (bypass) aorto-femoral es preferible a la tromboendarterectomía, siendo su mortalidad más baja (2 a 3%) y mejor su permeabilidad a largo plazo (82% después de 5 anos y 72% después de 10 años). La tromboendarterectomía debe ser reservada para lesiones segmentarias de la arteria ilíaca primitiva. La derivación (bypass) extra-anatómica posee una tasa similar de permeabilidad, pero debe ser reservada para aquellos casos en que el aproche directo esté contraindicado. La cirugia directa en regiones inferiores al ligamento inguinal da resultados satisfactorios cuando la vena safena interna puede ser utilizada. Hay poca diferencia en la tasa de permeabilidad entre los diferentes tipos de prótesis en uso: depende del flujo de salida arterial preoperatoria, y se reduce de 76% a 42% después de 5 años en presencia de lesiones periféricas. En casos de dolor, de reposo o de problemas tróficos, la cirugía reconstructiva es obligatoria. La revascularización de la arterial femoral profunda asegura el salvamento de la extremidad. Es posible evitar la amputación de la pierna cuando hay enfermedad distal, siempre y cuando el arco pédeo se encuentre funcionando. Las indicaciones para amputación primaria todavía dependen de la edad del paciente y del estado general de salud.
    Notes: Abstract The treatment plan in chronic atheromatous limb ischemia can only be established after quantifying functional problems by noninvasive techniques and after comparing the risk of the natural progression of the disease with the risk and possible outcome of surgical intervention. When claudication is at an advanced stage, reconstructive surgery is indicated. Aortofemoral bypass is preferable to thromboendarterectomy, its mortality rate being lower (2–3%) and long-term patency rate better (82% after 5 years and 72% after 10 years). Thromboendarterectomy must be reserved for segmental lesions of the common iliac artery. The extra-anatomical bypass has a similar patency rate, but it should be reserved for patients in whom a direct approach is contraindicated. Direct surgery below the inguinal ligament gives satisfactory results when the internal saphenous vein can be utilized. There is little difference in patency rate among the types of prostheses in use; it depends on the preoperative outflow and it decreases from 76% to 42% after 5 years if peripheral lesions are present. In the case of rest pain or trophic problems, reconstructive surgery is obligatory. The revascularization of the profunda femoris artery ensures limb salvage. It is possible to avoid amputation of the leg if distal disease is present provided that 1 pedal arch is functioning. Indications for primary amputation still depend on the patient's age and general state of health.

    Notes: Abstract Two cases of liver angiomyolipoma (AML) are presented. The first case has the classical imaging findings previously reported. The second case has no fat content on the various imaging modalities. AML should probably be included in the differential diagnosis of any hypervascular lesion of the liver. Preoperative diagnosis with core biopsy is possible.
