    Notes: The cellular localization of immunoreactive tissue kallikrein and kininogen was studied in normal and psoriatic human skin. Immunoreactivity to both enzyme and substrate was observed in secretory granules of the dark cells in the secretory fundus (acinus) of the sweat glands. Double immunostaining revealed a segmental distribution of the two antigens. Each acinar section contained either tissue kallikrein or kininogen. However, there appeared to be a junctional zone in which both were present, but in separate dark cells. Immunoreactivity for both antigens was also observed in close apposition to the luminal microvilli of the duct cells. No specific immunostaining was seen in sebaceous glands, hair follicles, keratinocytes and other cells of the secretory unit such as myoepithelial or clear cells. In psoriatic skin there were in addition many neutrophils immunoreactive for tissue kallikrein in the epidermis and psoriatic scales. Mitogenic action of kinins may account to some extent for the characteristic accelerated turnover of epidermal cells in psoriasis and locally applied kinin antagonists may prove of value in the treatment of this disease.

    Notes: Summary  Focal cerebral contusions can be dynamic and expansive, leading to delayed neurological deterioration. Due to the high mortality associated with such cerebral contusions, our standard practice had evolved into evacuating contusions in patients who had a deterioration in level of consciousness, lesions〉30 cc and CT suggestion of raised ICP. Experimental brain edema studies have implicated kinins in causing 2° brain swelling. CP-0127 (Bradycor TM), a specific bradykinin antagonist, has been found to reduce cerebral edema in a cold lesion model in rats. In a randomized, single blind pilot study, a 7 day infusion of CP-0127 (3.0 ug/kg/min) was compared to placebo in patients with focal cerebral contusions presenting within 24–96 hours of closed head injury with an initial GCS 9–14. The ICP, GCS, and vital signs were monitored hourly. The total lesion burden (TLB) was measured on serial CT scans.  There were no differences in age, baseline GCS, TLB, initial ICP, or laboratory findings between the two groups (n=20). The mean (±s.d) rise in peak ICP from baseline was greater in the placebo group than with CP-0127 (21.9±4.7 vs 9.5±2.0, P=0.018). In addition, the mean reduction in GCS in the placebo group was significantly greater than in the CP-0127 group (4±1.0 vs 0.6±0.4, P=0.002). Significantly raised ICP and clinically significant neurological deterioration occurred in 7/9 patients on placebo (77%) and only in 1 patient (9%; n=11) on CP-0127, mandating surgery (P=0.005). There were no adverse drug reactions, significant changes in vital signs or variations in the laboratory values. The cerebral perfusion pressure was adequately maintained in all patients irrespective of therapy.  These preliminary results with CP-0127 provide supporting evidence that the kinin-kallikrein system could be involved in cerebral edema. In this study, treatment with CP-0127 appeared to alter the natural history of traumatic brain contusions by preventing the 2° brain swelling. In addition, CP-0127 obviated the need for surgery in the majority of treated patients. CP-0127 could act on the cerebral vasculature to limit dys-autoregulation and brain swelling or on the blood brain barrier to reduce cerebral edema.

    Notes: Summary The cellular localisation of kininogen and its relationships with tissue kallikrein containing cells was studied in the human kidney by the peroxidase-antiperoxidase method using antisera to human LMW kininogen and to human tissue kallikrein. Immunoreactive kininogen was localised in the principal cells of collecting ducts. Immunoreactive tissue kallikrein was detected in the connecting tubule cells segment of the nephron preceeding the cortical collecting ducts. The co-existence of tissue kallikrein and kininogen in the same transitional tubule, but in different cells, was established by the use of serial sections and double immunostaining. This anatomical relationship is in accordance with known studies that describe intermingling of principal cells and connecting tubule cells where connecting tubules merge into cortical collecting ducts in the human nephron. the close relationship between cells that contain tissue kallikrein and its substrate, kininogen, suggests that kinins could be generated in the lumen of distal cortical segments of the human nephron.

    Notes: Summary Discrete, PAS-positive granules of relatively uniform electron-density and size characterise the intercalated duct cells of mammalian submaxillary glands. Smaller, electron-dense organelles are seen in the cells at the junction of the intercalary-striated duct region in the guinea-pig. The large granules of variable electron-density which are observed in the proximal, modified intercalary cells in the rabbit closely resemble the granules in the acinar cells of the guinea-pig. Several populations of granules differing in size are found in the striated granular tubules of the rat and hamster; the organelles in the rat show two grades of electron-density whereas those in the hamster are uniformly dense. Numerous small granules with compactly arranged intragranular material occupy the apical part of the striated ducts of the cat, dog and rabbit. The chemical composition of each population of duct cell granules is unknown. The question whether granules containing kallikrein, trypsin-like enzymes and amylase are stored in the duct cells is discussed.

    Notes: Summary Granules in acinar cells show considerable variations in size, shape, electron-density and molecular organisation of secretory material. Discrete organelles with an electron-dense homogenous matrix are seen in the guinea-pig and to some extent in the male hamster. Similar organelles with moderate electron-density are seen in the cat and dog. Acinar cells of the cat, hamster, guinea-pig and to a lesser extent the rat, contain discrete, pale granules and also confluent organelles arising from two to three of the pale particles. Composite, electron-pale secretory units are observed in the dog and rabbit. No correlation could be elicited between the histochemical reactivity of the acinar cells and the content of the secretory enzymes. The relationship between the histochemical reactivity and ultrastructural appearance of acinar cell granules and the organelles containing kallikrein, trypsin-like proteases and amylase seems much more important and functionally relevant. Morphological aspects of the intracellular transport and secretion of granules are discussed.

A strong immunoreactivity for tissue kallikrein was observed in the seromucous glands of the nasal mucosa, trachea, and bronchi. In these glands, the immunostaining was restricted to the serous component of the acinus whereas mucous cells showed no staining. Since no immunoreactivity to kininogen was observed in any of the tissue constituents of the human and guinea-pig respiratory tree, transudation of the substrate from plasma was considered to be the preferred mode of delivery of the kininogen into the bronchopulmonary interstitium and lumen. Our results provide morphological evidence for the well documented presence of tissue kallikrein in bronchial lavage fluids and support the hypothesis that kinins may be one of the more important mediators involved during acute episodes of asthma and rhinitis.
