    Notes: The safety and efficacy of two birch pollen extracts, one chemically conjugated to alginate (Anjuvac®) the other adsorbed to aluminium hydroxide (Alutard®), were investigated in an open multicentre comparative study of 63 birch pollen allergic patients. Both extracts decreased the nasal symptoms during the birch pollen season. The changes in specific IgE and IgG were much the same in both treatment groups. The adverse reactions recorded were mild in both groups, but more frequent in the Anjtivac group, probably because of a more aggressive dose schedule though there were twice as many asthmatics in the Anjuvac group. The two investigated allergen extracts were useful alternatives for immunotherapy.

    Notes: Summary The effect of a single oral dose of pindolol on bronchi was studied in a double-blind cross-over trial in 20 adult patients, most of whom had moderate bronchial asthma. A single dose of pindolol 5 mg, sufficient to cause a significant fall in heart rate and blood pressure at rest, did not produce a significant fall in the forced expiratory volume (FEV1.0). However, two patients showed marked deterioration in lung function after pindolol. A conventional therapeutic dose of salbutamol by inhalation produced a significant increase in ventilatory function relative to placebo, but it did not outweigh the bronchial effect of pindolol.

    Notes: Summary In a prospective study in 73 patients with obstructive pulmonary disease, aged 63.5±13.5 years (SD), it was found that theophylline dose, cigarette smoking and age were all significant determinants of the steady-state trough plasma theophylline level during oral administration of the drug. As the predictive efficiency of the three factors combined amounted only to 25%, firm dosage recommendations cannot be made. Even among elderly patients, tobacco smokers had significantly lower plasma concentrations of theophylline. It is suggested that in order to obtain trough plasma concentrations of 50 µmol/l (≈9 µg/ml), a non-smoking 50 year-old patient would require 9.8 mg/kg/day of oral theophylline, the dose increasing to 14.2 mg/kg/day in smokers of the same age. These doses should probably be reduced by 15–20% in 75 year-old patients.

    Notes: Summary The bronchial effect of intravenous atenolol (ICI 66.082) has been studied in a doubleblind cross over trial in 10 patients with pronounced, labile bronchial asthma. A single i. v. dose of atenolol 3 mg, sufficient to cause a fall in heart rate and systolic blood pressure at rest, induced only a slight and clinically almost negligible impairment of ventilatory function. An ordinary therapeutic dose of salbutamol by inhalation far outweighed the bronchial effect of atenolol. The drug appears promising with regard to its cardio-selective properties.

    Description / Table of Contents: Zusammenfassung Im Rahmen einer Doppelblindstudie wurden Ofloxacin und Trimethoprim-Sulfamethoxazol (TMP/SMX) bezüglich ihrer Wirksamkeit und Sicherheit bei 137 Patienten mit Exazerbation einer chronischen Bronchitis bei chronisch obstruktiver Lungenerkrankung verglichen. Schwere Arzneimittelnebenwirkungen waren in der TMP/SMX-Gruppe häufiger und führten in 14,9% der Fälle zur vorzeitigen Beendigung der Therapie, in der Ofloxacingruppe nur in 6% der Fälle. Bei Patienten, die mit Ofloxacin behandelt wurden, war die Versagerquote signifikant geringer als bei Behandlung mit TMP/SMX (3,2 im Vergleich zu 13,8%). Ofloxacin erwies sich als wirksames Arzneimittel und verursachte nur wenige unerwünschte Nebenwirkungen.
    Notes: Summary In a double-blind study of 137 patients with exacerbation of chronic bronchitis and chronic obstructive lung disease, the efficacy and safety of ofloxacin was compared with that of trimethoprim-sulfamethoxazole (TMP/SMX). Both groups improved. The frequency of severe adverse reactions was highest in the TMP/SMX group, and 14.9% of the patients discontinued the treatment. In the ofloxacin group 6% had to stop the treatment. The failure rate was significantly lower in the ofloxacin-treated patients, 3.2% versus 13.8% in the TMP/SMX group. Ofloxacin was found to be an effective drug with few adverse reactions.
