    Description / Table of Contents: Résumé Dix-sept malades âgés chez qui on pouvait suspecter, sur des signes cliniques ou radiologiques, une fracture du pubis, ont bénéficié d'une scintigraphie au méthylène-diphosphonate de Tc 99m . Tous présentaient une hyperfixation au niveau du pubis et également de la région sacro-iliaque, sauf dans un cas où la fixation sacro-iliaque n'a pu être estimée. La fixation de l'isotope était légérement augmentée chez neuf malades et fortement chez les sept autres. On a réalisé un examen tomodensitométrique dans trois cas. Deux fois, alors qu'il existait une notable hyperfixation, on a pu mettre en évidence une fracture au voisinage de l'articulation sacro-iliaque. Dans le 3ème cas, où l'hyperfixation était seulement modérée, il n'y avait pas de fracture.
    Notes: Summary A consecutive series of 17 elderly patients with radiological or clinical suspicion of pubic fractures were investigated with bone scintigraphy using 99m Tc-methylene diphosphonate. All had an increased isotope uptake over the pubic bone and also over the sacro-iliac region, except in one patient where the sacro-iliac uptake could not be assessed. The isotope uptake was slightly increased in nine patients and markedly so in seven. Computed tomography was performed in three cases. In two who showed a marked increased isotope uptake, fractures adjacent to the sacro-iliac joint could be demonstrated, and in one with only slightly increased uptake, no fracture was found.

    Notes: Summary Eight patients with Wilson's disease (WD) were studied by magnetic resonance imaging (MRI) of the brain; seven also underwent X-ray computed tomography (CT) of the brain. We describe the changes in the brain and try to correlate them with the clinical manifestations and progress of the disease. Six patients were symptomatic, with predominantly neurological problems. Two were asymptomatic, diagnosed upon screening siblings of index cases. Of the six symptomatic patients, five had basal ganglia lesions, combined in four with brain stem changes; in one with only brain stem abnormalities, clinical findings were minimal despite pronounced MRI changes. In three patients MRI abnormalities regressed following chelating therapy. MRI can contribute to documentation of early neurological involvement in WD, especially in patients with no abnormalities on CT. However, MRI changes may not correlate with clinical presentation or response to therapy.

    Notes: Summary Regional intra-arterial drug administration has been advocated in order to reduce the risks of systemic complications in conjunction with systemic tumor chemotherapy. We have been using superselective angiographic techniques to administer the antitumor drug mitomycin C directly into the feeding arteries of the tumor in 15 patients with advanced malignant head and neck tumors. No complications were observed. In five patients the tumor decreased significantly in size, in four patients, tumor cell necrosis was found. A marked decrease in pain was reported by nine of the 15 patients.

    Notes: Summary A total of 103 brain tumor patients examined with CT, radionuclide brain scan, and angiography or pneumoencephalography, and all surgically verified, were studied to evaluate the impact of CT on the neurosurgical handling of brain tumors. CT alone was usually sufficient for optimal handling of astrocytoma patients, angiography in most meningioma cases, and pneumoencephalography in cases with sellar, suprasellar, and some other midline tumors. Information obtained only through CT sometimes altered the therapy. Sometimes it led to biopsy instead of a meaningless attempt at a ‘radical’ excision; in other cases it permitted a ‘radical’ excision otherwise not possible.

    Notes: Summary Swollen orbital extraocular muscles may mimic an orbital apex tumor on computed tomography. In five of our patients, in spite of highly suggestive findings on CT scanning, indicating the presence of an orbital apex lesion, the correct diagnosis proved to be endocrine exophthalmos. The value of complementary CT projections and of orbital ultrasonography to assist in the differential diagnosis is discussed, and the additional contribution of orbital phlebography for the confirmation of lesions in the orbital apex is emphasized.

    Notes: Summary In cases with radiation necrosis of the brain, conventional neuroradiological examinations usually demonstrate only an avascular mass lesion impossible to differentiate from intracerebral tumor or hemorrhage. Computed tomography, performed in addition to angiography in three cases, consistantly demonstrated the radiation necrosis as a low density area exhibiting enhancement following infusion of contrast medium. The importance of recognizing radiation necrosis as a differential diagnosis to brain neoplasm is emphasized.

    Notes: Summary Deficiency of 3-hydroxy-3-methylglutaryl-CoA lyase has been studied in 11 Saudi infants. The diagnosis was established by the measurement of enzyme activity in lymphocytes, in fibroblasts and, in seven patients, by the gas chromatography/mass spectrometer pattern of excreted organic acids in the urine. In seven infants the disease caused a devastating acidotic attack within the first day of life, while in two the crisis occurred by the third day of life. In two infants from one family the disease appeared later in infancy. The clinical presentation of an acidotic attack is lethargy, hyperpnoea, tachypnoea and seizures, either at birth (two infants), following first feeding (in five infants), or following vomiting or refusal of food in later infancy. The acidotic attacks recurred later in life following minor illness or refusal to eat. The acidosis of this enzyme deficiency progresses rapidly, leading to cardiopulmonary arrest and death within hours of onset unless treated promptly. In four surviving infants diagnosed and treated early, development is normal. Magnetic resonance and computerized tomography brain scans in these infants, however, show white matter lesions and mild atrophy.
