    Notes: Conclusion We have used this vascularised bony graft mainly for recurrent pseudarthrosis of the scaphoid bone. An account of our series will be the subject of a future publication. The use of the vascularised pedicle graft at the expense of the head of the second metacarpal has seemed to us to give encouraging results. The technique, although meticulous, is simple to carry out. The good vascularisation of the graft is assured through the large amount of fatty and connective tissue around the base of the pedicle. The use of vascularised bone in hand surgery has been shown to be useful. This graft, easy to raise, has through the length of its pedicle, a limited displacement. Other donor sites do not have the advantages described here.

    Description / Table of Contents: Résumé La vascularisation artérielle dorsale du pouce a été étudiée sur 25 pouces de cadavres non embaumés. Les auteurs ont mise en évidence l'existence constante d'un axe vasculaire artériel en provenant de l'artère radiale et d'une anastomose avec le circuit artériel palmaire au niveau du tiers moyen de la phalange proximale. La constance de cet axe artériel et son anastomose avec le circuit palmaire permettent la mobilisation d'un lambeau cutané dorsal métacarpien à pédicule distal et à flux inversé. Ce lambeau permet la couverture de pertes de substance dorsales et palmaires du pouce.
    Notes: Summary The pattern of the dorsal arterial supply of the thumb was studied by the dissection of 25 thumbs of fresh cadavers. A constant vascular axis was found, originating at the radial a. and communicating at the level of the middle third of the proximal phalanx with the arterial palmar circuit. The constant presence of this vascular axis and its connection with the palmar circuit permits the mobilisation of a dorsal metacarpal skin flap, with a distal pedicle and a reversed flow, that can be used for covering dorsal and palmar losses of substance in the thumb.

    Description / Table of Contents: Résumé Les auteurs décrivent un transfert ostéo-periosté vascularisé, prélevé sur la face antéro-latérale de l'extremité supérieure du tibia. L'étude anatomique porte sur soixante membres inférieurs, conservés au froid. La vascularisation métaphysaire du transfert provient de la branche récurrente tibiale antérieure et de ses rameaux, la vascularisation diaphysaire étant issue des branches musculo-périostiées proximales. Le transfert vascularisé est prélevé avec la membrane interosseuse et peut être pediculé ou libre. L'expérience clinique porte sur deux cas de pseudarthroses multiopérées. Les résultats cliniques et radiologiques sont bons à long terme.
    Notes: Summary A cortico-cancellous flap from the anterolateral aspect of the upper third of the tibia was presented. Sixty lower limbs of “fresh” cadavers were dissected. The vascular bundle includes the anterior tibial artery and its recurrent collateral branches and recurrent and muscllo periostal arteries. The flap is harvested with the interosseous membrane and can be used either free or pedicled. We used this flap for two patients suffering from pseudarthrosis. Long range clinical and radiological results are good.

(Transplantation sur la clavicule du tendon du court biceps avec un fragment de coracoïde). Cette technique qui amène le tendon sur la clavicule à un point situé au dessus de la coracoïde (et qui associe l'ouverture de l'articulation dans le cas où la réduction n'est pas facile à cause des obstacles intraarticulaires) a toujours donné de bons et très bons résultats.
    Notes: Summary A follow-up of 78 patients with acromioclavicular dislocation who have been operated on since 1956 is reported. The tendon of the short head of the biceps is transferred to the clavicle to obtain, and maintain, reduction. The site of transfer in the last 51 cases has been to a point on the clavicle immediately above the coracoid process. The acromioclavicular joint is only opened when an obstruction to reduction is present. The functional results in this group of patients are very good.

    Notes: Abstract Gastroduodenal motility of 16 patientscomplaining of functional dyspepsia and Helicobacterpylori gastritis was recorded by means of alow-compliance manometric system with four recordingports in the stomach and four in the duodenum.Clarithromycin (CLA) 250 mg (group A: 8 patients) ornormal saline solution (group B: 8 patients) was infusedintravenously randomly and in double-blind manner 30 minafter the end of the first recorded activity front(AF) of the migrating motor complex or, in the absenceof AFs, after 200 min of recording, continuing therecording until an AF was observed during the subsequent 200 min. CLA administration was followed by atypical gastroduodenal AF in a significantly highernumber of patients than saline administration. Inaddition, the time-lag between the drug administration and the appearance of AFs was 22 min ±7.4 (mean ± SD), significantly shorter than aftersaline (109 ± 56 min) and the CLA-relatedduodenal AFs showed a duration of 7.4 min ± 1.6in group A, significantly longer than that of the spontaneous AFs (3.5min ± 1), while in group B AF duration aftersaline was not significantly different from that of thespontaneous ones. In conclusion, clarithromycin is able to stimulate cyclic interdigestivegastroduodenal motility. This prokinetic property ofclarithromycin is not unexpected because it is amacrolide like erythromycin, the prokinetic activity ofwhich is well known, and could be utilized fortherapeutic uses.
