    Notes: Abstract : We have determined the exon-intron organization and characterized the 5′-flanking promoter region of DLG4. Encompassing ~30 kb, the DLG4 locus is composed of 22 exons that range in size from 28 to 1,218 nucleotides. All splice sites conform to the GT-AG rule, except for the splice acceptor site of intron 5, which is TG instead of AG. Three different exons of DLG4 were found to be alternatively spliced in a subset of tissues. Two of these variants result in altered postsynaptic density 95 (PSD95) isoforms that dramatically truncate the protein. The third splicing variant represents an extension of exon 4 that encodes an additional 33-amino acid segment. Analysis of the core promoter region for DLG4 suggests that the expression of this gene is controlled by a TATA-less promoter using a single transcriptional start site embedded within a CpG island. DLG4 maps to a region on chromosome 17p13.1 known to contain a locus for autosomal dominant cone dystrophy 5. Scanning for mutations in the DLG4 coding region and splice sites was performed in 15 cone dystrophy patients, including probands from five families showing linkage to the DLG4 region. No disease-causing mutations were identified in any patients, suggesting that DLG4 is not the causative gene for this genetic eye disorder.

    Description / Table of Contents: Résumé Après la perfusion hyperthermique isolée d'un membre avec du melphalan chez le patient ayant un mélanome malin à l'Hôpital Royal de Brisbane réponse et survie ont été analysées afin de déterminer les déterminants majeurs à la réponse au traitement. On a recueillis les renseignements inscrits dans le dossier et obtenus par interview avec le patient. La réponse a été analyséc d'après l'examen clinique du nodule et l'état clinique du patient. La survie a été calculée à partir de la date de la perfusion jusqu'au décès. Depuis 1965, 85 patients (38 hommes et 47 femmes) ont eu 109 perfusions (102 de membre inférieur, 7 de membre supérieur). L'âge moyen était de 58 ans (extrêmes 32–82 ans). La répartition des stades de la maladie (MD Anderson) a été comme suit: stade I: 5, stage II: 8, stade IIIab: 25, et stade IIIb: 8 patients. La durée moyenne de la perfusion été de 81 minutes. Le melphalan a été administré donné par doses filées. La dose totale a été de 1.39 mg/kg. La température maximale a été de 41.5°C en moyenne. Il y a eu 20 complications majeures (19%). Il y a eu un décès postopératoire du à un infarctus du myocarde et un patient a nécessité une amputation. La durée moyenne de suivi a été de 2.65 ans (extrêmes 1–27 ans). Une réponse complète a été obtenue chez 34 (40%) patients, une réponse partielle chez 36 (42%) et pas de réponse chez 11 (13%) des patients. La réponse n'a pu être évaluée chez quatre patients. La survie actuarielle pour le groupe dans son ensemble a été de 45% (stade I: 80%; stade II: 75%; stade IIIa: 44%, stade IIIab: 23% et stade IIIb: 56%). Les déterminants les plus pertinents de réponse à la perfusion ont été le stade de maladie (p=0.00005) et l'état ganglionnaire (p=0.03). Le déterminant majeur de survie a été la réponse compète à la perfusion (p=0.0012). Les maladies les moins évoluées ont mieux répondu à la perfusion et sont associées à une survie prolongée après perfusion.
    Abstract: Resumen En el Hospital Royal Brisbane hemos revisado la respuesta y la supervivencia en pacientes con melanoma maligno tratados con perfusión hipertérmica aislada con Melphalan, con el objeto de identificar los factores determinantes de la respuesta al tratamiento. Se obtuvo la información mediante revisión de las historias clínicas y contacto directo con el paciente. La respuesta fue valorada mediante observación clínica y medición de los nódulos. Se calculó la sobrevida desde el momento de la perfusión hasta la muerte del paciente. Desde 1965, 85 pacientes (38 masculinos, 47 femeninos) fueron sometidos a 109 perfusiones de extremidad (102 pierna, 7 brazo). La edad media fue 58 años (rango 32–82), y el estadío de la enfermedad (M.D. Anderson) fue: estado I-5 pacientes, estado II-8, estado IIIa-41, estado IIIab-25 y estado IIIb-8. El tiempo medio de perfusión fue 81 minutos. El Melphalan fue administrado en dosis divididas, con una dosis media total de 1.39 mg/kg. La temperatura media máxima de la extremidad fue 41.5°C. Se presentaron complicaciones mayores en 20 perfusiones (19%) y 1 muerte por infarto miocárdico; 1 paciente requirió amputación. El seguimiento medio fue 2.65 años (rango 1–27 años). Los dos determinantes más significativos de la respuesta a la perfusión fueron el estadío de la enfermedad (p=0.00005) y el estado de los ganglios (p=0.03). El mayor determinante de supervivencia fue la respuesta completa a la perfusión (p=0.0012). La enfermedad en estado menos avanzado responde mejor a la perfusión y se asocia con mayor supervivencia luego de la perfusión.
    Notes: Abstract Response to and survival after hyperthermic isolated limb perfusion (HILP) with melphalan in patients with malignant melanoma at the Royal Brisbane Hospital were reviewed in order to assess the major determinants of response to treatment. Data were collected by clinical chart review and direct patient contact. Response was assessed by clinical observation and measurement of nodules. Survival was calculated from the date of perfusion to the date of patient death. Since 1965 there have been 85 patients (38 men, 47 women) who have undergone 109 limb (102 leg, 7 arm) perfusions. The mean age was 58 years (range 32–82 years). The number of patients per disease stage (M.D. Anderson classification) was as follows: stage I, 5 patients; stage II, 8 patients; stage IIIa, 41 patients; stage IIIab, 25 patients; and stage IIIb, 8 patients. The mean duration of perfusion was 81 minutes. Melphalan was given in divided doses, with a mean total dose 1.39 mg/kg. The mean maximal limb temperature was 41.5°C. Major complications occurred during 20 perfusions (18%). There was one postoperative death from myocardial infarction, and one patient required amputation. Median patient follow-up was 2.65 years (range 1–27 years). A complete response was noted in 34 patients (40%), a partial response in 36 patients (42%), and no response in 11 patients (13%). response could not be assessed in four patients. The actuarial 5-year survival for the whole group was 45%: stage I, 80%; stage II, 75%; stage IIIa, 44%; stage IIIab, 23%; stage IIIb, 56%. The two significant determinants of response to perfusion were disease stage (p=0.00005) and nodal status (p=0.03). The major determinant of survival was a complete perfusion response (p=0.0012). Less advanced disease responds better to perfusion and is associated with increased survival after perfusion.

    Notes: Summary The mutationsvestigial (vg; recessive) andUltravestigial (vg U; dominant) ofDrosophila melanogaster give rise to identical mutant adult phenotypes in which much of the cases this results from cell death in the presumptive wing margin of the wing disc in the third larval instar, but the process of cell degeneration is quite different in the two mutants. Invg cell death occurs continuously throughout the third larval instar, while invg U it occurs only in the early third instar. Cells fragment and some of the fragments condense, becoming electron dense (“apoptosis”). Both condensed and ultrastructurally normal cell fragments are extruded to the basal side of thevg disc epithelium. They accumulate under the basal lamina in the wing pouch area until they are phagocytosed by blood cells entering the wing pouch during the six hours following pupariation. Fragments are not extruded from thevg U epithelium but are apparently phagocytosed by neighboring epithelial cells. The basal lamina undergoes mophological changes following pupariation and is phagocytosed by blood cells in both wild-type andvestigial, but investigial the degenerated cell fragments are also engulfed by the same blood cells.
