    Notes: Summary The reconstruction of a posterior canal wall was performed in radical cavities. Two series of operations were studied, using either autologous bone grafts or homologous cartilage.     Notes: Summary The recording of antidromic nerve action potentials of the facial nerve—as obtained from healthy anaesthetized cats—has been reported on the 45th meeting at Bad Reichenhall, May 1974. The method has now been used in humans. The NAP of the extratemporal nerve segment was first studied in cases of parotidectomy. The antidromic NAP was then recorded from healthy and paretic nerves in their mastoidal segment as well as from the internal auditory meatus. Technical difficulties and their management will be discussed.

    Notes: Summary The facial nerve of the cat has been electrically stimulated below the stylomastoid foramen. Antidromic action potentials were recorded from the facial nerve and, additionally, by placing electrodes in the tympanon and in the external meatus. The electronic evaluation of the potentials by using an averager (Linc 8) made it possible to identify an action potential of 300–400 μs latency, which was correlated to the electrical stimulus.

    Notes: Summary One hundred forty-nine patients 65–94 years old had a high frequency loss in the pure tone audiogram corresponding to data in literature. There was no significant difference between left and right. The SISI-score and Langenbeck-audiogram showed a cochlear type in 80%. The tests for central speech perception showed correlation to age in the following sequence: Dichotic discrimination, followed by discrimination of one — syllable — words, then hearing loss for words with four syllables, then binaural summation. Other parameters like serum-cholesterin and cardio-vascular function had a correlation to age much less pronounced than the audiological tests.

    Description / Table of Contents: Zusammenfassung Bei 41 Patienten mit peripheren Fazialisparesen und sieben gesunden Kontrollpersonen wurden nach Pilocarpinstimulation speichelchemische Analysen im Seitenvergleich durchgeführt. Dabei zeigten sich die Elektrolyte Natrium und Calcium sowie das Ferment Amylase im Speichel der paretischen Seite erhöht. Die Natriumkonzentrationserhöhungen werden nach entsprechenden Modellvorstellungen als Störung der physiologischen Natriumrückresorption (zelluläre Natriumpumpe) des Drüsengangepithels interpretiert. Die Natriumkonzentrationserhöhungen waren abhängig vom Schweregrad der Parese und bildeten sich während der Ausheilung zurück. Es liegen noch nicht genügend Befunde im Frühstadium einer Fazialisparese vor, um die Zuverlässigkeit dieses Parameters zur Fazialisdiagnostik beurteilen zu können. Für Kalium und Protein wurden im Submandibularisspeichel der paretischen und gesunden Seite keine Unterschiede festgestellt. Die Elektrolyttransportstörungen des Submandibularisgangsystems bei Fazialisparesen wurden als gut untersuchbares Modell einer Parasympathikusläsion aufgefaßt. Dies erscheint für die einer biochemischen Untersuchung schwer zugänglichen Labyrinthosen von Interesse, bei denen ursächlich Störungen des vegetativen Zusammenspiels diskutiert werden. Bei diesen könnte eine erhöhte Natriumkonzentration im Corti-Organ bedingt durch eine pathologische Natriumrückresorption einige Symptome ungeklärter Labyrinthosen verständlicher machen.
    Notes: Summary In 41 patients with facial paralysis and seven healthy test subjects, chemical analyses were carried out on saliva obtained after stimulation with pilocarpine, and a comparison made between the two sides. It was established that in the saliva of the involved side, the electrolytes sodium and calcium, and also the enzyme amylase, were elevated. In accordance with appropriate theoretical models, the elevated sodium concentrations are interpreted as being due to disorders in the physiological re-absoprtion (cellular sodium pump) in the epithelium of the glandular duct. The elevated concentrations of sodium depended upon the severity of the paresis and reverted to normal levels during the recovery phase. At present, the available findings in the early stage of a facial paralysis are insufficient to assess the reliability of this parameter in the diagnosis of the condition. In the case of potassium and protein, no differences between the involved and the normal side were found in submandibular saliva. The disturbances in the transport of electrolytes in the submandibular duct system in patients with facial paralysis were considered a good model for the simple detection of a parasympathetic lesion. This would appear of interest with respect to the biochemical examination of labyrinthine diseases that possibly result from disturbances of the vegetative coordination. In the case of such diseases, an elevatioin the sodium concentration in the organ of Corti, due to a pathological re-absorption of sodium, might make it easier to understand some of the symptoms accompanying labyrinthine disease of unexplained aetiology.

    Description / Table of Contents: Zusammenfassung Ein relativ wenig aufwendiges Verfahren zur Gewinnung der statistischen Verteilung von Potentialdauern (Dauerhistogramm) aus längeren elektromyographischen Registrierungen wird beschrieben. Dabei wird die verwendete Amplitudenschwelle so gelegt, daß im interessierenden Zeitbereich das gleichzeitig registrierte Intervallhistogramm (statistische Verteilung der Intervalle) keine nennenswerten Anteile aufweist. Damit ist sichergestellt, daß im statistischen Mittel die Dauern von Potentialspitzen gemessen werden, welche über einem durch schnelle Störspannungsschwankungen verschiedener Ursache hervorgerufenen Spannungspegel liegen. Es zeigt sich, daß das Verfahren auch bei stärkeren Überlagerungen (Interferenzmuster) brauchbare Aussagen beim Vergleich zwischen klinisch gesunder und geschädigter Motorik, insbesondere im Dauerhistogramm liefert.
    Notes: Summary A relatively simple method for obtaining the statistical distribution of the duration of spikes (duration histogram) from protracted electromyographic records is described. A criterion for differentiating spike-like oscillations from background noise and rapid oscillations of different origin (interference pattern) was established empirically through the additional use of the interval histogram. The results demonstrate that especially on the basis of duration histogram parathetic motoric systems could be distinguished from those which function normally.

    Description / Table of Contents: Zusammenfassung An 15 Patienten im Alter zwischen 36 und 74 Jahren, bei denen während einer Halsoperation die A.carotis communis freigelegt war, wurden simultan der intraarterielle Karotisdruck (P) mit einem Katheterspitzenmanometer und an derselben Stelle der arterielle Außendurchmesser (D) mit einem berührungsfreien photoelektrischen Verfahren registriert. Die relativen Durchmesseränderungen, bezogen auf den enddiastolischen Außendurchmesser, betrugen im Mittel 5,6% bei Druckamplituden von etwa 50 mm Hg. Die durch die Viskoelastizität der Arterienwand bedingten Hystereseschleifen der pulsatorischen P-D-Beziehungen wurden mit Hilfe eines iterativen Verfahrens unter Verwendung eines Digitalrechners eliminiert, wodurch eine Trennung der rein elastischen und der rein viskösen Komponenten der P-D-Beziehung ermöglicht wurde. Aus den rein elastischen P-D-Beziehungen wurden der tangentiale Elastizitätsmodul und die Pulswellengeschwindigkeit als Funktionen des Außendurchmessers berechnet und in normierter Form, d. h. bezogen auf den jeweiligen enddiastolischen Wert, dargestellt. Mit wachsendem Durchmesser stieg der Elastizitätsmodul während jedes Pulszyklus auf das 1,2- bis 3,5fache des enddiastolischen Wertes an. Entsprechend nahm die Pulswellengeschwindigkeit auf das 1,1- bis 1,9fache des enddiastolischen Wertes zu.
    Notes: Summary In exposed common carotid arteries of 15 patients (36–74 years) undergoing neck surgery, the intra-arterial pressure (P) was recorded by means of a catheter-tip manometer and, at the same site, the external diameter (D) by means of a contactfree photoelectric device. On the average, the pulsatile diameter changes were 5.6% of the end-diastolic diameter at pulse pressures of about 50 mm Hg. Due to viscoelasticity, the P-D diagrams exhibited hysteresis loops. Using the criterion of loop elimination, an iterative procedure was applied which permitted, by the use of an appropriate computer program, the separation of the purely elastic and the purely viscous components of the P-D relationships. In all cases, the purely elastic P-D curves markedly deviated from linearity. The tangential elastic modulus (Et) and the pulse wave velocity (c) calculated from these curves were normalized by dividing these quantities by the respective end-diastolic values and plotted against the normalized external diameters. During each pulse cycle, Et increased, with increasing diameter, by a factor between 1.2 and 3.5, while c increased by a factor between 1.1 and 1.9 with reference to the respective end-diastolic values.

    Notes: Summary Recordings of antidromic NAPs of the facial nerve from the external acustical meatus are supposed to give a quick account in conduction disturbances, especially in cases where the disturbance lies within the mastoid. The main obstacle in these measurements results from the short geometrical distance between the stimulus and recording electrodes, and from the relatively high voltages necessary for transdermal stimulation, respectively. A system consisting of a pre-amplifier, stimulus- and trend-suppressor and a main amplifier together which a movable bipolar stimulus electrode is used with the aim to rule out the large stimulus artifacts.
