    Description / Table of Contents: Résumé Ce travail a pour but la détection des métastases occultes chez les femmes porteuses d'un cancer du sein “débutant” sans signe de dissémination. Cinquante malades ont été soumises à une batterie d'examens comprenant la scintigraphie osseuse et hépatique, la mesure de l'excrétion urinaire d'hydroxyproline, la détermination du taux sérique des phosphatases alcalines d'origine hépatique et osseuse ainsi que du taux sérique de la gamma-glutamyl-transpeptidase. Chez 44% des malades, nous avons trouvé une ou plusieurs anomalies suggérant des métastases occultes squelettiques et/ou hépatiques. Il n'y avait pas de corrélation entre la positivité de ces tests et l'envahissement néoplasique des ganglions pectoraux (trouvé dans 42% des cas). Quoique la période d'observation soit courte, il semble exister une corrélation entre la positivité des tests et la récidive de la maladie. Ces examens préopératoires et l'étude histologique des ganglions pectoraux et axillaires permettent de préciser le stade de la maladie.
    Notes: Abstract In an effort to detect occult metastases in women with “early” breast cancer and no evidence of metastatic disease by conventional criteria, 50 women with “early” breast cancer were subjected to a battery of tests that included skeletal scintiscanning, liver scintiscanning, and measurements of urinary hydroxyproline, serum alkaline phosphatase of bone origin, serum alkaline phosphatase of liver origin and serum gamma glutamyl transpeptidase. One or more abnormalities suggestive of occult metastatic disease in the skeleton and/or the liver were found in 44% of patients. There was no correlation between positive test results and the finding of positive pectoral lymph nodes (found in 42% of patients). Short-term follow-up suggests a correlation between positive test results and development of recurrent disease. It appears that the performance of these tests preoperatively and the histological evaluation of pectoral and axillary lymph nodes are complementary approaches to the staging of breast cancer.

    Description / Table of Contents: Résumé De 1967 à 1973, 243 patients atteintes de cancer du sein classifié cliniquement comme stades I et II furent réparties au hasard en deux groupes divisés en un certain nombre de sousgroupes. Les deux groupes furent traités, l'un par mastectomie simple (avec biopsie ganglionnaire pectorale), l'autre par mastectomie radicale. Toutes les patientes ayant des ganglions envahis à l'examen histologique reÇurent un traitement postopératoire complémentaire: radiothérapie locale du creux axillaire pour les mastectomies simple; radiothérapie radicale du creux axillaire, de la paroi thoracique, de la région susclaviculaire et de la chaÎne mammaire interne pour les mastectomies radicales. Au 30 mars 1976, 166 des 243 patientes avaient été suivies pendant 5 ans. Quel que soit le traitement, nous n'avons pas observé de variations significatives pour la survie, la dissémination cancéreuse ou le délai d'apparition des métastases. La seule différence significative dans cet essai concerne les patientes ayant des ganglions envahis. Dans ce groupe, la mastectomie radicale complétée par une radiothérapie radicale est suivie d'un nombre de récidives locales plus faible qu'après traitement conservateur. L'étude de la morbidité chez 85 malades a montré que, lorsque les ganglions ne sont pas envahis, la fréquence des complications est moindre après mastectomie simple qu'après mastectomie radicale.
    Notes: Abstract Between 1967 and 1973, 243 patients with breast cancer classified clinically as stages I and II were assigned by random sample to 2 major treatment groups and a number of subgroups. The 2 groups were treated by either simple mastectomy (with pectoral lymph node biopsy) or radical mastectomy. All patients with positive lymph nodes on histological examination received supplementary postoperative treatment which, in the simple mastectomy group, consisted of local radiotherapy to the axilla and, in the radical mastectomy group, consisted of radical radiotherapy to the axilla, chest wall, supraclavicular region and internal mammary region. By March 30, 1976, 166 of the 243 patients had completed 5 years of follow-up observation. There was no significant difference in survival rate, duration of life, incidence of disseminated cancer, or time of appearance of metastases between the conservative and radical treatment groups. The only significant difference observed in the trial occurred in patients with positive lymph nodes in whom treatment by radical mastectomy plus radical radiotherapy was associated with a significantly lower incidence of local cancer recurrence than was conservative treatment. Assessment of morbidity in 85 patients showed that patients with negative lymph nodes had significantly less morbidity following simple mastectomy than after radical mastectomy.

    Notes: The Mass spectra of some 1,3-cyclopentanediones and 1,2,4-cyclopentanetriones, related to compounds encountered in the chemistry of hops bitter constituents have been determined. Methyl substituted compounds mostly show ring degradation, while for longer alkyl groups and acyl groups, fragmentations in the side chain carry most of the total ionization. When both alkyl (〉 2C) and acyl substituents are present, fragmentations in the alkyl group always occur before those in the acyl side chain. The observed degradation processes are useful for the interpretation of the spectra of hop bitter acids and derived products.
