    Notes: In patients with respiratory symptoms, several studies have provided data supporting the hypothesis that there is a causal relationship between allergen exposure and variations in bronchial reactivity. In order to determine if this relationship holds when atopy is defined only on the basis of positive skin tests to common allergens, we compared bronchial reactivity in a group of twelve healthy subjects with positive skin tests and twenty-eight healthy subjects with negative tests. The two groups were comparable in terms of gender, age, smoking habits and family history of atopic diseases. The slopes of the dose-response curves, using airway conductance as an index of response, were similar in the two groups. Thus, in this healthy group of subjects, there was no relationship between skin and bronchial reactivity. It can be hypothesized that, if genetic factors determine bronchial reactivity, such reactivity might not be revealed until skin-test positive subjects have received repeated bronchial stimulation through inhalant allergens.

    Notes: A prospective study was made in France to determine the frequency, nature, causes and consequences of systemic reactions occurring during specific immunotherapy. One hundred and fifty five reactions were recorded in 151, 997, injections given to 19,739 patients; a percentage of 0.1. It was higher with pollen extracts (0.2%) and practically nil with other extracts (house dust, Dermatophagoides, insect body, bacteria). Asthma, spasmodic rhinitis and urticaria were the most frequent, 80% of systemic reactions. In 59% no explanation could be found. The main known causes of adverse reactions were excessive doses of antigen, improper timing of treatment or incorrect technique of injection. After appropriate treatment the immunotherapy was continued in nearly 90% of the cases. Specific immunotherapy with the majority of extracts now being used, namely adsorbed extracts, is not dangerous but it must be precisely and cautiously done because errors are responsible for nearly 50% of the recorded systemic reactions.

    Notes: Twenty-one patients, who had previously experienced an anaphylactic reaction to suxamethonium during general anaesthesia, were selected for this study. Initially, skin tests with muscle relaxants were carried out in the twenty-one patients, detection of specific anti-choline IgE in nineteen, and leucocyte histamine release in seventeen. These three tests were then repeated between 1 year and 4 years after the initial evaluation. In the majority of patients, sensitization to the muscle relaxants persisted for more than 1 year after the anaphylactic reaction. Only three patients out of twenty-one (4%) had negative skin tests when retested 1–4 years later. A reduction in leucocyte histamine release was noticed in one of the seventeen retested patients (6%). Modifications of anti-choline IgE were observed in five of nineteen patients (26%). The persistence of sensitization to suxamethonium may result from repeated stimulation by occasional contacts with quaternary ammonium compounds. This study demonstrates the reliability of skin tests, leucocyte histamine release and detection of anti-choline IgE to diagnose allergic reactions to suxamethonium, even when they are performed a long time after the initial anaphylactic reaction.

    Notes: The effect of Paragerm spray from either a hand pressurized can or from a general room mist device (MCR device) on mattress mite populations was compared with the effect of a control spray. After 1 month a reduction in mite population due to the cleaning procedures was evident in the control treated mattresses; Paragerm treated mattresses had significantly fewer mites, notably those treated with the aerosol can spray, nine out of fifteen of which were mite free. Dissection of the mattresses showed that some mites survive and they could form the nucleus from which reinfestation could occur.

    Notes: Attempts to reduce the growth of house dust pyroglyphid mites by vacuum cleaning, and modifications of temperature and humidity are useful but not sufficient. Their absence in dust from hospital beds which are disinfected led the authors to test PARAGERM AK, a non toxic bactericidal and fungicidal spray. This proved highly effective as an acaricide, killing off cultures of Dermatophagoides spp. and other mites with acceptable arid easily produced concentrations, thus providing a new way of combating the house dust allergy mite.

    Notes: Phytohemagglutinin (PHA) was capable of inducing non-cytotoxic histamine release from human leucocytes. In the presence of deuterium oxide (D2O), PHA caused significantly greater histamine release. Dibutyryl cyclic AMP id-cAMP) could enhance the histamine release in the presence of D2O although it was an inhibitor of the release if used alone. However, a β agonist, isoproterenol, which increases intracellular level of cAMP was inhibitory with or without D2O. These data ask the question about dual effect of cAMP and suggest the possibility of different pools of cAMP in the target cells.

    Notes: Knowledge of specific IgE bound on immunosorbent as assayed by FLAST is useful but cannot give a complete description of the biological states of patients concerning IgE antibodies Using a computer program based on a least squares error method and knowing the amount of bound specific IgE antibodies to grass pollen antigens in a serum at each of the four tested dilutions it is possible to calculate the absolute value of specific IgE antibody level, die relative value of affinity constant and the relative proportion of IgE antibodies to the total amount of specific (IgE plus non-IgE) antibodies. It is found that specific IgE antibodies account for high percentages of the total IgE protein (mean of 20%. High differences in affinity constant and in the proportion of IgE antibodies to the total amount of antibodies could be found between sera. Severity of grass pollen allergy could be related to die absolute level of IgE antibody, but no definite conclusion could be made on the importance of affinity constant and the proportion of IgE antibodies to the total amount of antibodies.

    Notes: Asthma deaths are uncommon, but have recently increased in some countries due to problems in the management of the disease. Morbidity rates show large variations, which can be attributed to differences in defining the disease, but also to genuine variations, with a trend towards less asthma in northern Europe. It has been suggested that allergic diseases as a whole, and asthma in particular, may exhibit an upward secular trend. Risk factors include a genetic background and environmental triggering factors. The importance of genetic factors is illustrated by family studies and by extreme prevalence rates observed in some communities. Environmental factors include rapid air pollution variations which act as a trigger for asthma attacks. However, at levels currently prevailing in western Europe, air pollutants do not induce a higher incidence of asthma. Altitude generates a gradual decrease in Dermatophagoides, thus explaining both the clinical improvement in asthmatics living in altitude and a lower prevalence of asthma in populations born and living there. Among the other aero-allergens, grass pollens plays a major role in spring, elicitating asthma attacks. Some natural allergens transformed by man (castor bean, soja) can be responsible for asthma epidemics.
