    Notes: Abstract We describe a 53-year-old Japanese woman with absence of the posterior arch of the atlas. Computed tomography revealed that the residual posterior fragment which was displaced to the right side does not correspond to a rare ossification center in the midline and that there was no clear mass of the posterior arch except for a small fragment. Moreover, the midline center was not demonstrated as a soft tissue mass. Easy tilting of the posterior fragment on flexion-extension movement suggests that the cartilaginous arch was not present between the lateral masses and the posterior fragment of the atlas. These observations support the hypothesis that hypoplastic development of the arch plays a major role in the pathogenesis of this anomaly rather than incomplete ossification. Abnormal segmentation of the vertebrae may be implicated in the etiology of this condition, since fusion of the anterior vertebral masses between the C5-6 levels, as shown in Klippel-Feil syndrome, is also observed.

    Notes: Abstract We report a case of an aneurysm of a persistent sciatic artery which caused buttock pain. Preoperative diagnosis is very difficult. However, awareness of the presence of this rare embryonic abnormality is important, especially in elderly persons with atherosclerotic changes. Sagittal magnetic resonance imaging (MRI) was very useful in reaching this diagnosis.

    Notes: Summary Internal pressure and oxygen tension were measured in 24 patients with bone tumors and tumorous conditions. High internal pressures were observed in most of the rapidly growing lesions. The internal pressures of slowly growing and non-growing lesions were not different from those of normal bone marrow. Oxygen tension was commonly higher in the rapidly growing lesions than in simulataneously obtained peripheral venous samples from the same patients. The oxygen tension of the fluid from simple bone cysts was lower than that in venous samples from the same patients. There is a good correlation between internal pressure and the growth rate of the bone lesions in this study. It is suggested that internal pressure and oxygen tension of the lesions demonstrated the degree of blood supply to the bone tumors and tumorous conditions.

    Description / Table of Contents: Zusammenfassung Es wird über einen Fall von multiplen Knochenzysten berichtet, die sowohl in langen und kurzen Röhrenknochen als auch in platten Knochen lokalisiert waren. Simultane Messungen des Zystenbinnendruckes und der Sauerstoffspannung der Zystenflüssigkeit ergaben ähnliche Werte wie bei typischen solitären Knochenzysten. Kürettage und Knochentransplantation waren bei der Kalkaneuszyste erfolgreich, während die Steroidinjektion in die Metakarpalzyste keinen Effekt hatte. Multiples Anbohren der Ulna- und Radiuszysten war in gleicher Weise effektiv. Diese Resultate lassen vermuten, daß auch in der Ätiologie der multiplen Zysten möglicherweise die venöse Abbflußbehinderung eine wichtige Rolle spielt, wie dies für typische solitäre Knochenzysten gezeigt werden konnte. Es kann jedoch die Möglichkeit nicht ausgeschlossen werden, daß einfache Knochenzysten, solitär oder multipel, so etwas wie eine sekundäre Form anderer Erkrankungen darstellen oder daß es sich bei den einfachen Knochenzysten doch um eine heterogene Gruppe von Veränderungen handelt.
    Notes: Summary We report a case of multiple, simple bone cysts in long tubular, short tubular, and flat bones. The internal pressure of the cysts and the oxygen tension of cyst fluid were measured simultaneously. These values were similar to those of typical solitary cysts in other patients. Curettage and bone graft onto the calcaneal cyst were effective, although steroid injection therapy into the metacarpal cyst had no effect. Piercing the ulnar and radial cysts a number of times was uniformly effective. These results may suggest that venous obstruction of the lesions play an important role in the etiology of multiple simple cysts, as shown in typical bone cysts. However, we could not exclude the possibility that simple bone cysts, solitary or multiple, represent some kind of secondary forms of other disorders, or that a simple bone cyst is a heterogeneous entity.

    Notes: Summary Osteosarcoma of the thoracic spine developed in a 15-year-old Japanese boy, After his first admission with paralysis, multiple skeletal metastases were demonstrated in the absence of pulmonary metastasis. This rare condition may possibly be considered as a unicentric osteosarcoma with bone metastases, since there were no precursor lesions or history of exposure to radio-active materials or chemical agents. These multiple lesions may be an example of so-called “organ-specific metastasis,” although this “nonstochastic” process is rare.

    Notes: Summary Curettage without bone graft was performed in 17 patients with benign bone tumors and tumor-like lesions. New bone formation with uniformly increased radio-density appeared in serial plain radiographs within 3 months after the operation. The average period before full weight could be borne on the lower extremities was 14 weeks. Computed tomography revealed that the central part of the bone lesions persisted without bone formation. The thickening of cortical bone was predominant. These data indicate that enough mechanical strength for daily activity will be recovered by bone within 4 months after curettage without any filler, although remodeling continues for over a year. It is suggested that bone graft and implantation of “biomaterials” are not necessary in patients, especially younger ones, with benign bone tumors or tumorous conditions.

    Notes: Summary Levels of serum tumor markers including tissue polypeptide antigen (TPA), CA 15-3, CA 19-9, squamous cell carcinoma antigen, carcinoembryonic antigen, α-fetoprotein, and PAP were measured in 26 patients with bone metastasis and in 9 patients with primary bone tumors. More than one markers was elevated in 19 of the 26 patients with bone metastasis, although there was no elevation of the markers in 3 patients with renal cell carcinoma. TPA was the most sensitive marker in the diagnosis of metastasis. CA 15-3 was also a sensitive marker in this study, since metastasis from breast carcinoma may be the most common of all metastases in the skeleton. On the other hand, α-fetoprotein was uniformly unresponsive except in one case of gastric cancer. Combinations of markers are valuable for metastasis screening tests. No definite correlations were found between the markers in this study. On the other hand, there was a slight elevation of the markers observed in two of the nine patients with primary bone lesions. Serum tumor markers are useful in the diagnosis of bone metastasis to differentiate it from primary bone lesions. Especially in solitary bone lesions, serum markers may be the only way to make a differential diagnosis between the two.

    Description / Table of Contents: Résumé Présentation de cinq cas d'irrégularité apophysaire de la tubérosité ischiatique. Chez quatre de ces patients, la densité de la zone radiotransparente telle qu'on la voit sur les radiographies standard avait augmenté en l'espace de quelques mois à la suite d'un traumatisme mineur. La tomodensitométrie sérielle révéla un os nouvellement formé au travers de la plaque apophysaire élargie, bien que l'apophyse elle-même ne montrât aucune modification ni de densité ni de forme. Dans ces cas le décollement apophysaire s'est spontanément réparé et remodelé en une année, sauf chez trois patients chez qui un fragment osseux non vascularisé est apparu après dix mois, avec incorporation encore visible 3 ans plus tard sur les radiographies. Dans le dernier cas, la tomodensitométrie a montré que le point d'ossification apophysaire était plus petit que celui du côté opposé. En outre, il n'y avait aucune altération sur les tomodensitométries. Ces observations suggèrent que dans les quatre cas l'irrégularité apophysaire, appelée maladie de Kremser, pourrait être induite par une fracture par arrachement ou par un décollement épiphysaire. Cependant, dans le cinquième cas, une anomalie du point d'ossification a montré une irrégularité semblable à celle que l'on observe sur les radiographies dans la maladie de Kremser.
    Notes: Summary We report five cases of irregularity of the apophysis of the ischial tuberosity (Kremser's disease). In four, plain radiographs showed that the density of the radiolucent area increased within several months of a minor injury. Serial computerised tomography demonstrated new bone formation across the widened apophyseal plate, although there was no change in the shape or density of the apophysis itself. The separation repaired and remodelled spontaneously within a year. In one case, CT showed that the apophyseal ossification centre was smaller than that on the opposite side. Our findings showed that in four cases the irregularity of the apophysis of the ischial tuberosity may have been caused by an avulsion fracture, but the appearance in the fifth case was a normal variant of the ossification centre.

    Description / Table of Contents: Résumé Nous présentons un cas rare de kyste osseux simple chez un patient porteur d'une lésion ostéoblastique découverte un an et demi plus tôt. Un curetage associé à des perforations multiples a été efficace, aussi bien sur le plan clinique que radiologique, comme il est habituel dans les kystes osseux typiques. En nous basant sur ce cas, et sur des cas analogues déjà publiés, nous avançons l'hypothèse qu'une augmentation de la vascularisation régionale associée à la formation d'os entraîne un déséquilibre hydrodynamique, celui-ci jouant un rôle dans le déclenchement d'une obstruction veineuse aboutissant à la formation d'un kyste.
    Notes: Summary We describe a simple bone cyst in a patient who had an osteoblastic lesion which had been discovered 1 1/2 years earlier. Curettage and drilling proved effective treatment. We suggest that increased regional blood flow, in association with bone formation, produces a hydrodynamic disorder followed by venous obstruction leading to the formation of a bone cyst.
