    Notes: MAM-3 and MAM-6 antigens were detected immunohistochemically in 34 cases of adenoid cystic carcinomas (ACC) of the salivary glands and these patterns were compared to these of epithelial membrane antigen (EM A) and laminin. ACC was histologically divided into three types; the cribriform pattern, the tubular and trabecular pattern, and the solid cluster pattern. Immunostaining of EMA and MAM-6 antigen had a similar distributions in the luminal borders of luminal tumor cells, whereas the MAM-3 antigen was slight or negative in luminal borders. Myoepithelial derived tumor cells of ACC accompanying hyaline stroma demonstrated positive staining for the MAM-6 antigen (whole cell positive type), and luminal tumor cells of microcysts showed strong staining for the MAM-3 antigen. Laminin staining was confined to the basement membrane and surface borders in pseudocyst cavities. In salivary gland ACC, laminin staining can be used as a marker of pseudocyst surfaces and immunostaining of EMA and the MAM-6 antigen as a marker of luminal borders of cyst. These two histo-chemical markers were useful for discriminating pseudocyst and cyst.

    Notes: A total of 31 patients surgically treated for salivary mucoepidermoid tumor had a comprehensive follow-up. Two groups were classified as: 16 with favorable outcome (no recurrence or late recurrences at 5 yr postoperatively and still alive; 15 with poor outcome (local recurrences before 5 yr, metatases, lethal evolution in 11 cases). We evaluated nucleolar organizer in every case, using AgNOR count. The AgNOR count seemed better than histologic criteria for establishing the prognosis of mucoepidermoid tumors. It correlated significantly with the clinical course: a high count was principally found in lethal forms, low counts (less than 1.8) were always detected in patients with a good outcome.

    Notes: We have examined conjunctival biopsies from nine primary and two secondary cases of Sjögren's syndrome. In all cases histopathological examination revealed metaplasia of the conjunctival epithelium associated with a reduced number of goblet cells and a polymorphic inflammatory cell infiltrate of the stroma. Using monoclonal antibodies with texture analysis of stained preparations we have shown that in all cases me lymphocytic infiltrate was predominantly T-cell but with substantial numbers of IgA and IgG secretory B-cells in three cases. The number of Langerhans cells present was reduced in biopsies showing the most marked degree of epithelial metaplasia.

    Description / Table of Contents: Résumé Les auteurs ont effectué une étude des foramen intervertébraux sur des préparations anatomiques de rachis indemne de toutes lésions pathologiques dégénératives. Ils ont étudié des variations de forme et de taille de 38 foramen intervertébraux au cours des mouvements de flexion-extension, avant et après affaissement discal isolé. La technique utilisêe a pris en compte les reliefs osseux, le bord antérieur du disque et les structures capsuloligamentaires postérieures. En flexion, tous les diamètres du foramen sont maximum. En extension extrême, tous les diamètres diminuent de façon significative: les pédicules se rapprochent, le disque fait saillie en arrière et le ligament jaune est poussé en avant par le processus articulaire supérieur de la vertèbre sous-jacente. Un affaissement discal de 4 mm réduit tous les diamètres du foramen. Il apparaît donc qu'après affaissement discal, l'hyperextension lombaire rétrécit suffisamment le diamètre du foramen pour menacer la racine nerveuse.
    Notes: Summary The authors undertook a morphometric study of the intervertebral foramina in anatomic specimens of spines unaffected by degenerative lesions. They studied the variations in shape and size of 38 intervertebral foramina during flexion-extension movements and following an isolated disc collapse. The cast technique takes into account the bony prominences, the bulge of the disc anteriorly and of the capsulo-ligamentous structures posteriorly. In flexion, all the diameters of the foramina are maximal. In full extension all the diameters decrease significantly: the pedicles come closer together, the disc bulges posteriorly and the ligamentum flavum is pushed forward by the superior articular process of the underlying vertebra. A disc collapse of 4 mm decreases all the diameters, and in this case lumbar extension results in a sufficient decrease of foraminal diameter to threaten the nerve root.

    Description / Table of Contents: Résumé L'étude anatomique de 6 sujets leucémiques, décédés entre le 7ème et le 45ème jours du syndrome secondaire consécutif à une greffe allogénique de moelle osseuse après irradiation totale, a permis de constater, outre des lésions caractéristiques des é pithéliums (peau, intestin) des modifications particulières dans les aires lymphoïdes et la moelle osseuse. Les ganglions et la rate étaient, en effet, le siège d'un conflit immunologique (cellules hyperbasophiles et lymphonécrose) dont les conséquences aboutissaient, dans la rate, à des altérations artériolaires et à des foyers ischémiques. Dans la moelle osseuse la fréquence des micro-infarctus sous l'influence, là encore, d'un «conflit» entre éléments lymphoïdes et cellules endothéliales paraissait responsable d'une relative lenteur dans la restauration hémopoïétique. Par ailleurs, les formes les plus sévères du syndrome s'accompagnaient de certaines déterminations viscérales (poumons, cœur, système nerveux) dont les conséquences séméiologiques et l'influence sur l'évolution léthale paraissent peu contestables.
    Notes: Summary The anatomical study is reported of six patients with leukemia who died between the seventh and the forty-fifth day after total body irradiation and allogenic bone marrow graft. During the secondary disease characteristic changes developed in epithelia (skin, jejuno-ileum) and particularly in lymphoid organs and bone marrow. The lymph nodes and the spleen manifested an immunological reaction (hyperbasophilic cells, lymphocytonecrosis) the effects of which caused arteriolar injury and ischemic lesions. In the bone marrow, micro-infarcts frequently developed from the “conflict” between lymphoid and endothelial cells and seemed to be the cause of a relative slow hematopoietic restoration. Moreover, in the most serious form of the syndrome we observed visceral injury (lungs, heart, nervous system) as attested by the symptoms, signs and the lethal effects.

    Notes: Summary The first stages of the atherosclerotic lesions induced in the rabbit aorta by adrenalin- thyroxine, with (or without) an hypercholesterolic diet, have been studied on the 6th, 13th, and 22nd day. Major parietal changes, barely demonstrable with light microscopic standard techniques, were detected, as soon as the 6th day, by electronmicroscopy and histoenzymology. These changes were probably due solely to the hormonal treatment. In the early stage, they consisted of some fragmentations of the elastic sheets and some smooth muscle cell lysis. The oxidative and ATPase activities were greatly reduced. The increased lysosomal hydrolase activities of the outer layers of the vessel could be related to the increased vascular permeability and consecutive increased perfusion gradient induced by these lesions. Later, an obvious repair process was seen. The smooth muscle cells bearing numerous processes were bound by many microfilaments and by some elastic material. The energetic enzymatic activities were increased. The part taken by the lipidic diet in these changes, apart from the increase of the esterase activity, did not seem yet of significance in this first stage of atherosclerosis. However, one cannot exclude that the intensively accumulating lipids might, at some later stages, potentiate the consequences of the vascular lesions or impede the repair process.

    Notes: Summary Fifteen spleens with hairy cell leukemia are studied with immunofluorescent (7 cases), electron microscopic (5 cases), and usual histologic methods. The findings are: enlargment of the spleen (weight always superior to 400 g), diffuse red pulp infiltration by ambiguous cells with regular repartition of nuclei and clear spaces between them, hairy aspects of the cytoplasmic membrane which are especially observed on semi-thin and ultra-thin sections, presence of particular cytoplasmic inclusion bodies (polysome lamellae complex). The cellular infiltration is accompanied by some vascular modifications: pseudo angiomatosis lesions, nodular formations ressembling splenomas. An important hypertrophy of the splenic macrophages with erythrophagocytosis and siderosis is also observed. These two phenomena partly explain the anemia.

    Notes: Summary Light cytology, enzymology, immunofluorescence and scanning electron microscopy have been performed on 49 bronchoalveolar fluids recovered by bronchoscopic lavage. The patients had the following lung diseases: infectious pneumonitis (19 cases), pulmonary fibrosis (13 cases) including 5 sarcoïdosis, 3 idiopathic pulmonary fibrosis and 5 silicosis, hypersensitivity pneumonitis (5 cases) and miscellaneous lung tumors (12 cases). Cytologic studies in comparison with clinical aspects show 4 groups: Group I (chronic bronchopneumopathy and inactive fibrosis) presents numerous cells but few lymphocytes (less than 5%); in Group II (evolutive fibrosis) have an increase percentage of lymphocytes (20%); in Group III (hypersensitivity pneumonitis) is observed a very high percentage of lymphocytes (45%); and in Group IV (cancerous lung diseases) values are not far from normal percentage except for lymphomas. Small macrophages (diameter: 10 microns) with a central monocytoïde nucleus and few cytoplasma, are abundant in groups II and III where lymphocytosis is higher. Scanning electron microscopy shows irregular and rough surface, and numerous spontaneous adherences with erythrocytes, lymphocytes, or bacterias. Enzymatic activity (acid hydrolase, esterase, oxydase) increases in these cells. Lymphocytes have a smooth surface evocative of T origin which is confirmed by granular acid phosphatase positivity and rosette forming test. Immunofluorescence shows positive granules with IgG, C1Q and C3 in macrophages only for Groups II and III while free immunoglobulins were present in the recovered lavage fluid.

    Notes: Summary The finding of a focal and segmental glomerulonephritis (GN) in a patient with a Behçet's syndrome led us to perform systematic renal biopsies in ten other patients with the disease. Renal biopsy specimens of 11 patients with Behçet's syndrome (followed for 6 months to 15 years) have been studied by light, electron and immunofluorescence microscopy. In all cases, blood pressure and renal function were normal. Proteinuria was present in five patients. By light and electron microscopy, amyloidosis could not be demonstrated in any case. In one patient, the focal and segmental GN was associated with fibrinoid, electron dense, mesangial and irregular subepithelial deposits. These deposits were also detected in seven other patients but to a lesser degree. Arteriolosclerosis was present in all cases. By immuno fluorescence, small scattered granules of C3 were observed in 10 patients in the mesangium and along the capillary basement membrane. They were diffuse in six cases and focal in four. Small focal deposits containing IgA and/or IgG, Clq were also observed in four cases. Circulating immune complexes found in six out of seven patients in whom they were sought. Rare cases of focal and segmental GN and amyloidosis have been reported in Behçet's syndrome. To our knowledge, glomerular C3 deposits have not been yet reported. These findings with the presence of circulating immune complexes suggest that renal symptoms occasionnally observed in Behçet's syndrome could be related to immune complex deposition.

    Notes: Summary 1,000 endomyocardial biopsies performed in 110 patients treated by cardiac graft were reviewed. These biopsies permitted early detection of acute rejection after cyclosporin treatment and a good appreciation of its intensity. By this method, almost all rejection episodes were resolved after adequate treatment. Chronic rejection was diagnosed by arteriography used in vivo or in cardiac transplants removed by surgery or necropsy. Rejection provoked an obliterative fibrous endarteritis often complicated by atherosclerosis and its ischaemic consequences. 34 autopsies were performed in patients dead at a variable time after cardiac or cardio-pulmonary transplantation. In early death (14 cases), graft failure and systemic disorders were observed. Acute and chronic rejection was noted less frequently (9 cases). Systemic infections (10 cases) occured either early (post-surgical complications) or late (bacterial, fungal and parasitic lesions). In one case, death was due to a contemporaneous bladder carcinoma. The complications of cyclosporin treatment are briefly discussed.
