    Notes: Background Patients with severe asthma are often inadequately controlled on existing anti-asthma therapy, constituting an unmet clinical need.Objective This randomized, double-blind, placebo-controlled trial evaluated the ability of omalizumab, a humanized monoclonal anti-IgE antibody, to improve disease control sufficiently to enable inhaled corticosteroid reduction in patients with severe allergic asthma.Methods After a run-in period when an optimized fluticasone dose (〈inlineGraphic alt="geqslant R: gt-or-equal, slanted" extraInfo="nonStandardEntity" href="urn:x-wiley:09547894:CEA1916:ges" location="ges.gif"/〉1000 μg/day) was received for 4 weeks, patients were randomized to receive subcutaneous omalizumab [minimum 0.016 mg/kg/IgE (IU/mL) per 4 weeks; n=126] or matching placebo (n=120) at intervals of 2 or 4 weeks. The study comprised a 16-week add-on phase of treatment followed by a 16-week fluticasone-reduction phase. Short-/long-acting β2-agonists were allowed as needed.Results Median reductions in fluticasone dose were significantly greater with omalizumab than placebo: 60% vs. 50% (P=0.003). Some 73.8% and 50.8% of patients, respectively, achieved a 〈inlineGraphic alt="geqslant R: gt-or-equal, slanted" extraInfo="nonStandardEntity" href="urn:x-wiley:09547894:CEA1916:ges" location="ges.gif"/〉50% dose reduction (P=0.001). Fluticasone dose reduction to 〈inlineGraphic alt="leqslant R: less-than-or-eq, slant" extraInfo="nonStandardEntity" href="urn:x-wiley:09547894:CEA1916:les" location="les.gif"/〉500 μg/day occurred in 60.3% of omalizumab recipients vs. 45.8% of placebo-treated patients (P=0.026). Through both phases, omalizumab reduced rescue medication requirements, improved asthma symptoms and asthma-related quality of life compared to placebo.Conclusion Omalizumab treatment improves asthma control in severely allergic asthmatics, reducing inhaled corticosteroid requirements without worsening of symptom control or increase in rescue medication use.

    Notes: Abstract The effect of incubation medium osmolality on the respiratory burst of human neutrophils was studied using luminol-dependent chemiluminescence (CL) as an indicator of burst activity. Neutrophils were stimulated with N-formyl-Met-Leu-Phe (FMLP), phorbol-12-myristate-13-acetate (PMA), the calcium ionophore A23187, thermoaggregated IgG (IgGn), and opsonized zymosan (OZ). It was shown that increasing the osmolality of the incubation medium from 320 up to 420 mosM decreased the A231870 and OZ-induced CL responses by 90%. Under the same conditions PMA-, FMLP- and IgGn-induced CL responses were decreased by 40–60%. A decrease of osmolality to 200 mosM resulted in a 2–3 fold decrease of the A23187-, PMA- and FMLP-induced CL and in a 60–80% increase of OZ- and IgGn-induced CL. It is suggested that osmolality-mediated alteration of cell volume is an important mechanism for regulating neutrophil activity.

    Notes: Abstract Donnan potential recordings demonstrate that the preparation troventol exhibits a positive surface charge, atropine sulfate a negative surface charge, and atrovent a charge close to zero. Analysis of electrostatic interactions between these preparations and cells reveals that alveolar macrophages exhibit a more negative surface charge than peritoneal macrophages. More pronounced electrostatic interactions of the preparations with membrane macrophages are observed with troventol versus the other broncholytics studied.

    Notes: Abstract Molecular mechanism of the effect of the anticholinergic bronchodilator troventol on histamine secretion, the initial step in bronchospasm, is studied. Atrovent (ipratropium bromide) and atropine sulfate are used as reference preparations. Histamine secretion is induced by adding phorbol myristate acetate to cell suspension. In cells incubated for 5 min with troventol histamine secretion constitutes 52.4% of the maximum level, while atrovent and atropine have no effect on this process. Histamine secretion in mast cells is initiated by a sharp increase in cytosolic calcium. Troventol and atrovent reduce the initial rate of passive calcium entry into the cells by 56.3 and 28%, respectively, while atropine does not affect this parameter.

    Notes: Abstract Preincubation of phagocytes with ryosidine and EGTA increases the intracellular production of reactive oxygen species and decreases their extracellular production in cells activated with phorbol 12-myristate 13-acetate. Ryosidine and EGTA decreased intra-and extracellular production of reactive oxygen species in formyl peptide-stimulated leukocytes. The addition of the ionophore A23187 led to the appearance of reactive oxygen species in the extracellular, but not in the intracellular, medium.

    Notes: Abstract Submerging in a pressure chamber to a depth of up to 250 m was simulated in a 22-day experiment. The production of active oxygen forms in the blood leukocytes of divers was studied. The production of superoxide radical was 5–12 times decreased in the leukocytes stimulated with phorbol myristate acetate, which may be due to a 2- to 3-fold decrease in NADPH oxidase activity. A 5-fold increase in the ratio of luminol to lucigenin chemiluminescence indicates that production of active oxygen forms is shifted towards the forms other than superoxide. Increased incidence of infectious diseases in the divers after long deep submerging may result from disorders in primary nonspecific immunity in the presence of phagocytosing blood cells.

    Notes: Abstract The development of bronchospasm is shown to be accompanied by lipid peroxidation (LPO) activation; 3-fold and 8-fold rises of malondialdehyde concentration are found in homogenate of lung from sensitized animals and from animals provoked with egg albumin antigen, respectively. The use of luminol-dependent chemiluminescence (CL) reveals that in sensitized rats the production of oxygen free radicals is increased by alveolar macrophages activated with phorbol myristate acetate. Troventol at 10−3 mg/ml inhibits the CL response of phagocytes both in intact and in sensitized rats and lowers the level of Fe2+-induced LPO in lung tissue but not in the liver of intact animals.

    Notes: Abstract The possibility of using the anion gap, a parameter characterizing the electrolyte balance, for an evaluation of tissue oxygen uptake is studied. A reliable reverse linear dependence is found between the anion gap at rest and oxygen uptake at the onset of the anaerobic threshold, which allows the anion gap to be used for evaluating the efficiency of respiration as a whole and the involvement of compensatory mechanisms in patients with respiratory disorders. The anion gap parameter makes it possible to determine the adequacy of tissue oxygen uptake and tolerance for physical strain without exercise testing. The dynamics of this parameter reflects the changes in tissue oxygen uptake and tolerance for physical strain in the course of treatment.
