    Notes: Summary. The standard format in which NHS regions are invited to submit data from their perinatal mortality surveys for comparative analysis in the British Journal of Obstetrics and Gynaecology is described. Some examples of the way these data can and will be used to compare regional differences in mortality patterns are given and possible future developments are discussed. Although the term perinatal mortality is used in the title, it is hoped that surveys will cover stillbirths and neonatal deaths and also, where possible, late fetal deaths of less than 28 weeks gestation.

    Notes: The efficacy of graduated compression stockings in the prevention of deep vein thrombosis (DVT) after major gynaecological surgery was investigated in a controlled randomized prospective trial in 196 patients who were 〉35 years of age. The stockings were worn by 104 of the 196 patients throughout their stay in hospital, the other 92 patients did not wear the stockings (control group). All the patients were scanned for DVT postoperatively with the 125I-labelled fibrinogen test. None of the 104 patients who wore the stockings developed a thromboembolism, but four of the 92 control patients who did not wear the stockings had DVT. This difference between the two groups was statistically significant.

    Notes: The changes in systemic plasminogen activator and serum fibrinogen-fibrin degradation products (FDP) during and following elective Caesarean section have been studied in ten healthy pregnant women, and compared with the changes observed in ten age-matched non-pregnant women undergoing hysterectomy or salpingectomy. The Caesarean section patients showed a highly significant reduction in plasminogen activator response during operation. No significant difference was observed in the serum FDP responses. These results confirm the view that depression in the ability to mobilize plasminogen activator occurs in the later months of normal pregnancy. The clinical relationship of this to the development of uncompensated disseminated intravascular coagulation is discussed.

    Notes: Summary. National routinely collected hospital data for 12 months from June 1976 were used to provide information on patients with cervical suture during pregnancy. The past obstetric history and outcome of the pregnancy after suture was contrasted with the experience of all pregnant women during the same year. The nature of the data did not allow adequate examination of any change in the rate of fetal wastage, before and after suture; or in the secular change in fetal wastage in the pregnant population. After cervical suture the rates of both premature delivery and of perinatal mortality were higher than in the general pregnant population. The perinatal mortality rate in the cervical suture group showed a greater improvement after suture than was seen to occur between previous and current pregnancies in the whole population, although the excess improvement in pregnancy outcome that occurred after cervical suture could not be definitely ascribed to the suture.

    Notes: Summary. Using record linkage data derived from Scottish hospital discharge summaries, admissions to gynaecological units and hysterectomy rates have been studied following sterilization. The average annual gynaecological admission rate per 1000 women was 43·7 in the group of sterilized women and 21·5 in the control subjects. The average annual hysterectomy rate was 9·3 in the sterilized women and 2·5 in the control subjects. Although there were differences in age and pregnancy number between cases and controls, these did not account for the observed differences. This study was not able to determine whether the increase in gynaecological morbidity was due to the sterilization procedure or to the characteristics of the women sterilized.

    Notes: In women of reproductive age in a Northumbrian mining village, there was a weak inverse relationship between the amount of blood lost at menstruation and various haematological indices. In those who had not recently taken iron medicines this relationship was more pronounced: the greater the loss, the lower the indices. Against expectation, however, this negative association was as strong at low as at high levels of menstrual loss. It is concluded that, within the range experienced in this community, the association between menstrual losses and haematological indices is not likely to be due to iron depletion.

    Notes: Menstrual blood losses were measured in 348 women aged 17 to 45 years in a Northumbrian mining village. Menstrual loss was found to be related to parity and to the birthweight of previous children. It is possibly associated with height, and may be partly controlled by uterine size and blood flow.

    Notes: Abstract The present study investigated self- and teacher-perceptions of deaf adolescents in relation to cultural identity. Fifty-one deaf adolescents completed the Porteous Checklist and Deaf Identity Scale presented in British Sign Language. Subjects were assigned to deaf, hearing or dual identity groups. Results suggest that deaf adolescents' self-perceived concerns are not dissimilar in content or severity to those of their hearing peers, although certain issues may assume a particular significance in the presence of deafness. The hypothesis that the hearing identity group would report most problems was not supported. Teachers rated the dual identity group as having the fewest difficulties.

    Notes: Objective To determine the indications for singleton caesarean sections in Scotland in 1994.Design Prospective survey of singleton caesarean sections using information provided by clinicians entering data onto a computer in labour wards.Setting Twenty-three consultant-led obstetric units in Scotland.Population Women undergoing caesarean section in participating Scottish maternity units during 1994.Results Using routinely collected data it was found that the caesarean section rate varied by maternal age, parity, gestation, history of previous section and hospital of delivery, but there was no difference by area of deprivation. 87.4% (8369/9573) of Scottish caesarean sections were included in the survey of which 8098 were in women with singleton pregnancies. Most singleton caesarean sections were undertaken in primiparae (50.5%), 31.2% were in women with a history of previous section and 18.3% were in multiparous women who had not had a previous section; 38.9% of the operations were elective, 13.9% were emergencies before labour and 47.2% were emergencies during labour. Four main indications—elective section for breech presentation (10.7% of all sections); emergency Caesarean section before labour because of fetal problems (2.8%); emergency caesarean section during labour for fetal distress and/or failure to progress (30.3%); repeat section for reasons other than above (16.2%) accounted for 60% of all Caesarean sections in women with no other recorded complications. 7.7% of all singleton caesarean sections (19.8% of elective operations) were associated with maternal request for the operation.Discussion The survey has identified why caesarean sections are performed by Scottish obstetricians and highlighted some areas where there would be scope to increase the vaginal delivery rate. Before this can be attempted, agreement must be reached by clinicians about effective management of particular problems. Women also need to have ready access to evidenced-based information about caesarean section.
