    Notes: Summary In a human medullary carcinoma of thyroid gland containing calcitonin in light microscopic demonstration by the avidin biotin complex (ABC) method characteristic secretory granules were found electron microscopically in the cytoplasm of the tumour cells. They consisted in so-called type I granules (270±25 nm) and type II granules (135±17 nm). By the immuno gold staining (IGS) method the content of many secretory granules measuring 85–270 nm (152±18 nm) in diameter could be identified as calcitonin. These granules seemed to be predominantly of type II because of their nearly corresponding size and feature. The type I grnaules were less frequent in number and they showed no or little immunoreactivity. The results indicate that the IGS-method is practicable to demonstrate the ultrastructural localization of calcitonin and to identify clearly the nature of intracytoplasmic granules in electron microscopy.

    Description / Table of Contents: Zusammenfassung In 7 Fällen unter 98 Patienten mit perforierten Duodenalulcera konnte mikrobiologisch und histologisch bewiesen werden, daß an der Perforation Candidamykosen beteiligt waren. Alle Patienten hatten Risikofaktoren wie Diabetes mellitus, Mangelernährung, Tumorleiden, oder sie hatten zuvor ein Antibioticum oder ein Antacidum erhalten. Nach Diagnosestellung wurde die postoperative Behandlung auf Antimycotica umgestellt. Bei einer Patientin wurde die Candidainfektion nicht rechtzeitig erkannt. Sie verstarb an einer Candidasepsis. Die anderen Patienten überlebten.
    Notes: Summary In 7 cases of 98 patients with perforated duodenal ulcer, it was proved histologically and by microbiological samples that the perforation was induced by Candida mycosis. All patients were at risk because of problems like alimentary diabetes, malnutrition, tumors, or they had received antacids or antibiotics before. After the diagnosis of Candida in the intraoperative samples, the therapeutic regimen was altered to antimycotics. In the case of one female patient, the Candida infection was not detected in time, and she died of Candida sepsis. All other patients survived.

    Notes: Abstract In 42 human gastric adenocarcinomas of intestinal (n=25) and diffuse types (n=17) the expression of CD44v6 splice variants was investigated immunohistochemically and compared with the pattern of lymphogenic tumor spreading. Distinct differences were observed between the two cancer types: 92% of intestinal-type tumors expressed CD44v6 as in the intestinal metaplasia in chronic atrophic gastritis, while v6 expression occurred in only 17% of diffuse-type cancers. The analysis of RNA expression confirmed the immunohistochemical data. Intestinal-type cancers yielded a much more complex pattern of amplification products hybridizing to exon v6 than did normal mucosa, whereas diffuse-type tumors did not express exon v6. Also the pattern of lymphogenic spreading was quite different between the two cancer types: in diffuse-type tumors only a sinus carcinosis without CD44v6 expression was observed in a significantly higher number of lymph nodes than in intestinal-type cancers, which showed in particular infiltrative lymph node metastases always with CD44v6 expression as in the primary tumors. When infiltrative lymph node invasion occurred in v6-negative diffuse-type cancers, v6 neoexpression was also demonstrable in the lymph node metastases. Additionally, the number of infiltrative lymphogenic metastases increased with more extensive v6 expression in primary gastric cancers of both types. These data suggest that the expression of CD44v6 isoform is important for the infiltrative spreading of tumor cells into lymph nodes. Addtionally, the phenotypic similarities in v6 expression between intestinal metaplasia and intestinal-type cancers, but not of tumors of diffuse type, may support the Correa hypothesis.

    Notes: Summary A new human transitonal cell carcinoma cell line has been established in long term tissue culture. The BT-1 cells were derived from a poorly differentiated human bladder cancer. The tumor cell line produces tumors in nude mice. BT-1 has been characterized by cytogenetic and flow cytometric analysis and by isoenzyme typing. As in direct preparations of human bladder tumors, an isochromosome 5p is a consistent marker of the newly established line. The BT-1 cells produce transforming growth factors but do not respond to exogeneous EGF.

    Notes: Abstract Carcinoid tumors belong to the group of neuroendocrine tumors of epithelial origin, i.e., neuroendocrine carcinomas. These neoplasms usually occur in the gastrointestinal tract or bronchial system but are very rare neoplasms in the larynx. Since carcinoid tumors in this latter site may appear to be undifferentiated by light microscopy, they may possibly be misinterpreted and their neuroendocrine characteristics may remain unrecognized. Using immunohistochemical methods, three carcinoid tumors were studied and showed positive immunostaining for markers of epithelial origin (cytokeratins, epithelial membrane antigen, carcino-embryonic antigen) and, in particular, for markers of neuroendocrine differentiation (chromogranin, synaptophysin, neuron-specific enolase). All tumors expressed calcitonin-, serotonin- and adrenocorticotropic-hormone-like immunoreactivity. In contrast, three poorly differentiated squamous cell carcinomas showed positive immunostaining for epithelial markers but did not show any immunoreactivity with markers of endocrine characteristics. Fine structurally, carcinoid tumor cells contained neurosecretory-type granules scattered throughout the cytoplasm. The present study demonstrated that (1) carcinoid tumors of the larynx possess distinct immunohistochemical characteristics that allow a clear classification, (2) it is advisable to use a battery of primary antibodies rather than rely on specificity and sensitivity of a single marker to establish diagnosis and (3) the fine structural demonstration of neurosecretory-type granules serves as a reliable adjunct to diagnosis.

Im vorliegenden Fall erfolgte die Resektion des Lipoms wegen des Vorliegens einer symptomatischen arteriellen Hypertonie. Postoperativ normalisierten sich die Blutdruckwerte. Anhand dieses Falls und des Literaturüberblicks werden Diagnostik und Therapie des Nebennierenlipoms dargestellt.
    Notes: Summary Adrenal lipomas are extremely rarely occuring benign tumors being hormonally inactive. The patient described in this report underwent surgical excision of an adrenal lipoma because of the associated hypertension. The blood pressure decreased postoperatively to normal levels. This case report and the review of the literature consider diagnosis and therapy of the adrenal lipomas.
