    Description / Table of Contents: Résumé Dans une série de plus de 1.000 résections non urgentes pour anévrisme de l'aorte abdominale avec mise en place d'une prothèse, faites au Massachusetts General Hospital au cours des 20 dernières années, la mortalité est tombée de 9.6 à 1.7%. Cette amélioration résulte, entre autres, d'un diagnostic plus précoce, d'une meilleure préparation pré-opératoire, de l'utilisation en routine de l'artériographie, de l'expérience acquise, des perfectionnements de l'anesthésie et de la surveillance postopératoire. La fréquence des échecs à 5 ans est inférieure à 2%. Comme la fréquence des ruptures est de l'ordre de 25% pour les anévrismes qui ont entre 4 et 7 cm de diamètre, nous estimons que tout malade ayant un anévrisme de cette taille et qui peut s'adresser à un bon centre chirurgical doit être opéré.
    Notes: Abstract During the past 20 years at the Massachusetts General Hospital the mortality rate for elective resection and grafting of abdominal aortic aneurysms (AAA) has fallen from 9.6% to 1.7% in a series of over 1000 patients. The improvement in mortality rate is partially the result of earlier diagnosis, better preoperative preparation, routine use of aortography, and increased technical experience. Improvements in anesthesia and postoperative care have played a large part in reducing the mortality rate of AAA resection and graft. The postgraft failure rate at 5 years is less than 2%. Since the incidence of rupture of AAA measuring between 4 and 7 cm in diameter is approximately 25%, we believe that patients with AAA of this size in a good surgical environment should have elective resection.

    Notes: Abstract Purpose: To evaluate the role of metallic stents in treating stenoses involving prosthetic arterial bypass grafts. Methods: Patients undergoing stent placement within a failing prosthetic bypass graft, during a 41-month period, were reviewed for treatment outcome and complications. The indications for stent placement in 15 patients included severe claudication (n= 3), rest pain (n= 9), and minor or major tissue loss (n= 3). Lesions were at the proximal anastomosis (n= 6), the distal anastomosis (n= 3), or within the graft (n= 6). Results: Treatment with metallic stents was successful in all patients. There was one acute stent thrombosis, successfully treated with thrombolytic therapy. Follow-up data are available for a mean duration of 12.3 months. The mean duration of primary patency was 9.4 months with 6- and 12-month primary patency rates of 51.9% and 37.0%, respectively. The mean duration of secondary patency was 12.1 months with 6- and 12-month secondary patency rates of 80.0% and 72.7%, respectively. Two patients with discontinuous runoff and preexisting gangrene required a below-knee amputation. Six patients were revised surgically after stent placement (at a mean of 10.8 months). Three late deaths occurred during follow-up. Conclusion: Given the mortality risks of surgical revision and the reduced life expectancy of this patient population, metallic stent placement represents a viable, short-term treatment option for stenoses within or at the anastomoses of prosthetic grafts. Further evaluation is warranted to compare intragraft stent placement with surgical graft revision.

    Notes: Abstract Carotid body tumors may occur sporadically (90%) or in a familial pattern (10%). One third of the patients with familial disease have bilateral tumors. We report the case of a patient with bilateral carotid body tumors and a strong family history of such tumors. Details of his evaluation and treatment are reviewed. Relatives of patients with carotid body tumors should be examined carefully, and any suspicious neck mass should be appropriately investigated.

    Notes: Abstract Aortic graft infection represents one of the most formidable challenges encountered by the vascular surgeon. Current principles of treatment are based on experience primarily derived from infection with Staphylococcus and enteric bacteria. Anaerobic prosthetic infection is a case event. Infection with Clostridium has heretofore been reported only twice. An additional case of clostridial infection of an aortic prosthesis is presented with review of the literature. Its clinical significance and management are discussed.
