    Notes: The incidence of thrombocythaemia (a platelet count 〉600×l09/l) in a 10-year survey was 7 per 106 population per year (R. M. Pettit, personal communication). It is generally a disease of late middle age, but a second population of young and mainly female patients has been described (Hoagland & Silverstein 1978). An increased platelet count can be secondary (after splenec-tomy, associated with inflammation, malignancy or iron deficiency) or due to a primary myeloproliferative disorder. We describe four patients in whom a series of miscarriages before 20 weeks gestation appears associated with thrombocythaemia and estimate the prevalence of this association in patients with unexplained repeated miscarriage.

    Notes: Eighty newborn babies were studied by ultrasound brain scan before and after their first general anaesthetic. Periventricular haemorrhage or ischaemia occurred in 23 patients (29%) and affected both term and premature babies. Four new lesions developed but only three of 19 existing lesions deteriorated. The very premature (〈 33 weeks gestation) were most at risk (four of eight deteriorated). Deterioration or development of lesions occurred only in very ill babies with known risk factors. Anaesthesia did not appear to be implicated in any case.

    Notes: A case of prolonged action of suxamethonium in a patient with gestational trophoblastic disease is reported. Postoperatively the patient was found to have markedly reduced plasma cholinesterase activity (363 IU/litre) with a normal cholinesterase phenotype. Consequently plasma cholinesterase activity and phenotype were measured in six other patients with the condition and these results compared with those of 22 patients with normal first trimester pregnancies undergoing therapeutic abortion. Plasma cholinesterase phenotype was normal in all patients studied. The activity was significantly decreased (p 〈 0.05) from the normal range (620–1370 lU/litre) in all patients with trophoblastic disease. In the 22 patients with normal pregnancies, 14 had activity values in the abnormal range (〈 620 IU/litre) while the mean cholinesterase activity of the group as a whole was significantly decreased (561.8 IU/litre, p 〈 0.05) below the normal range. These results confirm the presence of a decrease in plasma cholinesterase activity in early pregnancy and provide new evidence for a decrease in activity in a pseudopregnancy state.

    Notes: The incidence and severity of three diseases of oilseed rape, caused by beet western yellows luteovirus (BWYV), cauliflower mosaic caulimovirus (CaMV) and turnip mosaic potyvirus (TuMV), were assessed in England and Wales in the springs of 1992 and 1993. In 1992, 62% of oilseed rape crops examined were found to contain BWYV, with an average of 28% plants infected; 14% of crops contained CaMV, with an average of 5% plants infected; and TuMV with 3% crops and 〈 1% plants infected. In 1993, BWYV was found in 42% of crops and 12% of plants infected; CaMV in 25% of crops and 7% of plants infected; and TuMV in 14% of crops and 5% plants infected. The yields of plants showing severe virus symptoms were reduced by an estimated 70 to 79%. The high incidence of CaMV and TuMV in oilseed rape crops could have important consequences for rapeseed production in the UK and also serve as a source of virus for vegetable brassicae.

    Notes: Background IgE is the pivotal-specific effector molecule of allergic reactions yet it remains unclear whether the elevated production of IgE in atopic individuals is due to superantigen activation of B cell populations, increased antibody class switching to IgE or oligoclonal allergen-driven IgE responses.Objectives To increase our understanding of the mechanisms driving IgE responses in allergic disease we examined immunoglobulin variable regions of IgE heavy chain transcripts from three patients with seasonal rhinitis due to grass pollen allergy.Methods Variable domain of heavy chain-epsilon constant domain 1 cDNAs were amplified from peripheral blood using a two-step semi-nested PCR, cloned and sequenced.Results The VH gene family usage in subject A was broadly based, but there were two clusters of sequences using genes VH 3-9 and 3-11 with unusually low levels of somatic mutations, 0–3%. Subject B repeatedly used VH 1-69 and subject C repeatedly used VH 1-02, 1-46 and 5a genes. Most clones were highly mutated being only 86–95% homologous to their germline VH gene counterparts and somatic mutations were more abundant at the complementarity determining rather than framework regions. Multiple sequence alignment revealed both repeated use of particular VH genes as well as clonal relatedness among clusters of IgE transcripts.Conclusion In contrast to previous studies we observed no preferred VH gene common to IgE transcripts of the three subjects allergic to grass pollen. Moreover, most of the VH gene characteristics of the IgE transcripts were consistent with oligoclonal antigen-driven IgE responses.
