    Notes: Abstract Penetration of the spleen is a rare complication of benign gastric ulcers, with fewer than 10 cases reported. The rareness is most likely due to the uncommon occurrence of benign ulcers in the areas of the stomach contiguous with the spleen, the fundus, and the proximal portion of the greater curvature. In none of the reported cases was the preoperative diagnosis of splenic penetration made on radiologic studies. We present a case of a benign gastric ulcer penetrating into the spleen which was diagnosed preoperatively on the basis of a computed tomographic (CT) examination supplemented with prone scanning.

    Notes: Abstract Abdominal computed tomography is often the critical step in the evaluation of patients presenting in the emergency department with abdominal pain and/or suspected intra-abdominal infection. Gas is normally found in the stomach, small bowel, colon, rectum, and occasionally the appendix. However, extraluminal gas collections can be identified on abdominal radiographs or computed tomographic examinations, and their presence often suggests specific pathology. Knowledge of different intra-abdominal pathologic gas collections and the underlying physiology contributing to each is important in understanding the clinical significance of each entity. The spectrum of intra-abdominal gas collections, as well as their clinical significance, will be described.

    Notes: Abstract The retroperitoneum is a common location for injury following blunt or penetrating trauma. Additionally, there are a wide variety of nontraumatic emergencies that involve the retroperitoneum. Patients with these entities may present with symptoms ranging from acute abdominal or back pain to hypotension and/or fever. Computed tomography (CT) is the imaging modality of choice to evaluate acute processes of the retroperitoneum, as sonography has been historically less sensitive than CT. Knowledge of the compartments of the retroperitoneal space as well as the fascial planes is crucial to diagnose and understand the pathway of spread of pathology in this region. In this pictorial essay the spectrum of both traumatic and nontraumatic retroperitoneal emergencies will be demonstrated, as well as their effect on the different compartments of the retroperitoneum.
