Notes: Summary The hamartoma of the tuber cinereum, a midline dysraphic syndrome, corresponds to a mass of normal neuronal tissue implanted in the area of the mamillary bodies. It has no evolutionary potential, but association to other cerebral malformations has been observed. On the basis of 18 personal cases and a review of the literature, the authors evaluate the actual diagnostic criteria and the incidence of different clinical signs such as precocious isosexual puberty, seizures, and mental impairment.

Notes: Summary A study of 31 observations of congenital toxoplasmosis shows that there is a clear relationship between the cerebral lesions as observed on CT scan, neurological symptoms, and the date of maternal infection. The appearance of the CT scan is characteristic of cases with early maternal seroconversion (before the 20th week of pregnancy), of cases with maternal infection between the 20 and 30th week of pregnancy, and of cases with late maternal seroconversion (after the 30th week of pregnancy). Preventive maternal treatment does not change the pattern of the cerebral lesions observed on CT scan and thus proves ineffective in conferring real protection.

Notes: Summary A review of cranial CT studies of 233 patients for the Second Medulloblastoma Trial of the International Society of Paediatric Oncology showed “typical” CT appearances in only 30% of patients. The varied appearances encountered are described with particular emphasis on atypical CT features.

Notes: Summary This is a report of seven cases of aneurysm of the vein of Galen (AVG) with a review of the clinical and radiological aspects of 48 cases in the literature. The natural evolution and pathophysiology of this vascular malformation are discussed. The clinical signs of this condition are often misleading. The appearance of AVG on CT is pathognomic. Surveillance of the vascular malformation and associated cerebral lesions can be maintained with subsequent CT. Spontaneous thrombosis of the aneurysm was observed in three cases. The diagnosis of thrombosis of the AVG was always made at operation or by histological examination. The radiological appearance of a thrombosed aneurysm is typical.

Keywords: Adrenoleukodystrophy ; CT scan appearance
Source: Springer Online Journal Archives 1860-2000
    Topics: Medicine
Notes: Summary The Ct appearance of adrenoleukodystrophy is discussed on the basis of 16 personal observations and a review of the literature. CT appearance was typical in 10 of the 16 cases, atypical but suggestive of adrenoleukodystrophy in four cases, and misleading in two cases. Atypical CT presentations were most often observed at an early phase of the disease and included unilateral lesions or lesions without opacification at the periphery of the edemalike areas after contrast enhancement. In three cases, neurological signs, CT scans, and conjunctival and skin biopsies were highly suggestive of adrenoleukodystrophy, but there was no adrenal insufficiency. These cases may correspond to an evolutive or phenotypic variant of adrenoleukodystrophy.

Notes: Summary Cephaloceles are congenital malformations with herniation of intracranial structures through a defect in the cranium. On the basis of a review of the literature and 31 personal observations the authors discuss the clinical and neuroradiological presentation of their various anatomical locations: sphenoidal, ethmoidal, frontal, occipital and parietal.

Notes: Summary Hematomas of the brainstem are revealed by signs of brainstem dysfunction. Sudden onset is characteristic but only demonstration of subarachnoid hemorrhage and typical CT-Scan appearance are diagnostic. Surgical evacuation of the hematoma may help in the acute stage but does not avoid recurrence; pronosis is guarded.
