    Notes: Summary The aim of this study was to determine whether an anterior approach to the spine with a fusion of the growth centers of the anterior column of the spine, simultaneous by with a posterior fusion and instrumentation of the spine, in young patients with severe scoliosis who have considerable remaining growth, leads to prevention of the crankshaft phenomenon. Twelve patients who have had anterior fusion of the spine associated with posterior fusion and instrumentation of the spine were studied. In 10 of them, growth progression was demonstrated by modification of the Risser sign; for these patients no important progression of the spinal deformity was noted. One patient had no progression of the Risser sign and no progression of spinal deformity. One patient had progression of spinal deformity due to the disruption of the sacral anchorage of instrumentation. We think that this procedure leads to the prevention of the crankshaft phenomenon, and we recommend this procedure in young patients with severe scoliosis and considerable remaining growth. It must include all intervertebral levels of the rigid segment of the curve.

    Description / Table of Contents: Résumé Les auteurs rapportent les résultats de leur expérience récente du corset de Cheneau. Il s'agit d'une étude de 26 cas de scoliose idiopathique de l'adolescent traitée par corset. Le recul est de 16 mois de traitement , le port du corset n'étant pas interrompu. L'observance du traitement a dans tous les cas été parfaite (port du corset 23h/24). Les 19 filles et 7 garçons, d'âge moyen 15 ans 3 mois, avaient un test de Risser inférieur à 2 dans 50% des cas en début de traitement. Au recul maximum, 69% des patients avaient un test de Risser supérieur à 3. Les courbures étaient thoraciques, lombaires ou combinées à l'exception d'un cas. 73% des patients ont été améliorés dans le plan frontal et 27% stabilisés à 2° près. L'angulation frontale initiale moyenne était de 30° en thoracique, 24° en lombaire et le gain moyen au recul maximum était respectivement de 27% et 37,5%. Dans le plan sagittal une perte angulaire de 23% en dorsal et de 9% en lombaire ont été constatées entre l'état avant mise en place du corset et le recul maximum avec corset. L'effet de dérotation a été nul, mais la rotation a été stabilisée. La translation apicale a été améliorée de 23%. Nos résultats confirment l'efficacité du corset de Cheneau dans le traitement orthopédique de la scoliose idiopathique de l'adolescent pour les courbures thoraciques, lombaires ou combinées. Cependant nous mettons l'accent sur la nécessité de surveillance des courbures sagittales des scolioses traitées en raison de l'effet délétère de ce corset à ce niveau.
    Notes: Summary This study aims at presenting the preliminary result of personal experience using the Cheneau-brace in the treatment of idiopathic scoliosis in 26 cases. The average follow-up was 16 months. In every case, wear compliance was perfect ( brace worn 23h out of 24h). At the beginning of the treatment the 19 girls and 7 boys, whose average age was 15 years and 3 months, had a Risser test below 2 in 50% of the cases. At follow-up 69% of the patients had a Risser test higher than 3. In every case but one , the curves were thoracic, lumbar or combined. In frontal plane 73% of the patients were improved and 27% stabilized to within 2 degrees. Initially the average frontal Cobb angle measured 30° at thoracic level and 24° at lumbar level. At follow-up the average improvement was respectively 27% and 37,5%. In sagittal plane a 23% angular loss at dorsal level and a 9% loss at lumbar level were detected at follow-up with brace. We did not notice any effect on vertebral rotation but the movement of translation of the apical vertebra toward the chest wall in the convex side was improved (23%). We confirm that Cheneau-brace is efficient in the conservative treatment of adolescent idiopathic scoliosis but we must point out a decrease of kyphosis and lordosis was detected.
