    Description / Table of Contents: Zusammenfassung Ein erwachsener Patient mit Fanconi-Anaemie wird beschrieben, die 5 Jahre vor der klinischen Manifestation der Krankheit durch den Nachweis einer erhöhten Chromosomenbrüchigkeit diagnostiziert wurde. Bei dem bis heute unbehandelten Patienten wurden in einer Beobachtungsperiode von 6 Jahren wiederholt klinische, hämatologische und biochemische Befunde erhoben. In der gleichen Zeit wurden zahlreiche zytogenetische Analysen durchgeführt, deren Ergebnisse keine Korrelation zu dem klinischen Bild sowie den hämatologischen Befunden zeigten. Vier gut erkennbare Lymphozyten-Klone wurden entdeckt. Der Patient wird weiterhin klinisch und zytogenetisch beobachtet.
    Notes: Summary A male adult patient suffering from Fanconi's anemia is described who was diagnosed 5 years before the onset of clinical symptoms by cytogenetic findings of chromosomeinstability in a lymphocyte culture. Repeated clinical, haematological and biochemical investigations of the untreated patient have been made during the observation period of six years. In the same period of time cytogenetic studies have been carried out which show no correlation in results compared with the clinical or physical findings. Four well defined lymphocyte clones have been discovered. The patient is still under observation of the clinic and the cytogenetic department.

    Notes: Summary The object of the study was to evaluate the effectiveness of ifosfamide/etoposide and mesna therapy in advanced breast cancer. A total of 44 patients with breast cancer were included in the trial. Eligibility criteria included measurable, refractory disease; prior anthracycline therapy (or its contraindication); a life expectancy of at least 3 months; and adequate hepatic, renal, CNS and bone marrow function. All patients were≤70 years of age and had a Karnofsky performance status of≥50%. There were 36 evaluable cases. Sites of metastatic disease included bone (19), skin (18), liver (9), lung (14), lymph node (19), and miscellaneous (7). Treatment consisted of 1,500 mg/m2 ifosfamide given i.v. on days 1–5, 120 mg/m2 etoposide given i.v. on days 1–3, and 400 mg i.v. mesna given with and at 4 and 8 h after ifosfamide. Cycles were repeated every 28 days. Initial doses were reduced by 25% or 50% in patients who had previously undergone both chemotherapy and radiotherapy. A median of 4 cycles (range, 2–8) were given. The myelotoxicity was marked: WHO grades 3/4 leukopenia (n=37), grades 3/4 thrombocytopenia (n=12), and grades 2/3 anemia (n=13). Due to myelotoxicity, dose reduction or prolongation of treatment-free intervals was necessary in 28 cases. Alopecia was seen in 35 patients and CNS toxicity, in 8. Partial remission (PR) was obtained in five cases and complete remission (CR), in three. Sites of response included the lung (5), skin (4), lymph node (5), and peritoneum (1). The duration of response was 4 (n=2) and 8 (n=1) months for CR and 2 (n=2), 6 (n=2), and 10 (n=1) months for PR. We conclude that the ifosfamide/etoposide and mesna regimen is effective, but its myelotoxicity is treatment-limiting.

    Notes: Abstract The expression of the oncogenes c-fos, c-jun, c-myc, c-erbB-1 and c-erbB-2 at the protein level was analyzed in squamous cell lung carcinomas of 121 patients by means of immunohistochemistry. Patients with overexpression of proteins encoded by the oncogenes c-fos, c-jun and c-erbB-1 had significantly shorter survival times than these without overexpression of these oncogene products (c-fos:p=0.009; c-jun:p=0.029; c-erbB-1:p=0.018). No significant correlations were found between the expression of c-myc and c-erbB-2 products and the survival of the patients. In addition to the univariate analyses (Kaplan-Meier-estimates) multivariate analyses (Cox-regression-model) revealed that protein expression of the oncogenes c-fos, c-jun and c-erbB1 are significant prognostic factors in addition to staging.

    Notes: Abstract Locally advanced non-small-cell lung cancer (NSCLC) in most cases is not curable at the present time. Owing to the local extent of the tumor, the rate of complete resections is low and, therefore, survival in these patients is poor. For this reason, induction chemotherapy is being investigated in patients expected to have a poor prognosis after standard surgery and radiotherapy. The rationale for induction chemotherapy is to increase the rate of complete resections and achieve early elimination of micrometastases. Clinical investigations have reported an improvement of survival in stage III NSCLC after induction chemotherapy by using different combinations of cytotoxic drugs. Vindesine ranks among the most active single agents in this disease and has been part of a number of combination regimens in induction chemotherapy. The combination of mitomycin, vindesine or vinblastine and cisplatin has produced encouraging results in several studies, indicating a possible improvement of survival in stage III NSCLC, although its superiority to other combinations yet has to be demonstrated.

    Notes: Abstract Their physical properties make lasers ideal instruments for endoscopic surgical procedures in the narrow tracheobronchial system. By the thermal effects of the Nd-YAG laser, pathological benign and, especially, malignant lesions can be destroyed under direct vision. Working without contact with the tissue, sparing the risk of bleeding and further mechanical obstruction the laser has replaced mechanical and electrical devices and cryoprobes in interventional bronchoscopy to a large extent. Thus many patients with benign lesions can be spared the risk of major thoracic surgery of the large airways. To patients suffering from tumours of the central airways the chance of long-term palliation can be given by resolution of life-threatening complications. Eurthermore, by photodynamic laser therapy after application of haematoporphyrin derivatives some patients may even be cured.

    Notes: In the current study the relationship between the incidence of metastatic spread and expression (at the protein level) of various proto-oncogenes was investigated in 217 human non-small cell lung carcinomas. Tumors with an overexpression of proteins encoded by the oncogenes c-jun and c-myc showed a significantly increased formation of metastases (c-jun: P = 0.008; c-myc: P = 0.018). No significant correlations were found between the expression of the c-fos, c-erbB1, c-neu and c-ras products and metastatic spread.
