    Notes: In four patients with juvenile periodontitis (group A), five adults with advanced periodontitis (group B), and in four controls (group C) the concentration of serum immunoglobulins, serum complement (C3C4, and C5) T and B lymphocyte ratio, leukocyte migration inhibition and lymphocyte transformation responses to dental plaque bacteria (A. viscosus, B. melaninogenicus, F. nucleatum, and V. parvula) and PPD or PHA, and lymphocyte ATP-ase activity were assessed before and during development of experimental gingivitis.All the patients in group A and B Had completed extensive periodontal treatment, and at the time of the experiment the periodontal conditions were excellent. Plaque accumulation and gingival inflammation were assessed and blood samples were drawn: at day 0; after plaque accumulation for 14 d (no oral hygiene) (period I); after 14 d with optimal oral hygiene (period II); after plaque accumulation for 14 d (no oral hygiene) and concomitant immunostimulatory treatment with levamisole (period III); after 14 d with optimal oral hygiene. After period I, gingival inflammation and increased gingival fluid flow developed in all groups; however, in group C some of the clinical and immunological responses were slightly enhanced as compared wiht group A and group B. On the other hand, after period III (plaque accumulation + levamisole) more pronounced clinical and immunological changes were recorded in group A and B than in group C, including a significant rise in gingival fluid flow, LIF production, and lymphocyte ATP-ase activity. The present study has supported the asumption that a state of immunodeficiency can be an underlying, predisposing factor for juvenile periodontitis. The results, however, give only vague information on the nature of this deficiency

    Notes: Detailed quantitative studies of phagocytosis were performed on purified blood neutrophils isolated from 12 young and 10 adult patients with advanced periodontitis and from 22 age and sex matched controls without periodontitis. Chemotaxis was significantly reduced only in the young patients whereas no difference could be demonstrated between young and adult patients or controls in uptake of opsonized zymosan particles, in the concentration and consumption rate of ATP, in the rate of oxygen consumption and superoxide liberation, or in the exocytosis of β-glucuronidase and lysozyme during phagocytosis. Serum opsonization was equally effective in the 3 groups. It is concluded that defective chemotaxis in juvenile periodontitis does not originate from defective mobilization of energy in the neutrophils during active movements.

    Notes: Summary.— Deficient folate activity was diagnosed in all of 7 patients treated for psoriasis with methotrexate (Mtx) by applying a new method (Ellegaard and Esmann, 1972a) of assaying folate activity which utilizes the folate-dependent incorporation of 14C-formate into serine of lymphocytes during incubation under standardized conditions. The arithmetical mean of this folate activity was 4–8%± 0.6 (SEM) of added 14C-activity per 109 lymphocytes per 4 hours, as compared to 9.2%± 0.4 in 24 normals. The 14C-formate incorporation into RNA was severely depressed in each case. Mtx added to lymphocytes in vitro had no effect on the folate activity.In addition, the new observation was made that patients under treatment with Mtx develop decreasing serum concentrations of B12, possibly due to an impaired B12-absorption.

    Notes: Summary In 27 patients initially diagnosed as refractory anaemia (RA) or RA with sideroblasts (RA-S) according to the FAB-classification a number of clinical, morphological and cytogenetic parameters were correlated for prognostic significance. From these correlations it emerged that severe cytopenia is centrally positioned with regard to clinical course in RA and RA-S. Positive correlations were found to initial diagnosis, clonal cytogenetic abnormalities, progression to RA with an excess of blasts (RAEB) or acute myeloid leukaemia (AML), the percentage of bone marrow blast cells and prolonged half life for radioactively labeled iron. The degree of peripheral blood granulocytopenia, alone, was correlated to bone marrow hypoplasia. Moreover, the frequency of abnormal karyotypes was inversely correlated to bone marrow cellularity and proportional to the frequency of bone marrow blast cells. From these relationships it may be proposed that chromosome abnormalities are associated with prolonged blast cell generation times and inhibition of blast cell maturation resulting in reduced marrow cellularity and blast cell accumulation, and, in the peripheral blood, falling percentages of neutrophil granulocytes. With the blast cell accumulation the bone marrow cellularity again becomes hyperplastic and the preleukaemic condition is transformed into RAEB or AML.

    Notes: Summary Phenotypic characterization of neoplastic cells from 5 patients with hairy cell leukemia (HCL) was performed with 29 monoclonal and 6 polyclonal (anti-Ig) antibodies using immunoperoxidase staining of fresh frozen splenic tissue. Monotypic Ig was expressed in 4 cases, one case was non-expressive. Strong staining was obtained in all cases by monoclonal antibodies (MAs) specific for 3 pan-B-lymphocyte antigens (by anti-B 1, To 15, anti-Leu 12). Five other B-cell related antigens detectable with appropriate MAs (BA-1, anti-B 2, DAKO-C 3 b R, Tü 1, 38.13) were absent in all cases. The stainings with 13 T-cell associated MAs (OKT 3, OKT 4, anti-Leu 3 a, OKT 6, OKT 8, Tü 68, OKT 10, anti-Lyt 2, Tü 71, OKT 11, anti-Lyt 3, Tü 14, Tü 33) were all negative. Stainings with 4 MAs recognizing myelocytic and/or monocytic antigens (OKM 1, anti-Mo 1, anti-Mo 2, 3 C 4) were also negative. We included 14 frozen biopsies with B-type chronic lymphatic leukemia (B-CLL) into our immunohistological study in order to establish phenotypic differences between HCL and B-CLL. Five MAs (Tü 1, anti-Lyt 2, Tü 71, BA-1 and anti-B 2) gave consistently negative staining in HCL cases but positive staining in most or all B-CLL cases. The study provides significant evidence for the B-cell nature of HCL and also establishes important phenotypic differences between HCL and B-CLL.

    Notes: Abstract Possible factors predisposing to the development of intraoral ulcers in immunocompromised patients with haematologic malignancies were investigated. Among 46 patients undergoing antineoplastic treatment, 18 developed an intraoral ulcer during the prospective study. Patients with or without ulcers were comparable with respect to underlying disease, presence of teeth, the qualitative composition of the aerobic and facultatively anaerobic oral microflora, herpes simplex virus (HSV) titer positivity, a past history of herpes labialis, leukocyte and thrombocyte counts, and duration of fever. Viral cultures from saliva revealed an association between the presence of HSV in saliva and the presence of intraoral ulcers. HSV was not isolated from the saliva of any patient without ulcers. Viral cultures from the ulcers revealed growth of HSV in 11 (61 %) of the 18 patients with ulcers. HSV-positive ulcers were located more often on the alveolar process than elsewhere in the oral cavity. Ulcers on the lateral borders of the tongue and on the buccal mucosa were found only in dentate patients. Development of intraoral ulcers was not associated with an increase of the HSV titer in serum in any patient. It is concluded that some intraoral ulcers, especially when located on the alveolar process, are associated with and probably caused by HSV, while mechanical trauma during mastication seems likely to be involved in the pathogenesis of ulcers elsewhere in the oral cavity.
