    Notes: Abstract: This article describes the experiences of five community health nursing faculty who are jointly appointed between McMaster University School of Nursing and the Hamilton-Wentworth Department of Public Health Services in Hamilton, Ontario, Canada. The appointees are actively involved in service, education, and research activities within the teaching health unit, a specially funded project of the Ontario Ministry of Health. Various kinds of consultation and expertise are offered by joint appointees to staff and management within the Department of Public Health Services. The amount of time and effort invested in a particular activity or project varies as staff, students, and joint appointees each make their contributions. Several lessons are learned in acting out our diverse roles in this exciting and challenging service environment.

    Notes: Community health nursing in China is an emerging specialty. A multi-component collaborative endeavor between the Schools of Nursing of Tianjin Medical University, China, and the University of Ottawa, Canada is described. This project, funded by the Canadian International Development Agency, commenced in 1989. It has laid the groundwork for an expanded role for community health nurses in Tianjin, a municipality of 11 million people located in Northeast China. The historical context for the evolution of community health nursing in China and the emergence of community health nursing as a priority area within the project are described. Major project activities are highlighted, illustrating several underlying principles for strengthening the educational preparation of baccalaureate nurses who can apply community health skills. These include creating a critical mass of faculty who can teach community health nursing, modelling classroom and clinical teaching of community health nursing, bridging the gap between nursing in the community and nursing in the hospital, and developing a prototype for baccalaureate community health nursing experience. Lessons learned from this initiative are summarized.

    Factors Associated with Cane Use Among Community Dwelling Older Adults (2000)

    Notes: Guided by the Theory of Planned Behavior (TPB), this study examined factors associated with cane use among community dwelling older adults. Data were collected in a cross-sectional survey of a convenience sample of 106 community residing older adults in Ottawa, Canada. Using a stepwise discriminant analysis, subjective norms, attitudes, and age surfaced as the key variables associated with cane use in this sample. The discriminant function accounted for 67% of the variance in cane use and correctly classified 91% of cases (Wilks's λ= 0.33, χ2= 110.12, df = 3, p 〈 0.0001). The findings provide evidence for the utility of the TPB in its application to understanding cane use behaviors of older persons and have important implications for the design of theory-based fall prevention interventions to enhance the acceptance and effective use of mobility aids.

    Notes: Abstract Falls are a common barrier to independent living among elderly persons. In recent years, growing awareness of the incidence of falls has led to the development of many community based fall prevention programs for older adults. However, the potential impact of these programs is diminished by the lack of research on factors that may influence older persons' decisions to adopt or reject fall prevention behaviors. This exploratory descriptive study employed a focus group approach to elicit qualitative data on seniors' views on the use of assistive devices in fall prevention. Four focus group interviews were conducted with a convenience sample of 30 community-living older adults from Italian- and British-Canadian backgrounds in Ottawa, Canada. The interviews documented personal experiences with and the meaning of falls, aging, and assistive device use for older adults. The findings have important implications for the public health nursing practice in the realms of individual counseling, social marketing, and policy change to prevent falls among elderly persons. The study also provides direction for future research on this topic.

    Notes: Abstract Several issues were encountered by nurse-managers in a community agency while implementing an audit of community health nursing charts. They included the following: (1) what is the purpose of the audit? (2) are the required resources available to implement the audit process? (3) what is the selection process for charts to be audited? (4) how many charts have to be audited? (5) is the audit tool based on relevant and measurable criteria and standards? (6) have the measurement properties of the audit tool been determined, including validity and reliability? The validity, reliability, and weighting of scores of the Craig audit tool are explored. Results of an assessment of this tool's reliability are presented.

    Notes: Bath grab bars can minimize the effects of many age-related deficits that may contribute to bath-related falls. Despite their potential value, bathroom safety devices remain largely underutilized by many community-living older adults and knowledge concerning attitudinal factors that influence the use of grab bars is sparse. This void of knowledge is due, at least in part, to the lack of instruments to measure the psychosocial constructs influencing bathroom safety device use. 
This study examined the psychometric properties of a newly developed Grab Bar Use Attitude scale (GUAS). Instrument formation, including item generation, evaluation by a panel of experts, and pilot testing of the draft instrument to establish its face and content validity, was followed by instrument validation using 546 community-living seniors. 
Results of principal components analysis of the GUAS revealed a two-factor solution, explaining 56% of the variance. The two constructs may best be described as functional/safety and psychosocial consequences of using grab bars. Psychometric analyses of the 9-item scale provided empirical evidence of the internal consistency of the total scale and each subscale. Finally, the GUAS distinguished between regular grab bar users and nonregular users. Implications for use are discussed.

    Notes: The purpose of this study was to describe women's smoking experience during pregnancy and postpartum. Primiparous women recruited during a randomized controlled trial to assess the effectiveness of postpartum visiting were invited to participate in a qualitative study.〈section xml:id="abs1-2"〉〈title type="main"〉Methods:A semistructured protocol guided the face-to-face interview. Interviews were audiotaped and transcribed. Data were coded independently by the authors, and the final coding system for emergent themes was developed through a consensus process.〈section xml:id="abs1-3"〉〈title type="main"〉Results:Of the 47 women invited to participate, 22 agreed to be interviewed. Three major themes emerged from the analysis: pregnancy as a context for stopping smoking, returning to smoking, and social pressures on smoking behavior. Concerns about the baby's health were cited as central reasons for stopping. Breastfeeding provided a reason for continued smoking cessation, whereas social events often demarcated a resumption of women's smoking choices.〈section xml:id="abs1-4"〉〈title type="main"〉Conclusions:Results provide guidance for the timing and content of interventions to prevent smoking relapse.
