    Notes: The cause of acute leukaemia is unknown Untreated, the disease is generally fatal after a few months The therapeutic regimen is chemotherapy, and sometimes also bone marrow transplantation, and the aim is to cure the patient Not only does the person live under pressure from a life-threatening disease with all the physiological complications that therapy can result in, the therapeutic regimen also involves a great psychological effect The concept of quality of life (QOL) is individual for each person, as is his/her experience of the value of the contents of life The principal aim of the study was to describe what QOL means to adults with acute leukaemia, not to describe the degree of QOL The second aim was to give the concept of QOL a theoretical and empirical significance relevant to nursing care Eight adults with diagnosed acute leukaemia detected in adulthood have been interviewed about their quality of life The concept was given one superior dimension in all the interviews, positive attitude to life Below the superior dimension were two further dimensions, interpersonal relationships and autonomy Different qualities were given below these dimensions, viz security, support, respect, information and conversation

    Notes: The purpose of the study was to describe nurses’ routines in connection with the planning, care and handling, and documentation of a peripheral intravenous cannula (PIV) and also to examine factors that control how nurses act. Nurses and physicians in three English hospitals were interviewed. The results showed a difference between nurses’ and physicians’ opinions about PIV routines, except in few cases. The PIV was inserted by the physicians while the nurses took care of and documented its care and handling. In many cases the insertion of the PIV was also documented by the nurses. Although small PIVs and short insertion time had previously been recommended, many physicians preferred large cannulae and an insertion time of between 3 and 7 days. All nurses and six physicians had the opinion that the cannula should be changed more often, but this did not happen. Some physicians maintained that the nurses’ care and handling varied. On the other hand some criticism had been expressed by the nurses about unclear directives from the physicians. English participants were aware that unsatisfactory routines existed and that the policy had not always been followed.

    Notes: Pressure sores are a great problem for patients, staff and society. The aim of this study was to examine the prevalence, treatment and prevention of pressure sores in a public health service area in Sweden. Criteria used for pressure sore assessment were persistent discoloration, epithelial damage and damage to the full thickness of the skin, without or with a cavity. The data were collected during 2 weeks in April 1995 from 1173 inpatients. The pressure-sore prevalence rate was 3.75%; 44 patients had a total of 68 sores. Men were as prone to developing pressure sores as women. The most frequently reported preventive measures were antidecubitus mattresses and turning schedules. Relief from pressure and occlusive dressings were the most common treatment measures. There was no statistical difference in pressure-sore prevalence when compared with a similar study from 1980. Patients were, however, older in 1995.

    Notes: A risk assessment scale for the prediction of pressure sore development: reliability and validity Background. The ability to assess the risk of a patient developing pressure sores is a major issue in pressure sore prevention. Risk assessment scales should be valid, reliable and easy to use in clinical practice. Aim. To develop further a risk assessment scale, for predicting pressure sore development and, in addition, to present the validity and reliability of this scale. Methods. The risk assessment pressure sore (RAPS) scale, includes 12 variables, five from the re-modified Norton scale, three from the Braden scale and three from other research results. Five hundred and thirty patients without pressure sores on admission were included in the study and assessed over a maximum period of 12 weeks. Internal consistency was examined by item analysis and equivalence by interrater reliability. To estimate equivalence, 10 pairs of nurses assessed a total of 116 patients. The underlying dimensions of the scale were examined by factor analysis. The predictive validity was examined by determination of sensitivity, specificity and predictive value. Results. Two variables were excluded as a result of low item–item and item–total correlations. The average percentage of agreement and the intraclass correlation between raters were 70% and 0·83, respectively. The factor analysis gave three factors, with a total variance explained of 65·1%. Sensitivity, specificity and predictive value were high among patients at medical and infection wards. Conclusions. The RAPS scale is a reliable scale for predicting pressure sore development. The validity is especially good for patients undergoing treatment in medical wards and wards for infectious diseases. This indicates that the RAPS scale may be useful in clinical practice for these groups of patients. For patients undergoing surgical treatment, further analysis will be performed.

    Notes: Meals combine human beings’ nutritional requirements with cultural needs The aim of this paper was to evaluate how individual patients’ meals were planned, carried out and followed up by nursing staff related to nutritional requirements and ability to eat in the ward Eighteen newly admitted long-term care patients hospitalized for more than 3 weeks were observed for up to 6 weeks The patients’ conditions were examined using interviews, observations, recorded data and analyses for nutritional assessment Fourteen patients experienced difficulties related to motor dysfunctions in the upper extremities, mouth and pharynx in handling food Six of those were prescribed feeding aids or nutritional supplement Biochemistry was not used as a basis for these interventions and four at risk of malnutrition on admittance were missed Thus, this study exposed defective nursing assessment strategies However, eating dependency was prevented and nutritional status tended to recover among those where interventions were made

    Notes: The purpose of this study was to analyse the actual routines surrounding the use of peripheral cannulae and the occurrence of complications Thirty surgery patients and 30 internal medicine patients were observed daily at a medium-sized hospital The patients were followed from the time the cannula was inserted until after withdrawal and until both the insertion site and the vein were free from pain The results showed that most of the cannulae were placed on the upper side of the hand and sizes most frequently used were 1-0 and 1-2 mm The fixation was unsatisfactory in 23 cases already after the second day Twenty-three cannulae were removed after 24 hours Thirty-seven (62%) patients were stated to have thrombophlebitis phlebitis in different degrees Only seven cannulae fulfilled the criteria for good cannula care and handling The frequency of complication was especially high when fructose-glucose, antibiotics or anticoagulants were given There was no documentation in the patient record, according to current laws in Sweden The complications observed were redness, swelling, haematoma, subcutaneous swelling and suppurating infection The study showed that the longer the cannula had been in situ, the greater were the complications (very distinct after 24 hours) The care and handling was unsatisfactory to very unsatisfactory in 52% of the cases Complications can last for a very long time In this research, pain was noted up to 5 months after the cannulae were removed

    Notes: The purpose of this paper is to describe the Me situation, the frequency of sick-listing and the work situation of people with ulcerative colitis and Crohn's disease who have undergone an ileostomy. The survey which was undertaken in Sweden included 53 people between the ages of 17 and 65 who had had their ileostomy for between 6 months and 25 years Personal interviews were carried out and information about the operation and complications was acquired from the medical records Half of the respondents felt themselves handicapped to some extent Despite this, most of them did not consider their daily activities to be adversely affected by the sickness/ileostomy. There were, however, individual variations depending on the sickness, how well the ileostomy worked and other factors affecting the individual's situation. Twenty-two had been off sick once or several times during the previous 2 years, nine of them had been sick-listed over a longer period. Most of them stated that their working capacity had not been affected by the operation/sickness, while 10 of them had some form of disability pension or part-tune work A positive reaction noted was that when the sickness no longer steered one's life, it became easier to plan both work and leisure time.

    Notes: The aim of this study was to investigate the relationship between the performance of breast self-examiniation (BSE) and age, place of residence, ethnic background and religion, as well as whether a group willing to take part in an interview regarding BSE mirrored the whole population. Six hundred and ninety-four (69.4%) women, aged 25–80, responded to the questionnaire. Of gthese, 69.6% examined their breasts. Whether the subjects lived in urban or rural areas was of no importance for practising BSE, nor was country of birth or number of years in Sweden. A larger proportion of women aged 45–80 practised BSE compared with women aged 25–44 (P 〈 0.001), the former practising BSE once a month or more. Step-wise logistic regression analysis showed that, for the whole sample as well as for the interview group, age was the only significant predictor of breast self-examination (P 〈 0.001). There were no significant differences between the whole sample and the interview group. The importance of younger women performing the examiniation is stressed. Dur to nurses being strategically located in a wide range of geographical locations, allowing them to meet women in different settings, they are a good choice for motivating women to prctise BSE.

    Notes: A descriptive study was carried out within a public health services area in Sweden in order to determine the incidence of pressure sores in acute and long-term care. Methods of prevention, treatment and the characteristics of patients were also noted. The criteria for the assessment of pressure sores were: skin discolouration, epithelial damage and damage to the full thickness of the skin. The data were collected by means of interviews during 1 week in February, 1980. It was found that within the public health services area there was a pressure sore frequency of 4%; i.e. 71 patients of a total of 1776 had pressure sores. Some of these patients had more than one pressure sore; the total number of sores was 109. The methods of prevention and treatment were numerous. The patients who developed pressure sores were predominantly women over 65 years of age, incontinent, and with poor mobility.

    Notes: The aim of the study was to describe the nutritional intake, meal patterns, physical activity and need for help in nine women living in their own homes and being treated for venous leg ulcers. Food habits were identified by use of interviews and food diaries completed by the women during a period of seven days. The intake of energy and nutrients from 304 eating events during seven days was calculated and meal patterns were evaluated using a qualitative system for meal classification. Physical activity and the degree of need were identified with the help of interviews. The intakes of energy and key nutrients for wound healing, such as protein, vitamin C and zinc, were not optimal according to the Swedish nutrition recommendations, although food habits were well organized. Most of the women had hardly any physical activities and the need of help and support varied, from daily visits to visits every second week.
