    Notes: A 10 year-old boy presenting with an empyema and a previous medical history of recurrent and disseminated infections was investigated immunologically. The serum immunoelectrophoresis showed hypogammaglobulinaemia. The in vitro lymphocyte proliferation to T-cell mitogens (PHA and Con A) was diminished and the response to a T-dependent B cell mitogen (PWM) was severely diminished. Phenotypic analysis of his lymphocytes with monoclonal antibodies revealed normal CD3+, CD4+ and CD8+ cells with a normal CD4: CD8 ratio. Double immunofluorescence with Leu 8 antibody revealed a selective deficiency of the CD4+ LAM 1− subset. Co-culture experiments between patient's B cells and supernatants from normal T cells showed that the patient's B cells could secrete Ig in vitro. We suggest that the Leu 8 monoclonal antibody may be useful in defining abnormalities in the regulatory subsets of T lymphocytes and that these abnormalities may be relevant in the pathogenesis of common variable immunodeficiency.

    Notes: Abstract. Ultrathin metal films consisting of two-dimensional clusters are typically unstable: the cluster ensemble has the tendency to reduce its total free energy via Ostwald ripening or dynamic coalescence of mobile clusters. In this paper we give an overview of recent model experiments addressing these coarsening mechanisms. The experiments have been performed using STM on ensembles consisting of adatom or vacancy clusters with typical diameters in the nanometer range on fcc(111)-metal surfaces. Agreement with and deviations from conventional theories are discussed.

    Notes: Zusammenfassung Es wird über eine in vitro-Methode für den autoradiographischen Nachweis des Protein- und RNS-Stoffwechsels der Haut berichtet. Die bisherigen Untersuchungen an der gesunden Epidermis nach Inkubationszeiten der Tracer von 0,5–8 Std zeigen, daß der Histidin-Einbau vorwiegend im Str. granulosum, der Uridin-Einbau hingegen vorwiegend im Str. basale erfolgt. Hieraus wird gefolgert. daß die Proteinsynthese im Str. granulosum wohl nicht auf einer RNS-Syntheseleistung beruhen kann, da der Protein- und RNS-Aufbau in keinem entsprechenden Verhältnis zueinander steht. Diese Methode verspricht weiteren Aufschluß über allgemeine und pathologische Stoffwechselvorgänge der Haut.

    Description / Table of Contents: Zusammenfassung Bei autoradiographischen Untersuchungen, deren in vitro-Methode beschrieben wird, wurde dem Nährmedium, in dem sich Stücke normaler und psoriatischer Haut befanden, neben den Tracern H-3-Histidin und H-3-Uridin Vitamin C und Folsäure zugesetzt. Der Einfluß der Vitamine auf die Radioaktivität als Nachweis des RNS- und des Protein-Stoffwechsels in den verschiedenen Epidermisschichten wurde mit vitaminfreien Kontrollen verglichen. Dabei zeigte sich durch Vitamin C eine Hemmung sowohl des Protein- als auch des RNS-Stoffwechsels bei gesunder und psoriatischer Haut. Bei Folsäurezusatz war die Wirkung nicht einheitlich. Der RNS-Stoffwechsel der normalen Haut wurde nicht beeinflußt, während er in der psoriatischen Haut sogar gesteigert wurde. Dagegen wurde in normaler Haut der Protein-Stoffwechsel durch Folsäure gebremst, dies war bei der psoriatischen Haut nicht der Fall.
    Notes: Summary In vitro experiments with normal and psoriatic human skin were carried out. H-3-histidine resp. H-3-uridine as radioactive tracers and vitamin C resp. folic acid were added to the medium. The influence of the vitamines on radioactivity as a proof of the RNA- and protein metabolism in the different layers of epidermis was compared with controls. Vitamin C inhibited as well the protein metabolism as the RNA metabolism in normal and psoriatic human skin. The effect was different when folic acid was added. In normal human epidermis there was no influence on the RNA metabolism, whereas it was even increased in psoriasis. On the other side folic acid inhibited the protein synthesis of normal human epidermis but had no influence on psoriatic skin.

Thoraxtraumatisierte profitieren in besonderem Maße von einer adäquaten, zeitgerecht initiierten und konsequent fortgeführten notärztlichen Therapie. Gerade aber beim stumpfen Thoraxtrauma ist die präklinische Beurteilung der Verletzungsschwere mit einem hohen Grad an Unsicherheit verbunden. Ziel der vorliegenden Studie war es, an einem Kollektiv von 255 Patienten den Nutzen eines adjuvanten präklinischen pulsoxymetrischen Monitorings hinsichtlich einer Optimierung der notärztlichen Diagnostik sowie Therapie beim stumpfen Thoraxtrauma zu untersuchen. Die Ergebnisse dieser Untersuchung lassen den Schluß zu, daß die Pulsoxymetrie in Ergänzung zu der rein klinischen Beurteilung des Notfallpatienten eine wertvolle Hilfestellung bei der notärztlichen Diagnostik der Lungenkontusion sowie bei der Detektion eines Spannungspneumothorax leistet. Die Kombination von klinischer Untersuchung und pulsoxymetrischem Monitoring erlaubt demnach bereits präklinisch eine deutlich differenziertere notärztliche Diagnostik des stumpfen Thoraxtraumas, als dies bisher möglich war und schafft die Voraussetzungen für eine Optimierung der präklinischen Therapie.
    Notes: Summary. Chest trauma cases benefit to a great degree from adequate, timely initiated and consistent prehospital treatment. However, prehospital determination and evaluation of blunt chest trauma is associated with a high degree of uncertainety. The purpose of our study was to examine and determine, in a collective of 255 trauma patients, the value of additional prehospital pulse oximetric monitoring regarding the optimization of the emergency physician's diagnosis and treatment of blunt chest trauma. We conclude from our findings that, in addition to the physical examination, with the application of pulse oximetry the prehospital diagnosis of lung contusion as well as the early detection of tension pneumothorax are more accurate. Therefore, the combination of physical examination and pulse oximetric monitoring is a requirement for the optimization of prehospital diagnosis and therapy in blunt chest trauma.
