    Notes: Abstract Surgical specimens from 60 Japanese women with breast cancer were studied histochemically to detect estrogen receptors (ER). Forty-six were analyzed in a double blind study designed to compare the histochemical method using fluorescent estradiol conjugate for detection of ER (FITC method) with findings in case of the currently employed dextran coated charcoal (DCC method) techniques. The ER test was positive in 39 (65 percent) of the 60 cases studied by FITC method. Of the 46 cases analysed in a double blind study, 41 (89.8 percent) showed the same results. Of the 5 with inconsistent test results, 2 showed positive DCC and negative FITC, and 3, negative DCC and positive FITC. According to the histologic type, the FITC method demonstrated a low positive ER response rate in scirrhous cases (57 per cent), whereas the positive rate was 67 percent among papillotubular-type cases and 65 percent among medullary tubullar-type cases. In relation to menstrual status, the positive rates were 68 percent and 64 percent among premenopausal and postmenopausal women, respectively. However, in relation to tumor size, no difference was noted in the positive response rate by either method. Therefore, both the DCC and FITC methods are clinically useful to determine the indications for hormone therapy.

    Notes: Abstract A 37-year-old male with adrenocortical tumor presenting marked gynecomastia and impotence as initial symptoms was experienced. Abnormal elevations in urinary 17-KS, 17-OHCS and blood estrogen levels were noted. Total surgical removal of the tumor weghing 800g was performed. Postoperatively the gynecomastia disappeared and he regained libido with no signs of recurrence one year later.

    Notes: Abstract Two patients with thyroid carcinoma infiltrating bilateral internal jugular veins were treated. In reconstruction of the internal jugular vein by implantation of an autogenous venous segment or Gore-Tex artificial vessel, the repaired area soon became obstructed. The repaired area with the Impra-Flex artificial vessel became obstructed again one month after the implantation. In the reconstruction by ipsilateral end-to-end anastomosis between the internal and external jugular veins, good circulation was apparent even 2 years after the operation. The internal-external jugular vein anastomosis is expected to increase the safety of single-stage bilateral excision of the internal jugular vein, as an adjunct to total thyroidectomy, in the surgical treatment of thyroid carcinoma.

    Notes: Abstract Using adult mongrel dogs, a portion of the anterior gastric wall of the pyloric gland area was transplanted into the fundic gland area using the Z-plastic procedure forming an islet. Three weeks postoperatively, 2 per cent histamine in sesame oil was injected intramuscularly in a daily dose of 2 mg/kg body weight. Ulcers developed in the graft in nine of 19 animals. The factors responsible to the development of the ulcers may well be circulatory disturbance and reduction of mucosal resistance following surgical insult, and continuous irritation caused by gastric juice. When a portion of the anterior gastric wall of the fundic gland area was transplanted into the fundic gland area followed by administration of histamine in sesame oil, ulcers developed in the graft only in four of 12 animals. Histological examination revealed the regenerated epithelium at the margin of the ulcers. Thus the ulcers presently induced may provide an useful experimental model for ulcers in the fundic gland area.

    Notes: Abstract To clarify the neural and humoral factors which control gastric receptive relaxation, the effects of various types of vagotomy or splanchnicotomy on receptive relaxation of the canine innervated corpus pouch were investigated. The influence of gastrin, histamine H1-receptor antagonist, and anti-histamine releasing agents on receptive relaxation were also examined. Selective proximal vagotomy or splanchnicotomy resulted in a slight disturbance in receptive relaxation. Truncal vagotomy produced a marked disturbance in receptive relaxation. Administration of tetragastrin potently inhibited receptive relaxation, however, recovery occurred by injecting a histamine H1-receptor antagonist. Increased serum gastrin induced by perfusion of the antrum with liver extract solution disturbed receptive relaxation and this response was inhibited by pretreatment with transamine, an inhibitor of histamine release from histamine secreting cells. These results indicate that receptive relaxation in the canine stomach is controlled not only by vagal and splanchnic nerves, but also by gastrin and histamine.

    Notes: Abstract We treated 44 patients with multiple primary cancers, at the Second Department of Surgery, Hiroshima University, during the 14 year period from 1968–1981. The total number of malignant tumors was 1,934 during this period, thus the incidence of multiple primary cancers was 2.3 per cent. Nine of the 44 were atomic bomb survivors in Hiroshima and were characterized by the frequent incidence of the second tumor about 30 years after exposure to the atomic bomb. In addition, the second tumor tended to develop later in patients prescribed radiotherapy for the first tumor. As for the site of tumor, there was no tendency of more frequent development of tumors in organs highly sensitive to radiation other than the thyroid.

    Notes: Abstract Microscopic cancer is extremely rare even among “occult breast cancers”. We treated a 70 year old woman with complaints of serosanguineous nipple discharge which yielded cytologically Class V cancer cells. Microdochectomy was performed and intraductal carcinoma was verified histologically in serially cut sections. Following a simple mastectomy, she has been well with no evidence of recurrence or metastasis, at this time.

    Description / Table of Contents: Résumé L'anastomose termino-terminale du nerf récurrent après blessure chirurgicale a été abandonnée en raison de l'échec de la régénération par la majorité des chirurgiens. Les auteurs ont remarqué cependant que si les mouvements des cordes vocales ne peuvent être rétablis la tentative de restoration de la continuité du nerf donne de meilleurs résultats que l'abandon des deux extrémités sectionnées. La raucité de la voix s'améliore après la tentative d'anastomose parce qu'elle prévient l'atrophie de la corde vocale. L'anastomose termino-terminale après blessure du nerf laryngé inférieur mérite donc d'être toujours tentée.
    Notes: Abstract End-to-end anastomosis of the recurrent laryngeal nerve had been abandoned because movement of the vocal cords could not be restored because of misdirected regeneration. However, the authors discovered that, although vocal cord movement could not be restored, phonation was better in those in whom the recurrent laryngeal nerve was anastomosed than in those in whom it had been left in the severed state. Hoarseness improves after nerve anastomosis because it prevents vocal cord atrophy and the vocal cord gap is smaller. End-to-end anastomosis of the recurrent laryngeal nerve is, therefore, considered to be of value.
