    Notes: Objectives To evaluate the efficacy of treatment of plaque-type psoriasis with a moderate and a strong corticosteroid under continuous hydrocolloid occlusion, with weekly change of dressing, compared with treatment with the same corticosteroids alone, and the acceptance by the patients of his type of treatment.Setting Dermatological Departments of two University Hospitals and four General Hospitals.Design Open parallel study divided in two parts depending on the two different corticosteroid preparations applied. Each part was an intrasubject half-side study with left-right comparison.Subjects Fifty five patients with symmetrical, stable psoriasis plaques on elbows or knees.Methods Twenty nine patients were treated with clobetason butyrate cream (EmovatcR) twice daily as well as once weekly covered with a hydrocolloid dressing (ContreetR) for four weeks. Twenty six patients were treated with clobetasol propionate cream (DermovateR) twice daily as well as once weekly covered by the hydrocolloid dressing for 2 weeks. Induration, erythema and scaling were assessed using a four-point scale. Plaque size was measured by multiplying the largest and the smallest diameter. Healing was defined as score 0 for scaling and induration and score 0 or 1 for erythema. Acceptance was evaluated by questions related to the wearing of the dressings: comfort of wearing, pain, itching, adhesiveness and sticking to clothing, and the performance of daily activities.Statistics analysis Sign test for the score values, parametric paired Student t-test for plaque size, chi-square test for healing.Results With occlusion a statistically significant increased reduction of severity was obtained compared to treatment without occlusion; this effect was more pronounced with the application of the strong corticosteroid. The questions related to the wearing of the dressing were answered positively by the majority of the patients.Conclusions Treatment of psoriasis plaques with, especially strong, corticosteroid creams under occlusion with a hydrocolloid dressing with a high adherence is rapid and effective. The acceptance of this type of treatment by the patients is high, and it permits normal daily activities.

    Notes: Summary A patient is described who developed a granulomatous skin reaction to injections of protamine-insulin for the treatment of her diabetes mellitus. By means of skin testing it was shown that this reaction was caused by protamine. No adverse effects were seen when she was injected with insulin which did not contain protamine.

    Notes: Sensory testing by means of a graded bristle technique was performed following cryosurgical treatment of 183 skin lesions in 169 patients.Mild, generally transient, sensory loss was found in 28% of the treated lesions. This sensory loss appeared not to be influenced by differences in the cryosurgical procedures nor by postoperative complications in wound healing, but was dependent on the area of the body where cryosurgery was performed. Treated lesions on the trunk and neck and on the arms and legs showed the most profound and longest lasting sensory loss.

    Notes: Seven patients who developed a characteristic pruritic rash in late pregnancy are described. Clinically the lesions consisted of erythematous urticarial papules and plaques, with vesicles in some cases. The eruption started in the striae on the abdomen and spread over the thighs and in most cases also over the arms and buttocks. Biopsy showed lymphocytic vasculitis with a varying admixture of eosinophils and only minor epidermal changes. Immunofluorescence examination of six cases showed C3 deposition along the dermoepidermal junction in only one case. The lymphocytes, identified by a specific anti-T-cell serum, appeared to be mostly T-cells.The nomenclature of pruritic skin disease in late pregnancy is confusing, but the term ‘late prurigo of pregnancy’ seems appropriate for this condition.

    Notes: Neopterin, a product of γ-interferon-activated macrophages, was measured in sera from 28 patients (12 patients with cutaneous leishmaniasis and 16 patients with visceral leishmaniasis) to determine the utility as a marker of disease activity and therapeutic efficacy. Patients originated from Kenya (n=5) and from the Academic Medical Center, Amsterdam, The Netherlands (n=23). In seven patients follow-up sera after treatment were available. Two patients at the time of diagnosis of visceral leishmaniasis were co-infected with HIV. The 12 patients with cutaneous leishmaniasis had serum neopterin levels below the upper limit of the normal range. All 16 patients with visceral leishmaniasis had elevated levels of serum neopterin before treatment. In six out of seven patients with visceral leishmaniasis followed during treatment neopterin levels decreased to values below the upper limit of the normal range (10 nmol l−1). Sequential measurements of serum neopterin levels may be useful for monitoring therapeutic efficacy in patients with visceral leishmaniasis.

    Notes: Reactions, a relatively common phenomenon among leprosy patients in treatment, require early detection and proper management to prevent serious sequelae. It is generally accepted that reactional states are immunologically mediated and, as such, usually improve with immunomodulatory treatments such as corticosteroids or thalidomide. Neopterin, a product of γ-interferon-activated macrophages, is a marker for cell-mediated immune activation and may be useful to detect reactional states in leprosy. Here, we compared neopterin levels in single serum samples from leprosy patients with and without reaction with untreated controls and, when available, serial samples among patients with and without reaction. Levels in the single sample measurements, conducted in 22 patients with a reversal reaction (mean 14.5 nmol l−1, S.D. 8.7) and 13 with erythema nodosum leprosum (mean 16.9 nmol l−1, S.D. 13.6), were significantly higher (P=0.02 and P=0.001, respectively) than levels in 26 untreated patients (mean 9.1 nmol l−1, S.D. 7.3). Values above the upper limit of normal (10 nmol l−1) were found in seven of 26 untreated patients, 14 of the 22 reversal reaction patients (P=0.01) and 10 of the 13 ENL patients (P=0.003). Serial serum samples, obtained from six patients that developed reactions and 14 that remained free of reaction, indicated that reversal reaction or erythema nodosum leprosum paralleled a concomitant increase in the serum neopterin level. Neopterin levels generally declined upon corticosteroid therapy. Neopterin may be a useful marker for reactional states in leprosy by providing a laboratory parameter to assess the onset, progression, response to therapy and resolution.
