    Notizen: From a group of 374 patients with ulcerative colitis who underwent colectomy and ileo-rectal anastomosis between 1952 and 1976, 104 presented for follow-up rectal biopsies over a 5 year period to 1986. These patients have been followed for an average of 28 years since the onset of the disease (range 11–56) and provide a unique model for the study of the development of carcinoma in colitis. A total of 443 biopsies was examined and epithelial morphology assessed according to the classification of Riddell et al. (1983). Five patients developed carcinoma of whom two showed dysplasia in biopsies taken a year or more before the diagnosis; one showed dysplasia 2 months before the appearance of cancer, another synchronous with it, and in the fifth patient dysplasia was not detected. Of 20 biopsies classified as ‘indefinite for dysplasia, probably negative’, 80% were associated with subsequent resolution, while of nine ‘indefinite for dysplasia, probably positive’ biopsies, eight (89%) were subsequently associated with either carcinoma (six) or dysplasia (two). The results emphasize the danger of absolute reliance upon dysplasia in assessing individual cancer risk in colitics, appear to demonstrate the usefulness of Riddell's classification and provide additional information on the natural history of dysplasia.

    Notizen: Colectomy specimens from 6 cases of familial polyposis coli (FPC) were examined. Macroscopically normal mucosa between polyps was taken at intervals along the ascending, transverse and descending colons and studied at EM level. All three regions show the same cell populations varying only in their ration and height of the epithelium. The same cell types seen in the normal crypt epithelium are present in teh mucosa from FPC. As in the normal, ‘intermediate’ cells are present in FPC but they are less conspicuous than in the mucosa adjacent to colonic carcinoma. However, there are ultrastructural features which distinguish the ‘normal’ mucosa in FPC from the true normal: (a): the presence of ‘clear columnar’ cells; (b) increased vesiculation in the absorptive cells; (c) large number of lysosomes; and (d)the presence of small electron-dense bodies. The ultrastructure of the ‘clear columnar’ cells suggests their being incompletely differentiated cells, thus raising the possibility that their presence in the upper crypt and surface epithelium may represent an early stage in the process of polyp and cancer formation.

    Notizen: Gastrectomy specimens including 32 cases of carcinoma and 35 cases of peptic ulcer were examined in detail histologically and histochemically to show the distribution of intestinal metaplasia (IM) and types of mucin secreted. A variant of IM, with extensive involvement of both antrum and body and excessive secretion of sulphomucin, was found to be associated with carcinomas showing appreciable extracellular sulphomucin secretion. The identification of such a variant of IM may help the interpretation of gastric biopsies.

    Notizen: Immunohistochemistry using the PC10 antibody to proliferating cell nuclear antigen (PCNA) was applied to archival material from mucosa adjacent to gastric carcinoma (‘normal’, hyperplasia, complete and incomplete intestinal metaplasia and dysplasia) and non-cancer controls (normal and complete intestinal metaplasia). Overall, increased PCNA indices, with expansion and altered location of the proliferative zones, were observed in carcinoma fields and compared with controls (P≤0.001). These differences were particularly significant in ‘normal’ mucosa far from carcinoma as compared with normal in controls (P≤0.001). In carcinoma ‘fields’ distinct patterns of PCNA expression were noted in complete and incomplete intestinal metaplasia. Similarly, in dysplastic lesions high PCNA indices were present either throughout the gland or found predominantly in the upper compartment. We conclude that these differences in PCNA index and staining patterns might prove useful in monitoring the evolution of the disease in the follow-up of patients at risk of developing gastric cancer.

    Notizen: We have explored the merit of a simultaneous study of sialomucin content and carcinoembryonic antigen (CEA) expression in the identification of early malignancy in adenomas. One hundred and thirteen colorectal adenomas were investigated by histochemical and immunocytochemical techniques. We compared adenomas from ‘high risk’ patients having synchronous carcinoma and ‘low risk’ groups with incidental polyps only. Twenty-three metaplastic and eight inflammatory polyps were also included. Our data suggest that size and dysplasia are not always closely related and that synchronous adenomas seem to carry a higher malignant potential than incidental polyps irrespective of size. The degree of O-acylation of sialic acids appears to be a sensitive indicator of early malignant change: loss of O-acylation was seen in all 11 adenomas with highly atypical foci (‘focal carcinoma’) but noted in only four of the remaining polyps with lower grade dysplasia (P〈 0.005). By contrast the intensity of staining for CEA was a gradual phenomenon and showed no statistically significant increase with the onset of malignancy. Inflammatory polyps showed staining characteristics similar to normal mucosa. Metaplastic polyps, however, revealed increased expression of CEA and reduced O-acylation with increased size which may reflect a disorder of growth and differentiation. Finally, by comparing these profiles of staining with those of normal mucosa,‘transitional’ mucosa adjacent to carcinoma and carcinoma, we further illustrate a progression of changes occurring in colonic mucosa in carcinogenesis.

    Notizen: Synopsis A periodic acid-chromic acid-silver methenamine technique for visualizing glycoproteins at the electron microscope level was applied to colonic mucosa taken from areas adjacent to and remote from carcinoma. Normal control mucosa was obtained by biopsy of patients with no known gastrointestinal disease. Non-oxidized control sections were run in parallel. Quantitative and qualitative differences in glycoproteins were detected in the muscosa adjacent to carcinoma (‘transitional’ mucosa, as we call it) as compared with the normal. Furthermore, the vesicles in both the ‘intermediate’ and absorptive cells elaborate a glycoprotein product and it seems that a direct relationship exists between the increased vesiculation and the markedly developed ‘fuzzy coat’ in the ‘transitional’ mucosa. It is suggested that these findings may represent one of the features of an early stage of carcinogenesis. Histochemical and ultrastructural techniques of the kind used in this study may thus be of value in identifying or predicting malignancy in the colonic epithelium.

    Notizen: Summary The alterations in carbohydrate metabolism which occur in the distal colon of rats during carcinogenesis induced by dimethylhydrazine were investigated using [3H]galactose and [3H]fucose as glycoprotein precursors. A statistically significant decrease in [3H]galactose uptake was observed in dysplastic epithelia. These findings are consistent with the alterations in mucin composition with predominance of sialomucins shown in these areas by histochemical methods. Furthermore, changes in the gradient of [3H]galactose incorporation along the crypt epithelium were also found in the histological and histochemically non-involved colonic mucosa of dimethylhydrazine-treated rats, as compared with controls. No significant variations were seen in [3H]fucose incorporation. These results correlate well with our previous histochemical observations and are further evidence of the profound alterations in glycoprotein synthesis affecting the whole colonic mucosa during carcinogenesis.
