    Description / Table of Contents: Résumé A partir d'une étude prospective portant sur 30 fémurs droits ou gauches prélevés de façon continue au laboratoire d'anatomie, les auteurs étudient les dimensions du canal endosté de l'extrémité supérieure du fémur. Les fémurs sont analysés par 12 coupes scanner, en-dessous du petit trochanter. Les coupes ont été numérisées et les paramètres dimensionnels ont été calculés par l'ordinateur, grâce à un protocole original. Les résultats font l'objet d'une analyse descriptive et d'une analyse corrélative. Sur ces bases dimensionnelles, il apparaît que l'anatomie de l'extrémité supérieure du fémur n'est pas aléatoire ; il existe des canons morphotypiques ; une équation mathématique régit les lois d'un canon normal.
    Notes: Summary Based on a prospective study of 30 right or left femurs removed in a continuous series in the anatomy deparment, a study was made of the dimensions of the endosteal canal of the upper end of the femur. The femora were analysed by means of 12 CT scans below the lesser trochanter. The sections were numbered and the dimensional parameters calaculated by computer, using an original plan. The results were subjected to descriptive and correlative analysis. From these dimensional findings it appeared that the anatomy of the upper end of the femur is not random; morphotypic models exist and the laws of a normal model are regulated by a mathematical equation.

    Description / Table of Contents: Résumé Il existe au sein de la population une grande variabilité dans l'anatomie du canal endosté de l'extrémité proximale du fémur. Cette variation de l'anatomie pourrait compromettre le résultat de l'arthroplastie totale de hanche lorsque le chirurgien utilise une tige fémorale sans ciment. En effet, si la fixation secondaire d'un implant reste sous la dépendance de nombreux paramètres, il faut avant toute chose obtenir une excellente stabilité primaire, obtenue par l'adéquation morphologique os-implant. Les auteurs se proposent de définir les indications des prothèses fémorales sans ciment, en matière d'arthroplastie totale de hanche, en définissant deux paramètres radio-anatomiques : l'Index Cortico-Médullaire et l'Index d'Evasement Fémoral.
    Notes: Summary There is within the population a considerable variation in the endosteal canal anatomy of the proximal extremity of the femur. This anatomical variation might jeopardize the result of the total hip arthroplasty when the surgeon uses a cementless femoral shaft. Inded, if the secondary fixation of an implant stays under the dependency of many parameters, one has to obtain before anything else a very good primary stability, obtained by the morphological appropriateness of the bone-implant. The authors propose to define the cementless indications, as regards total hip arthroplasty, defining two radio-anatomical parameters: the Cortico-Medullary Index and the Femoral Flare Index.

    Notes: Conclusion La méthode de prévention irremplaçable des accidents vasculaires iatrogéniques, survenant lors d'une chirurgie de hanche, reste le bilan vasculaire préopératoire. En cours d'intervention, certaines mesures chirurgicales doivent être suivies, notamment si les repères osseux sont modifiés. L'aide du quadrillage acétabulaire permet de positionner en toute sécurité les vis de fixation, en évitant les quadrants antérieurs et la zone centrale de l'acétabulum. Le forage des trous ou des plots d'ancrage doit se faire selon une direction perpendiculaire à la surface semi-lunaire afin de guider la mèche en territoire connu. Il convient, par ailleurs, de ne pas trop dépasser le plan de la corticale interne lors du forage, et d'implanter des vis dont la longueur ne dépasse pas de 10 mm l'épaisseur osseuse mesurée, au risque de menacer des structures vasculaires majeures même dans les quadrants postérieurs. Il faut par ailleurs souligner que le respect de la corticale interne est tout aussi important lors du scellement d'une cupule, la présence de ciment dans le petit bassin pouvant avoir les mêmes conséquences délétères sur les vaisseaux. Ainsi la connaissance des trajets vasculaires et de leur projection sur l'acétabulum permet une implantation en toute sécurité des implants acétabulaires lors des arthroplasties totales de hanche. Enfin, elle explique parfaitement la corrélation que l'on trouve dans les fractures du bassin, entre l'importance des lésions osseuses et les lésions vasculaires tronculaires observées.

    Description / Table of Contents: Résumé A partir de deux incidences radiologiques orthogonales, les auteurs proposent une étude morphométrique de l'art. talo-crurale. Sur une population de 50 sujets non pathologiques, dix paramètres ont été mesurés et répartis en trois groupes : le groupe des paramètres de la surface articulaire distale du tibia, le groupe des paramètres malléolaires, le groupe des paramètre du talus. Ces paramètres ont fait l'objet d'une analyse descriptive et d'une analyse corrélative. Si l'art. talo-crurale est une ginglyme dont la surface articulaire du talus peut être simplifiée et assimilée à un segment de cylindre, les auteurs, par le calcul, peuvent en définir le rayon de courbure. L'analyse corrélative permet alors de définir les paramètres du talus et les paramètres correspondant de la surface articulaire distale du tibia. Les paramètres malléolaires en restent indépendants. Ce travail est une première analyse morphologique pour concevoir une arthroplastie de cheville.
    Notes: Summary Based on two orthogonal radiologic views, the authors present a morphometric study of the talocrural joint. In 50 normal subjects, 10 parameters were measured and divided into 3 groups: the distal tibial joint surface parameters, the malleolar parameters and the talar parameters. These parameters were treated in both a descriptive and a correlative analysis. If the talocrural joint is a hinge joint whose talar articular surface can be simplified and classed as a cylinder segment, it is possible to calculate its curve radius. Then the correlative analysis allows to define the talar parameters and the corresponding parameters of the distal tibial joint surface. The malleolar parameters are independent factors. This study is the first morphologic analysis to serve as a basis for an ankle arthroplasty.

    Notes: Abstract Most direct vascular trauma occurring during hip surgery results from injury to pelvic vascular structures which are not visible during the procedures of reaming, drilling holes or the fixation of screws. In this study, 5 pelves of fresh cadavers were injected with a radiopaque mixture and were visualised with a scanner according to 5 predetermined sections. Bone depth of the acetabulum was measured in each section. A calculation was made describing the minimal distance separating the inner cortex from the principal pelvic vessels. After an anatomic dissection of each pelvis, the relationship between the vessels and screws of the fixation cup, implanted identically on the quadranted acetabulum, was observed. The screws placed in the anterior and inferior quadrants and the center of the acetabulum endangered the external iliac v. and a. and the obturator pedicle. The depth of the periacetabular bone was greater in the superior and posterior quadrants. The inferior gluteal, pudendal and superior gluteal aa. were more than ten mm from the posterior wall. Conversely, the external iliac and obturator pedicles came in contact with the osseous surface on which they lay. A projection of the vessels on the acetabulum was made, and the reproducible character of the acetabular-quadrant system was verified. The superior quadrant offers all the characteristics of a vascular safe zone. A knowledge of these anatomic relationships explain vascular trauma in pelvic fractures and helps to prevent vascular injury in hip surgery.

    Description / Table of Contents: Résumé En chirurgie prothétique de l'articulation coxo-fémorale, il est fondamental de recentrer la hanche pour recréer la balance du muscle moyen fessier. La détermination du centre de rotation de la hanche est difficile si les deux articulations coxo-fémorales sont détruites. A partir de 70 radiographies du bassin de face debout, les auteurs mesurent la position du centre de la tête fémorale par rapport à la ligne de Kohler et par rapport à la ligne TD, unissant les deux U radiologiques. Sur cette même radiographie, ils définissent des index horizontaux et des index verticaux. L'analyse descriptive de ces paramètres met l'accent sur l'influence du sexe sur les coordonnées du centre de rotation de la hanche. Les auteurs mettent en évidence des corrélations statistiques entre les coordonnées de ce centre de rotation de l'articulation et certains index horizontaux et verticaux. Il en résulte que la connaissance de ces index suffit à définir la position théorique du centre de rotation. La méthode est comparée aux autres principes énoncés dans la littérature. Le procédé trouve sa place en clinique, dans le cadre de la planification pré-opératoire des reconstructions prothétiques de l'acétabulum, chaque fois qu'il est détruit de façon bilatérale (séquelle traumatique, luxation congénitale, descellement acétabulaire, etc.).
    Notes: Summary In prosthetic surgery of the hip joint it is essential to recenter the hip to recreate the leverage of the gluteus medius muscle. Determination of the center of rotation of the hip is difficult if both hip joints have been destroyed. On the basis of 70 frontal radiographs of the hip in the standing position, the authors measured the position of the center of the femoral head in relation to Köhler's line and the TD line joining the two radiological Us (“tear-drops”). On the same radiograph they defined the horizontal and vertical indices. The descriptive analysis of these parameters stresses the influence of sex on the coordinates of the center of rotation of the hip. The authors demonstrated the statistical correlations between the coordinates of this center of rotation of the joint and certain horizontal and vertical indices. It emerged that knowledge of these indices suffices to define the theoretic position of the center of rotation. The method is compared with other principles set out in the literature. The procedure has a clinical application in the context of preoperative planning of prosthetic reconstruction of the acetabulum, whenever it is destroyed bilaterally (after trauma, in congenital dislocation or acetabular loosening, etc).
