    Notes: The serum immunoglobulin levels of 200 first-degree relatives and 23 spouses of 32 patients with myelomatosis were compared with those of age- and sex-matched control persons. First-degree relatives of myeloma patients appeared to have higher serum levels of IgG (P 〈 0.05), IgA (P 〈 0.05), and IgM (P 〈 0.01) than their age- and sex-matched controls. The spouses as a group did not differ from their controls in this respect. No statistically significant difference in the incidence of M-components among family members (0.6%) and spouses (4.3%) was found compared with the incidence in the general population (0.9%). Myelomatosis as such appeared to be more frequent than in the general population (P 〈 0.05).

    Notes: Bronchial inflammation plays a central role in asthma. We investigated whether parameters of inflammation were increased in peripheral blood. Furthermore, we tested whether fluticasone propionate (FP), a new inhaled corticosteroid (ICS), and beclomethasone dipropionate (BDP) affected these parameters. FP 750 μg/day and BDP 1500 μg/day were compared in a randomized, crossover study consisting of two 6-week treatment periods, each preceded by a 3-week placebo period. Twenty-one patients with symptomatic asthma completed the study. The results were compared with those of six normal subjects (controls). Immunophenotyping of inflammatory cells was performed in whole blood, and serum eosinophil cationic protein (ECP) was measured. With regard to clinical efficacy, ICS increased PCjo histamine by more than 1.9 doubling doses and FEV, by more than 0.34 1. The number of CD3/HLA-DR+ lymphocytes was significantly increased in asthmatics compared to the normal subjects, both after placebo (P〈0.01) and after therapy (P〈0.05). The CD3/HLA-DR-H lymphocytes decreased significantly after treatment with FP (P〈0.05). Serum ECP was elevated in patients without ICS and decreased after treatment with BDP (P〈0.001). In conclusion, the number of CD3/HLA-DR-I- lymphocytes and serum ECP levels were raised in the peripheral blood of symptomatic asthmatics, and decreased by clinically effective doses of ICS. In this respect, FP 750 ng/day was at least as effective as BDP 1500 μg/day.

    Notes: Abstract The quality of voice after radiotherapy is generally considered to be better than that after surgery for early glottic (Tla and Tlb) carcinomas. Studies concerning voice quality after radiotherapy are scarce, and results have been contradictory concerning actual normalization of voice following therapy. This study was designed to compare several voice parameters of patients successfully treated 1–12 years previously with radiotherapy (5750–7000 cGy) for early glottic carcinoma. Parameters involved an age- and sex-matched control group. Results showed that voice quality following radiotherapy was less than normal for maximum vocal intensity, dynamic vocal intensity range, jitter, and mean fundamental frequency. These findings showed that voice following radiotherapy could not be considered normal.

    Description / Table of Contents: Zusammenfassung Bei 59 Patienten mit Yersiniaassoziierter Spondylarthropathie wurde während 15 Monaten der Verlauf der IgA- und IgG-Antikörperantwort auf plasmidkodierte, äußere Membranproteine untersucht. Anfangs hatten alle Patienten spezifische IgA- und IgG-Antikörper gegen die 36 und 46 kDa und auch gegen die 26 und 58 kDa-Proteine, und dies korrelierte mit dem Nachweis virulenter Yersinien in Darmbiopsien. Bei 69% der unbehandelten Patienten war IgA nach neun Monaten verschwunden, bei 31% fand sich nach einem Jahr noch eine Persistenz. Bei Patienten, die vier bis sechs Wochen lang mit Antibiotika behandelt wurden, waren IgA- Antikörper innerhalb drei bis sechs Monaten in 81% der Fälle verschwunden; Persistenz nach einem Jahr war in 6% der Fälle nachzuweisen (p〈0,002). IgG-Antikörper gegen die 36 und 46 kDa-Proteine persistierten bei 80% aller Patienten. Wenn IgA verschwand, war Yersinia auch nicht mehr in Darmbiopsien nachzuweisen.
    Notes: Summary The IgA and IgG antibody response to plasmid-encoded outer membrane proteins was studied in 59 patients with yersinia-associated spondylarthropathy during 15 months of follow-up. Initially, all patients had specific IgA and IgG antibodies to the 36 and 46 kDa and 30% also to the 26 and 58 kDa released proteins, which correlated with the finding of virulentYersinia bacilli in intestinal biopsies. IgA disappeared in 69% of untreated patients after nine months and persisted in 31% after one year. IgA disappeared within three to six months in 81% of the patients treated with antibiotics for four to six weeks and persisted in 6% after one year (p〈0.002). IgG antibodies to the 36 and 46 kDa outer membrane proteins persisted in 80% of all patients. Disappearance of IgA was coupled with disappearance of yersinia from intestinal biopsies.

    Notes: Summary A native from Surinam (South America), known to have a chronic gonarthritis for 20 years, died from an acute disseminated histoplasmosis. His gonarthritis was retrospectively shown to have been due to histoplasmosis. Although histoplasmosis rarely presents as a monarthritis, unacquaintance with this entity may result in fatal acute dissemination of the histoplasmosis.
