    Notes: One-hundred patients completed a questionnaire designed to assess the disability resulting from acne; patient acne severity was also graded clinically. Ten questions which correlated strongly with clinical acne severity were used to form an Acne Disability Index (ADI). This ADI correlated with the severity of facial acne (r=0·246, P 〈 0·01), chest acne (r= 0·347, P 〈 0·001) and back acne (r= 0·436, P 〈 0·001). Measures were made of the financial value to patients of acne treatment: when hypotheticaly offered either acne or £500, 87% of patients preferred the treatment rather than the money. All 13 patients who stated a preference for the £500 had minimal acne. There is no correlation between the clinical grading of acne and the amount patients would be prepared to pay for a hypothetical cure but there is a correlation between the acne disability score and the amount patients would pay (r= 0·229, P 〈 0·05).

    Notes: The physical integration of out-patient clinics, outpatient treatment and in-patient management areas allows full cohesion and continuity of medical and nursing care of dermatology patients.The conversion of a dermatology ward to an outpatient treatment centre has resulted in major benefits in the daily management of dermatology patients, allowing increased flexibility of care despite a reduction in bed numbers and financial savings. Broad changes in concepts of dermatology management are described and arguments given which may be used in the negotiation for improved facilities.

    Notes: Clinically normal buttock skin of 20 diabetic and eight control subjects, aged 16–30 years, was examined using electron microscopy. The number of layers of cellular material in vessel walls, external to the endothelial cell, was increased in patients with a long duration of diabetes, but was never more than two layers in control subjects. Prominent collagen deposition in the wall of dermal blood vessels was observed only in diabetic skin. No ultrastructural features were significantly related to recent glycemic control, or with the presence of identified microvascular complications of diabetes. The difficulties involved in the interpretation of electron microscopy of diabetic-skin arc discussed. Routine electron microscopical examination of clinically normal diabetic skin provides little information of clinical value on an individual basis.

    Notes: Combined short contact crude coal tar (CCT) and dithranol therapy for chronic plaque psoriasis were compared with dithranol therapy alone in a single-blind paired comparison study. There was no significant difference between the two treatments. The combination of 30% CCT for 1 h and dithranol for 23 h, for treatment of chronic plaque psoriasis, is not advocated.

    Notes: Background Thalidomide is a potentially useful drug for several dermatological disorders. Objectives To assess prescribing and monitoring practices for this drug in Wales. Methods A questionnaire was completed by 17 of 19 consultant dermatologists concerning thalidomide usage in Wales (population 2·93 million). Results Eleven of the 17 respondents had used thalidomide in 40 patients. Only two consultants gave information leaflets and only five obtained written consent. Four obtained baseline nerve conduction studies and nine obtained these during therapy. Of seven women of child-bearing age currently taking thalidomide, none had had baseline pregnancy tests. Conclusions We describe variability in prescribing practices for thalidomide. Published guidelines are reviewed and suggestions made concerning consent forms, pregnancy testing, nerve conduction studies and patient information.

    Notes: One hundred and thirty-seven patients with Darier's disease and 66 patients with Hailey-Hailey disease completed the Dermatology Life Quality Index (DLQ1). The average DLQI score for Darier's disease was 5.89 (19.6% of the maximum score), median = 4, quartiles: 1.9. The average DLQl score for Hailey-Hailey disease was 6.06 (20.2% ofthe maximum score), median = 5, quartiles: 2.9. The DLQf score did not correlate with the physician's assessment of clinical severity in both diseases, thus emphasizing the importance of remembering the potential for handicap even in patients with disease that seems to be mild. The domains of the questionnaire with the highest mean scores in both diseases were for questions relating to symptoms and feelings. Domains relating to personal relationships and work obtained low mean scores even in severe disease. It is encouraging that most patients with Darier's disease and Hailey-Hailey disease maintain good relationships at work and home and seem to be able to adapt to their skin disease.
