    Notes: The most appropriate technique for performing tracheal intubation in patients with cervical spine injury is debatable. Recently, a new device enabling blind oral intubation (Augustine GuideTM) with the patient's head and neck in the neutral position has been introduced. The aim of this study was to compare the extent of upper cervical spine movement during intubation with this device compared to direct laryngoscopy. Twelve patients (Mallampati I and II), without a cervical spine injury, were intubated using the Augustine GuideTM and afterwards by direct laryngoscopy. Both procedures were viewed radiographically. Extension in the upper cervical spine was determined at the point of the maximum excursion. By evaluating the joints occiput-C3 together as a functional unit, blind oral intubation caused 17° (median) less extension compared to direct laryngoscopy (p 〈 0.01). The median differences observed for the individual joints were: 7° in occiput-C1 (p 〈 0.05), 5° in C1-2 (p 〈 0.01) and 6° in C2-3 (p 〈 0.01) respectively. Since we assume that intubation-induced excursions of the injured spine are even higher, blind oral intubation might be a safe alternative for airway management in this special group of trauma victims.

    Notes: In a prospective, randomised trial, 75 patients scheduled for routine surgery were randomly allocated to one of three groups to evaluate trauma and postoperative complications after insertion of the Combitube, tracheal tube or laryngeal mask airway. Insertion of the Combitube was associated with a higher incidence of sore throat (48% vs. 16% vs. 12% [p 〈 0.01]) and dysphagia (68% vs. 12% vs. 8% [p 〈 0.01]) compared with tracheal intubation or insertion of the laryngeal mask airway, respectively. Hoarseness was significantly less common in both the Combitube and the laryngeal mask groups (both 12%) than in the tracheal tube group (44%; p 〈 0.01). Haematoma occurred in 36% of the Combitube group compared with 4% in both the laryngeal mask and the tracheal tube groups (p 〈 0.01). The higher incidence of complications should be considered when using the Combitube.

    Notes: The effects of dietary α-tocopheryl acetate supplementation and different slaughtering methods were investigated on the flesh quality of farmed market-size turbot Scophthalmus maximus (L). Turbot were divided into three groups and fed commercial diets, supplemented with different levels of α-tocopheryl acetate at the following dietary inclusion levels: 72 (100), 547 (500), 969 (1000) (mg of α-tocopheryl acetate kg−1 diet, analytical values with diet codes in brackets). After 5 months, fish (mean weight 1056 ± 19.7 g) from each dietary treatment were sampled, applying three different slaughtering methods: (A) bleeding in ice water; (B) thermal shock, no bleeding; (C) percussion followed by bleeding in ice water. The time course of rigor mortis was evaluated, using pH, rigor index and mechanical compression tests. The results showed that the three parameters corresponded very well. Percussive stunning resulted in higher initial post-mortem pH (P 〈 0.01) and a significantly delayed onset of rigor mortis (P 〈 0.05). Diet significantly affected shelf-life, with fillets from fish fed diets 500 and 1000 having lower TBARS (thiobarbituric acid reactive substances) numbers from day 2 (P 〈 0.001) and less colour deterioration from day 7 of storage on ice onwards (P 〈 0.05). These results suggest that an increase in dietary α-tocopheryl acetate before slaughter as well as careful selection of the slaughtering method may greatly enhance the flesh quality of market-size turbot.

    Notes: Juvenile turbot (45 g, SE = 1.3) were reared under three photoperiods, 08L:16D, 12L:12D and 20L:04D at slightly elevated ambient temperature for Ireland. Over the 297-day experimental period, the overall growth rate of the 12L:12D (0.82% d−1) treatment was higher than for both 08L:16D (0.80% d−1) and 20L:04D (0.77% d−1). Overall relative feed intake (FI = % consumption*day−1) was higher for the 20L:04D (FI = 0.81% d−1, SE = 0.06) treatment than for the 08L:16D (0.63% d−1, 0.04) and 12L:12D (0.64% d−1 0.04) treatments, whereas feed conversion efficiency (FCE = weight gain* consumption−1) was lower in the 20L:04D (FCE = 0.67, SE = 0.08) group when compared with the 08L:16D (0.88, 0.06) and 12L:12D (0.88, 0.06) treatments. Present results show that the long-term extended fixed photoperiod may act as an irritant, inducing stress, suppressing growth and reducing feed utilization. It is hypothesized that the progression of size-dependent hierarchies over time can be divided into two distinct phases herein referred to as ‘hierarchy resolution’ and ‘hierarchy stabilization’ phases (or phases 1 and 2) characterized by increasing and decreasing growth heterogeneity respectively. Growth heterogeneity is measured as coefficient of variation of weight and rank correlation of initial weight of a phase and corresponding growth rate.

    Notes: Eleven brain-dead organ donors were studied during surgery. Plasma levels of adrenaline and noradrenaline were measured before and after skin incision, upon sternotomy and 15, 30 and 45min thereafter. Haemodynamic changes were measured continuously throughout the observation period. Blood pressure and heart rate increased after skin incision, remained high at sternotomy then decreased towards the end of the observation period in six of the 11 patients. Plasma catecholamines increased promptly with the onset of surgical stimuli. We conclude that surgical stress can evoke an excessive rise of plasma adrenaline and noradrenaline and thus could impair allograft function.

    Notes: Haemodynamic and hormonal responses to tracheal intubation can be profound and associated with serious cardiovascular and cerebral side effects. The Augustine Guide, a device enabling blind oral intubation, has been introduced recently. The aim of our study was to compare the haemodynamic and hormonal stress response of this method with direct laryngoscopy. Thirty five patients (ASA 1 and 2) were randomly assigned to undergo either direct laryngoscopy (n = 17), or blind oral intubation (n = 18). Haemodynamic responses and concentrations of adrenaline, noradrenaline and prolactin were determined prior to induction, before intubation and 5 min after intubation. The median duration of intubation was 22 s for direct laryngoscopy vs 46 s for blind oral intubation (p 〈 0.05). Between the groups, no significant differences were observed for heart rate, systolic or mean arterial blood pressure. Serum levels of adrenaline decreased slightly after induction and remained unaltered after intubation in both groups. Noradrenaline (1.01 vs 0.66 nmol.l-1) and prolactin (5.2 vs 2.9 nmol.l-1) levels were significantly higher after direct laryngoscopy compared to blind oral intubation. Although blind oral intubation took significantly longer to perform than direct laryngoscopy, hormonal stress response was less pronounced. Blind oral intubation should therefore not be withheld from patients with impaired cardiovascular reserve.

    Notes: Turbot Scophthalmus maximus is the focus of a rapidly expanding aquaculture industry, while at the same time wild catches appear to be in decline. As a preliminary investigation into the effects of hatchery rearing, genetic variation at three polymorphic microsatellite loci was assessed in two wild populations and two farmed strains of turbot, from Ireland and Norway. Although a considerable loss of rare alleles was observed in the Irish farmed strain, no statistically significant reductions were found in mean heterozygosity or allelic diversity in farmed strains compared to wild populations. Significant genetic heterogeneity was found between wild and farmed samples from each country but not between the two wild populations. Genetic differentiation between the farmed strains was presumed to be caused by drift in the hatcheries. The utility of these particular microsatellite loci in comparing these samples and the importance of molecular genetic testing of farmed strains is stressed.

    Notes: Fish fillet quality has been shown to be influenced by the level of antioxidants in preslaughter diet. Thus, an experiment was conducted to study the effect of different levels of vitamin E and C on the fillet quality of market-size reared turbot (Scophthalmus maximus). Turbot of a mean initial weight of 347 ± 20 g were divided into four groups and fed commercial turbot diets (60% protein, 12% fat), supplemented with α-tocopheryl acetate (mg kg−1) and ascorbyl-2 monophosphate (mg kg−1) at the following dietary levels: 500/100, 1000/100, 100/1000, 100/100 respectively. Over a dietary supplementation period of 15 weeks, fish were fed to satiation and reached a final mean weight of 916 ± 29 g. α-Tocopherol levels increased significantly (P 〈 0.001) in tissue (i.e. muscle, liver, heart and kidney) of fish fed diets containing elevated levels of α-tocopheryl acetate. In ice storage, fillets of these fish exhibited significantly lower (P 〈 0.001) levels of lipid oxidation, and showed significantly less (P 〈 0.001) colour deterioration (higher hue angle and lower chroma). Elevated dietary α-tocopheryl acetate levels had a negative effect (P ≤ 0.001) on the concentration of ascorbic acid in muscle tissue. An increase in dietary vitamin C did not have any detectable effect on fillet quality. Prolonged feeding times had a negative effect on lipid oxidation (P 〈 0.001) and colour deterioration (P 〈 0.01). These results suggest that increased dietary α-tocopheryl acetate could prevent colour deterioration and lipid oxidation of turbot fillets in retail storage on ice.
