    Notes: Immunological and biochemical alterations in atopic dermatitis have been attributed to a deficient conversion of omega-6 fatty acids (i.e. linoleic acid, gamma-linolenic acid, and dihomo-gamma-linolenic acid) to prostaglandin (PG)E1. In patients with atopic dermatitis, however, the formation of PGE1 has not been evaluated so far. We therefore measured plasma concentrations of 15-keto-13.14-dihydro-PGE1 which reflects endogenous PGE1 release, by gas chromatography-mass spectrometry in 31 patients with atopic dermatitis (aged 18-41 years, median 26 years) and in 31 healthy, age and sex-matched control subjects. In order to exclude a metabolic shift from PGE1 to PGE2, we also measured the plasma levels of 15-keto-13, 14-dihydro-PGE2. There was no difference between patients and control subjects with respect to plasma concentrations of 15-keto-13, 14-dihydro.PGE1 (3.9-49.6, median 10.3pg/ml vs. 3.2-80.4, median 8.3pg/ml, P = 0.22), 15-keto-13,14-dihydro-PGE2 (11.6-201.0 median 24.8pg/ml vs. 8.6-201.0.median 19.6 pg/ml, P = 0.10), and the ratio of 15-keto-13,14-dihydro-PGE1 to 15-keto-13.14-dihydro-PGE2 (0.17-1.39, median 0.41 vs.0.2-0.17, median 0.45, P = 0.29). These results indicate that the endogenous formation of both PGE1 and PGE2 is normal in our patients. The results do not confirm the pivotal role that other authors have attributed to a deficient PGE1 formation in the pathogenesis of atopic dermatitis.

    Notes: AbstractA 46-year-old woman presented with multiple seborrheic keratoses disseminated over her trunk and thighs. A screening for internal malignancies revealed an adenocarcinoma of the colon that was successfully treated by surgery. During a follow-up period of 18 months, the seborrheic keratoses remained unchanged. There was no evidence of recurrence of the malignancy. We conclude that this case does not fulfill the criteria of the ‘sign of Leser-Trélat’; according to present knowledge the existence of this cutaneous paraneoplastic syndrome cannot be taken as a certain indication. Patients with multiple seborrheic keratoses do not necessarily present internal malignancies.

    Notes: Summary Background The aetiology of hand dermatoses (HD) is very heterogeneous. Psychological influences on severity and coping in hand eczema have been only rarely investigated. Objectives To investigate whether psychological factors correlate with somatic factors, in order to be able to estimate the possible need for psychosocial treatment of these patients. Methods In a cross-sectional study 101 hand dermatosis patients (49F, 52M) with psoriasis (n = 26), vesicular hand eczema (n = 33) or contact dermatitis (n = 42) were examined with regard to dermatological [diagnosis, severity, Erlanger Atopy Score (EAS)], allergic (patch test) and psychological aspects [Coping with Chronic Skin Diseases questionnaire (CSD), Allover Depression Scale (ADS), Social Readjustment Rating Scale (SRRS), questionnaire for measuring Factors of Aggression (FAF)],and Visual Analogue Scales (VAS) concerning itching, scratching and impairment. Subgroups of high stress responders (high-SR) vs. low stress responders (low-SR) were also examined. Results Of the patients with HD, 47·52% are convinced that ‘stress’ influences the course of their disease. Analysis of variance shows that the subjective reaction to stress (high-SR) correlates with higher severity scores, more itching, higher depression scores and more life events. High-SRs were younger and the onset of the disease was earlier compared with patients without a subjective reaction to stress (low-SR). In the CSD those with especially high-SR but negative patch-test results stated significantly higher values for itching, helplessness and search for information. Conclusions Psychological factors should be taken into consideration in the treatment of patients with HD. High-SR patients with a negative patch-test seem to require more adjuvant psychological care.

    Notes: To assess the relation among eosinophil-related variables in the peripheral blood, bronchial hyperreactivity, and the presence of atopic dermatitis in children aged 5–14 years, we studied 11 patients with atopic dermatitis alone, six with asthma and atopic dermatitis, 12 with asthma alone, and 12 healthy controls. Eosinophil counts, levels of eosinophil cationic protein, and the capacity of eosinophils to generate leukotriene (LT) C4, as well as bronchial hyperreactivity and a severity score for atopic dermatitis, were determined. Eosinophil variables were significantly higher in both patient groups with atopic dermatitis than in normal controls. In particular, ionophore A 23187 LTC4 generation was higher in patients with atopic dermatitis alone (median 82, range 25–273 ng/106 cells) and patients with combined asthma and atopic dermatitis (median 68, range 32–583 ng/106 cells) than in normal controls (median 9, range 1–67 ng/106 cells). However, there was no difference between the group of atopic dermatitis patients with asthma and without asthma. We conclude that eosinophil variables in the peripheral blood are mainly influenced by the presence of atopic dermatitis, and not the presence and the severity of asthma in patients with both asthma and atopic dermatitis.

    Notes: Abstract  The aim was to determine the relationship between mercury content of resting and stimulated saliva, and blood and urine. Eighty subjects participated; 40 of them attributed their self-reported complaints to dental amalgam (patients), the others were matched with respect to age, sex and amalgam restorations (controls). Serum, 24-h urine, resting and chewing stimulated saliva were analyzed for mercury using the ASS-technique. Quality, number, surfaces and total area of amalgam fillings were recorded clinically and using study models. Median (range) mercury levels in serum were 0.67 (0.1–1.52) µg/l for patients and 0.60 (0.1–1.3) for controls. In urine levels were found to be 0.77 (0.11–5.16) and 0.94 (0.17–3.01) µg/g creatinine respectively. No significant differences were found between the groups. Resting saliva contained 2.97 (0.10–45.46) µg/l in patients and 3.69 (0.34–55.41) in controls (not significant). Chewing mobilized an additional amount of 16.78 (–6.97 to 149.78) µg/l in patients and 49.49 (–1.36 to 504.63) in controls (P≤0.01). Only a weak correlation was found between mobilized mercury in saliva and serum (r=0.27; P≤0.05) or urine (r=0.47; P≤0.001). For resting saliva the respective values were r=0.45 (P≤0.001) and r=0.60 (P≤0.001). Saliva testing is not an appropriate measure for estimating the mercury burden derived from dental amalgam.

    Description / Table of Contents: Summary The effectiveness of concurrent psychotherapeutic treatment for various skin diseases can be regarded as sufficiently validated. Contributions on in-patient treatment concepts are rare, and there are up to now no reports on methods integrating psychotherapeutic treatment approaches. Our concept of a 4-week short-term psychotherapy – starting from a conflict hypothesis in the complaint intensifying, acute situation – includes the bimodal work on both understanding and practice oriented treatment focus. By means of 2 case studies with 1-year-follow-up, insight is given into the treatment approach from which effective factors of psychotherapeutic treatment were tentatively in with patients with skin diseases.
    Notes: Zusammenfassung Die Wirksamkeit psychotherapeutischer Mitbehandlung bei zahlreichen Hauterkrankungen kann als ausreichend gesichert gelten. Dabei stellen Beiträge über stationäre Behandlungskonzepte die Ausnahme dar, über methodenintegrierende psychotherapeutische Behandlungsansätze wurde bislang nicht berichtet. Unser Konzept einer 4wöchigen Kurzzeitpsychotherapie beinhaltet zentral – ausgehend von einer Konflikthypothese in der aktuell beschwerdeverstärkenden Situation – die Arbeit an einem bimodalen verstehens- und übungsorientierten Behandlungsfokus. Anhand von 2 Kasuistiken mit 1-Jahres-Katamnese wird ein Einblick in die Behandlungspraxis gegeben, aus der versuchsweise Wirkfaktoren psychotherapeutischer Mitbehandlung bei Patienten mit Hauterkrankungen abgeleitet wurden.
