    Notes: Topical application of ointment bases causes varying degrees of epidermal thickening in guinea-pigs This is reproducible and can be accurately measured. Suppression of these changes was produced by addition of beta-methasone 17-valerate, fluocinolone acetonide, fluocinonide and hydrocortisone acetate. The inhibitory effect of these steroids was related to the type of corticosteroid, its concentration and the ointment base. The effect was still evident at extremely high dilutions of the steroids and could be measured at concentrations as low as 5 × 10-5% of fluocinonide in FAPG.This technique has the merits of being reproducible, sensitive and accurate. It should find a place among existing methods in assaying the efficacy of topical corticosteroids and in aiding the selection of bases most suitable for formulation of these preparations.

    Notes: Psoriasiform eruptions of the skin are recognized as a not uncommon adverse effect of practolol and other β-adrenergic blocking drugs. Topical application of propranolol to guinea-pig skin causes thickening of the skin with histological changes strikingly resembling psoriasis in man. These findings suggest that propranolol has a direct pharmacological effect on the skin.

    Notes: Localised heat urticaria is very rare and it is characterised by the development of whealing limited to the area of heat contact. It is easily differentiated from the more common cholinergic type of heat urticaria which is induced by heat, moderate exertion and emotional stress. This paper describes a case report of localised heat urticaria. To date only two cases have been reported from Great Britain.Our investigations showed that the threshold temperature needed for induction of the heat urticaria varied between 38°C and 40.1°C. The urticaria only appeared on cooling and was accompanied by a rise in plasma histamine. The reaction was suppressed by local but not systemic antihistamines. It was found that tolerance to heat followed both sub-threshold and suprathreshold exposure.The patient was desensitised with minimal production of urticaria. He remains symptom free after 6 months provided that he is exposed to a hot bath every third day. Localised heat urticaria is the rarest of the physical urticarias. So far only nineteen cases have been reported. It is characterized by the development of whealing limited to the area of heat contact. The other, more common form of heat urticaria is the cholinergic variety, in which the urticaria is typically of small pattern and widespread.This report is concerned with a patient with non-cholinergic localised heat urticaria. We present the results of clinical and pharmacological studies and describe his successful desensitisation and the maintenance of tolerance to heat.

    Notes: We have tested eleven non-steroid anti-inflammatory agents and two antioxidants, known to have prostaglandin synthesis inhibitory activity, for their ability to suppress irritant-induced epidermal thickening in the guinea-pig. We have found that salicylates and indomethacin were the most active. From previous experience, this activity is thought to correspond to the behaviour of these drugs in human skin.No anti-inflammatory activity was demonstrated by the antioxidants and therefore the unifying concept of prostaglandin synthesis inhibition as an explanation for suppression of inflammation is questioned.

    Notes: Cell migration and DNA synthesis in organ cultured skin were studied. Cell migration leads to the formation of a new epithelial layer around the free edges of the dermis. No significant DNA synthesis occurs in this newly formed epithelium. In contrast, DNA synthesis in the original epidermis is maintained throughout the period of culture as shown by the uptake of radioactive thymidine.Addition of hydrocortisone to the culture medium increases cell migration, and phytohaemagglutinin, while dramatically stimulating mitotic activity, suppresses cell migration.These results show that an increase in epithelial cell division is accompanied by diminished cell migration whereas an increase in cell migration occurs in the presence of a decrease in epithelial cell division. The behaviour of skin in organ culture during the first 48–72 h can be compared with that seen in wound healing.

    Notes: There is a widely accepted belief that the epidermis in morphoea is either normal or atrophic with loss of rete ridges. However, measurements of epidermal thickness have shown that it is in fact increased. In systemic sclerosis, epidermal thickness was within normal limits and the rete ridges were more prominent than in controls. These quantitative measurements contradict earlier views based on subjective assessments.

    Notes: In this study, the effect of topical applications of coal tar and of fractions derived from it were investigated in guinea-pigs and humans. Coal tar and its fractions produced thickening of guinea-pig epidermis in various degrees which were accurately measured. There was great variability in the epidermal thickening caused by different batches of coal tar. Separation into ether soluble and insoluble fractions yielded products which caused greater epidermal thickening than the parent substance. The acid soluble and alkali soluble extracts caused only minimal skin changes. The acid soluble fraction appeared to have similar activity to that of coal tar in the treatment of psoriasis, whereas the alkali soluble extract was without activity in patients with psoriasis.Coal tar incorporated in emulsifying ointment as well as a mixture of bases known to be present in coal tar and incorporated into emulsifying ointment produced suppression of epidermal thickening in guinea-pigs when compared with appropriate controls. It must be concluded that coal tar contains both irritant (inflammation producing) and anti-inflammatory substsnces. Their precise relationship to therapeutic effectiveness is not clear.
