    Notes: Summary We have studied the incidence of hemoglobinopathies in 1059 individuals in Upper Volta. We have found that this population has a high frequency of HbS and HbC and α-thalassemia. The gene frequency of HbS was high (0.1 for the βS gene) in the arid Sahel portion of Upper Volta accompanied by a lower frequency for HbC (0.05 for the βc gene). The reverse was true in the humid Savanna region of this country (0.03 for the βS gene and 0.14 for the βc gene). There was no age dependency of the HbS gene frequency, but α-thalassemia, detectable in HbS heterozygotes, showed a statistically significant decrease with age. No homozygote for HbS was detected after the age of 1 year, and SC and CC genotypes were found at a lower incidence than expected. The environmental and medical conditions in Upper Volta preclude the survival of SS individuals and decrease the survival of SC and CC genotypes.

    Notes: Summary Twenty right brain-damaged patients and two left brain-damaged patients who showed neglect in the early stage of a stroke were reexamined at a mean time of 10.5 months. In four patients neglect was still as severe as on the initial examination and visual scanning was limited to the right space or even to its rightmost portion. Available CT scan findings suggest that in some patients thalamic lesions may play a critical role in the persistence of neglect. All the other patients improved with passage of time, but minor signs of hemiinattention could still be demonstrated in the great majority of them.

    Notes: Abstract To identify risk factors for A.I.D.S. among homosexual men in France, a highrisk group in a moderate-incidence area, we undertook a case-control study in Paris and its suburbs. Fifty-three cases were compared to 99 controls from a venereal disease clinic and 79 controls from the staff of an amateur gay broadcasting station. In our univariate analysis, cases were more likely than controls to belong to upper socio-economical classes, to have used local corticosteroids, to have regularly inhaled nitrites, to report history of syphilis and of herpes infections, to have higher level of promiscuity with occasional partners (≪ one night stands ≫), and to have had sexual encounters in the U.S.A.. In the multivariate analysis, history of syphilis and promiscuity with occasional partners appeared to be the main risk factors, as well as the use of local corticosteroids, especially during the prodrome period. The correlation between promiscuity and disease risk was weak in our study when we did not discriminate between occasional and regular sex partners, contrary to that found in epidemiological studies of A.I.D.S. and H.,I.V.-seropositivity in high,incidence areas. In association with the history of syphilis, the development of the syndrome in our moderate-incidence country is more correlated to the contact with ≪ one-night-stand ≫ partners rather than to the total number of the sex partners. The public health policies in our area should be adapted to these findings. Otherwise they may be less efficient than hoped.

    Notes: Abstract Twenty-nine patients with the AIDS Related Complex (ARC) were studied retrospectively. Cotton-wool spots (CWSs) were the main ophthalmological finding, present in 9 patients (31%) at this stage of the disease. The CWSs were asymptomatic and were therefore discovered during a routine examination. The proportion of patients with significant weight loss was greater among ARC patients with CWSs than among those with normal fundi. Leukocyte counts were found to be significantly lower (p = 0.03, odds ratio 10.0 [1.24 to 80.4])among ARC patients with CWSs. Lymphocyte counts and the CD4/CD8 ratio were also found to be diminished in these patients. Among the nine patients with CWSs during ARC, seven developped major opportunistic infections or HIV (Human Immunodefiency Virus) related neoplasias during the following months (3.8 ± 3.5 months). These findings are consistent with a poorer prognosis for HIV infected patients who have CWSs.

    Notes: Abstract Lipid accumulation has been described in the duodenal lamina propria of human immunodeficiency virus (HIV)-infected patients with diarrhea and malabsorption. Using light and electron microscopy, we studied duodenal biopsies obtained from 54 consecutive HIV-infected patients by means of upper gastrointestinal endoscopy after an overnight fast. The presence of diarrhea and weight loss were recorded, and all the patients had standard stool study for ova, parasites, and bacteria. Serum levels of albumin, triglycerides, and cholesterol were obtained within one week of the endoscopy. Fecal fat and fecal α1-antitrypsin clearance were measured in 11 patients. Lipid accumulation was observed in nine patients (16.6%). Fat droplets were seen in enterocytes, in their basolateral membrane spaces, and in the lamina propria. The mean serum levels of triglycerides (1.85±0.20 mmol/liter) and cholesterol (2.81±0.30 mmol/liter) were significantly lower in the patients with enteric steatosis than in patients without this anomaly (respectively, 3.38±0.39 and 3.97±0.18 mmol/liter,P〈0.001P〈0.01). The mean amount of fecal fat in the three patients with lipid accumulation (16±1.60 g/24 hr) was significantly larger than in the eight patients without lipid accumulation (4.50±0.62 g/24 hr,P〈0.01). These findings suggest that fat malabsorption in HIV-infected individuals is due to a blockage of transport through the duodenal mucosa. The frequency of diarrhea, weight loss, or identified enteric pathogens did not differ significantly between patients with and without enteric steatosis. Both the etiology and the pathophysiology of these alterations remain to be documented.
