    Notes: Summary Forty-four Parkinson patients (19 patients of the rigid-akinetic type, 13, of the rigid-akinetic-tremor type, and 12, of the tremor type) were included in a study in order to analyse correlations of the expression of the motor symptoms tremor, rigidity, akinesia, with other clinical parameters, computertomographic aspect of brain atrophy and psychometrically assessed cognitive parameters. Rigidity and akinesia are significantly positively correlated with the severity of motor dysability, stage of the disease, and brain atrophy, as is akinesia with a history of pharmacotoxic psychosis. Tremor is significantly negatively correlated with motor dysability, stage of the disease, and history of pharmacotoxic psychosis. Akinesia is correlated with visuomotor dysfunction (tested with Bender Gestalt Test) and rigidity with the depression score (Zung scale). The tremor type is favorable, the rigid-akinetic type unfavorable with respect to motor disability and psychosis.

    Description / Table of Contents: Zusammenfassung Anhand der klinischen Symptome von 12 Patienten wurde das Locked-in-Syndrom (LiS) in das klassische LiS mit vollständiger Bewegungslosigkeit, ausgenommen vertikaler Blickbewegungen und Blinzeln, und in das inkomplette LiS mit weiteren Bewegungsresten unterteilt. Bei absoluter Immobilität mit Einschluß aller Augenbewegungen, jedoch Zeichen für ein erhaltenes Bewußtsein im EEG, wurde ein totales LiS angenommen. Die anatomische Grundlage für diese Form sind bilaterale Läsionen in den Pedunculi cerebri mit einer Unterbrechung der Pyramidenbahn, der corticobulbären Bahnen, der zentralen Fasern für horizontale Blickbewegungen und der postnukleären Oculomotorius-Fasern. Als Verlaufsvarianten wurden das chronifizierte und das transiente LiS unterschieden.
    Notes: Summary The locked-in syndrome (LiS) was broken down on the basis of neurological symptoms in 12 patients. The criteria of classical LiS are total immobility except for vertical eye movements and blinking. If any other movements are present one should consider the condition as incomplete LiS. Total immobility, including all eye movements, combined with signs of undisturbed cortical function in the EEG led to the concept of total LiS. The anatomical basis for this condition consists of lesions in both cerebral peduncles which interrupt the pyramidal and corticobulbar tracts, the supranuclear fibers for horizontal gaze and the postnuclear oculomotor fibers. As to the course, chronic and transient LiS have been described.

    Notes: Summary A patient with serologically confirmed infection byBorrelia burgdorferi presenting with painful paresis and atrophy of the proximal muscles of both upper extremities and bilateral facial paresis is described. Electromyography showed a neurogenic and myopathic pattern, and creatine kinase was raised. Muscle biopsy revealed the typical signs of focal myositis. Treatment with i.v. penicillin led to dramatic clinical and serological improvement. Muscle biopsy was repeated 2 months later; neurogenic changes were still present, but no inflammatory signs could be seen anymore. Thus, the presented case may be the first reported of meningopolyneuritis accompanied by focal nodular myositis, in the second stage of Lyme borreliosis.

    Notes: Summary A previously healthy seventeen-year-old boy developed loss of weight, poor appetite, and aversion to food. Physical examination being normal, anorexia nervosa was suspected. Thirteen months later a CT scan revealed a mass in the third ventricle histologically proven to be a malignant teratoma. To our knowledge anorexia nervosa is only extremely rarely the presenting feature of intracranial germ cell tumors.

    Description / Table of Contents: Zusammenfassung Fallbericht über einen 45jährigen Mann, bei dem es im Rahmen einer histologischen verifizierten Mycosis fungoides offensichtlich im Verlauf eines leukämischen Schubes zur Hirnbeteiligung mit zwei Herden in der linken Parietal- bzw. Parieto-Temporalregion kam. Bioptisch fanden sich lymphomartige Gefäß- und Gewebsinfiltrate mit einzelnen Riesenzellen (Lutzner-Zellen) ohne sichere Meningealaussaat. Die Liquorcytologie ergab präund postoperativ vereinzelt atypische Lymphocyten und Mononucleäre, darunter eine ultrastrukturell charakteristische Sézary-Zelle. Im peripheren Blut waren nach postoperativem Rückgang der hohen Leukocytenzahl die in den Hautinfiltraten vorhandenen atypischen Lymphocyten (Sézary-Zellen) morphologisch und an Hand cytochemischer (β-Glucuronidase) und immunologischer (Rosetten mit Schafserythrocyten) Marker nachweisbar. Das Krankheitsbild wird als cerebrale Manifestation in der leukämischen Phase einer Mycosis fungoides im Sinne eines Sézary-Syndroms angesehen.
    Notes: Summary Two foci of histologically verified mycosis fungoides were found in the left parietal region of a 45-year-old man with involvement of the brain during an exacerbation of leukemia. There was perivascular and diffuse infiltration with lymphoma-like cells and occasional giant (Lutzner) cells but no involvement of the meninges. Both before and after operation the CSF contained a few atypical lymphocytes and mononuclear cells, one of which showed ultrastructurally the deeply clefted nucleus of Sézary cells. Atypical cells were confirmed morphologically in the peripheral blood, as well as by cytochemical (β-glucuronidase) and immunological (rosettes with SRBC) markers, after the postoperative leucocytosis had subsided. It is suggested that this CNS involvement during a leukemic conversion phase of mycosis fungoides is consistent with the Sézary syndrome.

    Description / Table of Contents: Résumé A cause de plusieurs cas on rapporte d'une complication se présentante souvent au syndrome traumatique apallique qui se montre par une ossification des parties près des articulations et qui doit être nommée, apres notre opinion, une ossification périarticulaire. De tous les 84 cas d'un syndrome traumatique apallique l'ossification périarticulaire s'est montrée en 16 cas et dont 12 cas à plusieurs articulations. On traite la clinique, les modifications morphologiques et la pathogenèse et aussi les mesures prophylactiques et thérapeutiques sont discutées. Par expérience d'un grand nombre de malades à un syndrome traumatique apallique, il faut indiquer l'importance de prévenir des complications quant aux ossifications périarticulaires.
    Abstract: Zusammenfassung An Hand von mehreren Fällen wird über eine beim traumatischen apallischen Syndrom häufig auftretende Komplikation berichtet, die sich in einer Verknöcherung von gelenksnahen Abschnitten zeigt und nach unserer Meinung als periartikuläre Ossifikation zu bezeichnen ist. Von insgesamt 84 Patienten mit einem traumatischen apallischen Syndrom war die periartikuläre Ossifikation 16mal aufgetreten, davon in 12 Fällen an mehreren Gelenken. Es wird auf Klinik, die morphologischen Veränderungen und die Pathogenese der Ossifikationen eingegangen und die prophylaktischen sowie die therapeutischen Maßnahmen besprochen. Aus der Erfahrung an einer größeren Anzahl von Patienten mit einem traumatischen apallischen Syndrom muß auf die Wichtigkeit der Verhinderung von Komplikationen im Sinne von periartikulären Ossifikationen hingewiesen werden.
    Notes: Summary By several cases it is reported upon a complication of frequent occurrence to the traumatic apallic syndrome, which is appearing as an ossification of close-articular parts and which in our opinion is to call a periarticular ossification. From altogether 84 patients with a traumatic apallic syndrome we found 16 cases of periarticular ossification, by 12 cases of them at several joints. Clinic, morphological change and pathogenesis are reviewed and prophylactic as well as therapeutic measures are discussed. From experience of a greater number of patients with a traumatic apallic syndrome it is necessary to refer to the importance of preventing complications in relation to periarticular ossifications.

    Notes: Summary The most frequent sequelae after severe brain injury include changes in personality traits, disturbances of emotional behaviour and impairment of cognitive functions. In particular, emotional changes and/or verbal and non verbal dysfunctions were found in patients with bilateral or unilateral temporal lobe lesions. The aim of our study is to correlate the localization of the brain damage after severe brain injury, in particular of the temporal lobe, with the cognitive impairment and the emotional and behavioural changes resulting from these lesions. The patients with right temporal lobe lesions showed significantly better scores in verbal intelligence and verbal memory in comparison with patients with left temporal lobe lesions and those with other focal brain lesions or diffuse brain damage. In contradistinction, study of the personality and the emotional changes (MMPI and FAF) failed to demonstrate pathological scores in the 3 groups with different CT lesions, without any significant difference being found between the groups with temporal lesions and those with other focal brain lesions or diffuse brain damage. The severity of the brain injury and the prolongation of the disturbance of consciousness could, in our patients, account for prevalence of congnitive impairment on personality and emotional changes.

    Notes: Summary The histories of two patients with spinal epidural abscess are reported. Attention is drawn to the route of infection by means of direct inoculation of pathogens in the wake of paravertebral infiltration of local anaesthetics for relieving lower backpain, furthermore we emphasize the rare occurrence of a relapsing acute spinal epidural abscess. Additionally the diagnostic superiority of nuclear magnetic resonance tomography is described in a patient known to be allergic to commonly used contrast material.

The pattern of C.S.F. amino-acids in Parkinson's disease differs from the normal in that the amount of glutamic acid is strongly reduced, whilst the concentration of glycocoll, serine, threonine, cysteine and methionine is increased. The total amount of amino acids is, however, the same in normal persons as in patients suffering from Parkinson's disease. 2. I.v. administration ofl-Dopa in normal and pathological cases results after transient decrease in an increase of amino acid concentration in C.S.F. and serum. This change is mainly due to shifting of the concentration of glutamic acid, arginine and serine. 3. In patients suffering from Parkinson's disease afterl-Dopa administration the concentration of tyrosine in C.S.F. and serum shows a transient increase.
    Notes: Zusammenfassung 1. Die Aminosäurezusammensetzung des Liquor cerebrospinalis Parkinson-Kranker unterscheidet sich von der Gesunder dadurch, daß der Glutaminsäuregehalt stark erniedrigt, dagegen die Konzentration am Glykokoll, Serin, Threonin, Cystein und Methionin erhöht ist. Die Gesamtmenge an Aminosäuren ist jedoch bei Gesunden und Parkinson-Kranken dieselbe. 2. Intravenöse Verabreichung vonl-Dopa führt in normalen und pathologischen Fällen nach einem vorübergehenden Absinken der Aminosäurekonzentration im Liquor und Serum zur Erhöhung der Konzentration dieser Stoffe; an der Konzentrationsbewegung sind vor allem Glutamin, Arginin und Serin beteiligt. 3. Der Tyrosinspiegel im Liquor und Serum steigt bei Parkinson-Kranken beil-Dopa-Belastung vorübergehend an.

    Description / Table of Contents: Summary The observation of a congenital arteriovenous angioma in the territory of the vertebral artery, which produced clinical symptoms of lesions of the anterior and posterior thalamus, of the internal capsule, and of the Corpus Luysii (Hemiballismus), provided an opportunity for a short discussion of the vascular malformations and of the pathophysiology of ballistic hyperkinesia. The case reported is of interest because the ballismus, after lasting for 20 years, terminated in association with the clinical signs of a cerebral haemorrhage. In regard to the whole course and to the findings in brain sections, it is considered that for spontaneous remission to occur there must be an at least partial destruction of the oral-ventral nucleus of the thalamus. This observation seems to confirm the previous experiences of stereotactic operations in ballismus.
    Abstract: Résumé L'observation d'un angiome artérioveineux congénital, dans la région de l'artère vertébrale, qui donna des symptÔmes cliniques de lésions du thalamus antérieur et postérieur, de la capsule interne et du corps du Luys (hémiballisme) fournit l'occasion d'une courte discussion sur les malformations vasculaires et la pathophysiologie de l'hyperkinésie ballistique. Le cas rapporté ici est d'un certain intérÊt parce que le ballisme, après une durée de 20 ans, se termina avec les signes cliniques d'une hémorragie cérébrale. En considérant le cours entier de la maladie et les découvertes faites dans des sections du cerveau, l'on considère que, pour qu'une rémission spontanée survienne, il doit y avoir une destruction au moins partielle du noyau ventral antérieur du thalamus. Cette observation semble confirmer les expériences précédentes d'opérations stéréotaxiques dans le ballisme.
    Notes: Zusammenfassung Die Beobachtung eines kongenitalen arteriovenösen Angioms („Aneurysmas“) im Stromgebiet der Arteria vertebralis, die klinische Symptome von LÄsionen des vorderen und hinteren Thalamus, der inneren Kapsel und des Corpus subthalamicum Luysii (Hemiballismus) bot, gibt Anla\ zur kurzen Erörterung dieser GefÄ\mi\bildung sowie der Pathophysiologie der ballistischen Hyperkinese. Der mitgeteilte Fall ist deshalb von Interesse, weil der Ballismus nach über 20jÄhriger Dauer unter den klinischen Zeichen einer Cerebralblutung weitgehend zum Stillstand kam. Unter Berücksichtigung des Gesamtverlaufes und des Hirnsektionsbefundes wird für diese Spontanremission die zumindest teilweise Zerstörung der oralen Ventralkerne des Thalamus angesehen. Diese Beobachtung erscheint zur BestÄtigung der bisherigen Erfahrungen stereotaktischer Eingriffe beim Ballismus geeignet.
