Dans 8% des cas, les opérations endoscopiques n'ont pas réussi et ont entraîné une arthrotomie. Durant la dernière partie de cette analyse ce chiffre est tombé à 4% grâce à l'amélioration de la technique opératoire et de la forme des instruments. 92% des patients ont été cotés à l'aide d'une échelle d'évaluation de 2 à 51 mois après le traitement, soit en moyenne 13,8 mois (±7,7). L'opération des ruptures traumatiques a donné de meilleurs résultats que le traitement des ruptures dégénératives. Les résultats ont été meilleurs dans les méniscectomies internes que dans les ménisectomies externes. D'autres lésions du genou, telles qu'une altération du cartilage ou une instabilité antérolatérale, ont eu une influence négative sur le résultat de la ménisectomie. La période post-opératoire de réhabilitation a été courte. La plupart des patients ont pu reprendre leur travail deux semaines après l'opération et le sport quatre semaines plus tard. L'opération peut être effectuée sans hospitalisation.
    Notes: Summary A series of 264 endoscopic meniscectomies has been analysed. In 8% the endoscopic operation failed, and arthrotomy was performed at the same session. During the latter part of the study, this figure decreased to 4% owing to improved skill and instrument design. 92% of the patients were followed up by scoring-scale evaluation 13.8±7.7 (2–51) months after treatment. Treatment of bucket handle tears gave better results than treatment of degenerative tears. The results were less successful after lateral than after medial meniscectomy. Other pathological findings in the joint including cartilage degeneration or an anterior cruciate tear with anterolateral instability unfavourably affected the result of meniscectomy. The post operative rehabilitation period was short and most patients returned to work within two weeks and to athletic activities within four weeks. The operation can be performed as an out-patient procedure.

    Description / Table of Contents: Résumé L'expérience de plus de 1000 arthroscopies pratiquées en six ans a permis de sélectionner un jeu d'instruments de base pour réaliser la technique décrite dans cet article. L'équipement comporte deux optiques avec un angle de vision de 30° et de 70°. La gaine extérieure a un diamètre de 5 mm. L'irrigation articulaire est effectuée à l'aide d'une pompe à perfusion et d'un trocart de sortie spécialement destiné à cet usage. On utilise de petits crochets pour mobiliser les structures intraarticulaires. On peut également pratiquer l'ablation de corps étrangers sous contrôle de la vue grâce à l'arthroscope.
    Notes: Summary Experience with more than 1000 arthroscopies during six years has determined the selection of a basic set of instruments for the procedure which is described in this paper. The equipment comprises two telescopes, with 30° and 70°·angles of vision. The outer sheaths have a diameter of 5 mm. Joint irrigation is performed using an infusion pump and a specially made outflow cannula. Small hooks are used to manipulate the intraarticular structures. Removal of loose bodies has also been performed under direct vision using the arthroscope.

    Notes: Repair of full-thickness cartilage defects of the rabbit medial femoral condyle with artificial plugs and periosteal grafts was followed for 1 year. The morphological characteristics of the neocartilage after repair and the concentration of proteoglycan fragments in the joint fluid (CPFF) were evaluated. The CPFF showed only a weak correlation to the histological score of the neocartilage. Normal cartilage and subchondral bone did not develop with any of the repair alternatives. With a Dacron plug, neocartilage formation appeared earlier than with natural repair and was better than with periosteal grafting. Increased CPFFs were found in all specimens with repair at 3 months, probably reflecting the remaining effect of surgery. At 6 months, the CPFFs were again similar to those at the 3-month evaluation. This was paralleled by a degeneration of the neocartilage. We conclude that Dacron plugs could serve as a scaffold for neocartilage formation. The CPFF reflected surgical trauma and late degeneration of neocartilage but did not indicate minor differences in quality between cartilage repair techniques, probably because of the limited size of the grafted defect. © 1993 John Wiley & Sons, Inc.

    Notes: Implants of polyurethane-coated polyester (Dacron), polyurethane-coated PTFE (Teflon), or uncoated PTFE were used to substitute the medial rabbit meniscus, and were observed over a 3-month period. Joint mechanics were similar for sham-operation, meniscectomy, or substitution with a coated implant. Joints with an uncoated PTFE prosthesis tended to be less compliant and to show greater load dissipation during relaxation. Cartilage indentation showed softening with all alternatives compared to sham-operation. Ingrowth into the prosthesis periphery was best with the PTFE implants. Both types of coated prosthesis retained their original shape, in contrast to the uncoated prostheses. To the naked eye the cartilage appeared unaffected only after sham-operation or implantation of a coated PTFE prosthesis. Osteophyte formation was common with all alternatives except sham-operation. We conclude that replacement of the medial meniscus with coated prostheses, especially the PTFE, was better than meniscectomy or using the uncoated prosthesis. However, cartilage softening and osteophyte formation indicate that joint mechanics were not restored to normal. © 1993 John Wiley & Sons, Inc.
