    Notes: Summary The pharmacokinetics of caffeine was examined in 13 premature infants (gestational age 25–34 weeks, birth weight 920–2060 g, postnatal age 1–42 days) who received the drug for treatment of opnoea. Caffeine (1% aqueous solution) was given i.v. in single doses; guided by the clinical response infants received between one and seven (mean 2.6) doses of 15 mg/kg. Mean (± SE; range) Clb was extremely slow − 8.5 ml/kg/h (±0.4; 5.8–12.2), t1/2 was prolonged − 65.0 h (±3.7; 48.2–87.5 h) and Vd was 0.781/kg (±0.04; 0.47–1.01). No significant correlation was found between Clb, t1/2 and postnatal age in the whole group or in individual infants. Effective plasma concentrations varied over a wide range (12–36 µg/ml) and overlapped with subtherapeutic concentrations (⩽24 µg/ml). Single doses of 15 mg/kg i.v. or p.o. prevented apnoea in most cases, if necessary followed by additional doses. Monitoring the blood level of caffeine in infants receiving frequent repeated doses is necessary to prevent toxicity.

    Notes: Abstract Discitis is an inflammatory process of the intervertebral disc space occurring in the pediatric age range. Its etiology is usually unknown; staphylococcus aureus is the most common microorganism present in the few cases with positive bacteriological cultures. The clinical presentation can mimic medical or surgical emergencies and thus unnecessary investigations or surgical procedures are frequently performed. Irritability, refusal to sit or walk, low grade fever, high erythrocyte sedimentation rate and normal leukocyte count are the most common clinical findings. The disease is usually self-limited and has a benign course. Awareness of the physician is the key for the diagnosis which can be established by nuclear imaging procedures and radiological studies, yet, this disease is not described in standard pediatric textbooks. Eight cases of discitis in children affecting the dorsal, lumbar and cervical spine were diagnosed and treated in two medical centers in Israel. The purpose of this report is to focus the attention of physicians caring for children on this entity.

    Notes: Abstract Seventeen infants under 2 months of age are described who presented with methaemoglobinaemia and acute diarrhoea during a period of 2 years. No infants beyond this age presented with such characteristics. In none of them was one of the known mechanisms of methaemoglobin formation found. All infants recovered with conventional therapy. Methacmoglobin associated with diarrhoea relapsed in three infants before they were 2 months old. Twenty-six similar cases have been reported in the literature. The mechanism of methaemoglobinaemia in these infants is unclear.

    Notes: Abstract The case of a six-year-old boy who developed meningitis three months after a skull fracture is reported. Blood and cerebrospinal fluid yieldedPasteurella ureae. A two-week intravenous course of ampicillin and chloramphenicol resulted in full recovery. Two of the other four cases ofPasteurella ureae meningitis reported in the literature occurred after skull fractures, and one after intracranial surgery. The child described here is the youngest reported patient with meningitis due to this rare pathogen.
