Zusammenfassung Die fetale Hypotrophie als Ausdruck eines intrauterinen Schadens scheint mit einer gewissen Verlangsamung der Gewichts-, Längen- oder psychomotorischen Entwicklung des Individuums verbunden zu sein und vereinigt daher Kinder mit erhöhtem Risiko. Bei 5556 Frauen aus der Grossstadt, die in dreizehn Frauenspitälern von Paris im ersten Schwangerschaftstrimester zur Untersuchung kamen, wurde eine prospektive Untersuchung durchgeführt. Sie erlaubte, die Beziehung der fetalen Hypotrophie in Abhängigkeit mütterlicher Merkmale festzustellen, die schon zu Beginn der Schwangerschaft bekannt waren. Unter den wichtigen mütterlichen Faktoren sind zu erwähnen: konstitutionelle Faktoren (geringes mütterliches Gewicht, kleine Statur); Umweltfaktoren (ledige Mütter, Tabakkonsum, Alkoholkonsum); geburtshilfliche Faktoren (Primiparal, zahlreiche vorangegangene Schwangerschaften, frühere Totgeburten oder Geburten mit geringem Geburtsgewicht, frühere Aborte, Geburtsgewicht unter 2500 g bei der letzten Schwangerschaft). Die Hypotrophie wird definiert als ≤10. Percentile der «Intrauterinen Wachstumskurve» dieser Bevölkerung. Die Kurve wird in dieser Arbeit graphisch dargestellt, und die Frage der Definition der Hypotrophie wird erörtert.

Summary Foetal hypotrophy as an expression of intrauterine suffering seems to be related to a certain retardation of ponderal, statural or psychomotor development of the individual, and identifies children with increased risk. A prospective study of 5556 metropolitan women was conducted, who came for consultation in thirteen Paris maternity hospitals during the first trimester of their pregnancy. It allowed to investigate the relationship of foetal hypotrophy to certain maternal characteristics, known at the onset of pregnancy. To be mentioned among the important maternal factors are: constitutional factors (low maternal weight and height); environmental factors (unwed mothers, tobacco, alcohol); obstetrical factors (primiparous and highly multiparous women, history of stillbirths or deliveries of low birth weight children, miscarriages, birthweight below 2500 g at the preceding delivery). Hypotrophy is defined as equal or less than the tenth percentile of an «intra-uterine growth curve» established within the study population. A graphic representation of this curve is presented, and the question of the definition of hypotrophy is discussed.

Résumé L'hypotrophie fœtale, témoin d'une souffrance in utero, semble liée à certains retards du développement pondéral, statural ou psychomoteur de l'individu, et regroupe, à ce titre, des enfants à risque élevé. Une enquête prospective sur 5556 femmes métropolitaines venues consulter au cours du premier trimestre de la grossesse dans treize maternités hospitalières de Paris a permis d'étudier comment se répartissait l'hypotrophie fœtale en fonction de certains caractères maternels connus au début de la grossesse (tel l'âge, le poids, la consommation de tabac...). L'hypotrophie a été définie à partir du 10e percentile de la courbe de «croissance intra-utérine» de cette population dont nous donnons ici la représentation graphique. Une discussion sur cette limite de l'hypotrophie est abordée.
