    Notes: Abstract Purpose: This study was undertaken to elucidate characteristic findings of tuberculous (TB) colitis at double-contrast barium enema examination (DCBE). MATERIALS AND METHODS: Twenty-five patients with TB colitis diagnosed by biopsy (n=13) or therapeutic trial (n=12) were included. DCBE findings were retrospectively analyzed, with special emphasis on distribution of lesions and mucosal changes. RESULTS: Involvement was asymmetric in 12. The lesion was noted more commonly in the ascending colon (n=23), cecum (n=21), and terminal ileum (n=19) compared with the transverse colon (n=15) and descending and sigmoid colon (n=9). Skipped lesions were seen in 13 patients. Sixteen patients had multiple ulcers that were mostly transverse or circumferential in alignment (n=11). Depth of ulcers was superficial (〈2 mm) in 9 patients and deep in 13. Two patients had fistulas. Twelve patients had inflammatory polyposis. Three patients presented with mass, and misdiagnosis of malignant tumor was made in two. Thickening, deformity, and incompetence of the ileocecal valve were frequently noted (5, 6, and 8, respectively). CONCLUSION: DCBE revealed detailed mucosal changes of TB colitis. Ulcers aligned in transverse or circumferential pattern, involvement of the right side colon, and deformity of the ileocecal valve suggest diagnosis of TB colitis.

    Notes: Abstract In eight patients with tuberculous strictures of the urinary tract, antegrade balloon dilation and ureteral stenting was attempted. In six patients, antegrade balloon dilation was successfully performed; however, the procedure was aborted in the remaining two patients due to the failure in passing a guidewire through the stenotic lesions. In six patients in whom balloon dilation and ureteral stenting were performed, a total of nine stenotic lesions were dilated. Those were four ureteric lesions, two lesions of ureteropelvic junctions, two lesions of ureterovesical junctions, and a lesion of calyceal infundibulum. In all six patients in whom balloon dilation and ureteral stenting was successfully accomplished, intravenous urograms obtained 9–31 months after the procedure showed improvements both in contrast media excretion and in prestenotic dilatation. Antegrade balloon dilation of the urinary tract combined with ureteral stenting was an effective technique for the management of stenoses secondary to tuberculosis of the urinary tract.

    Notes: Abstract Fasciola hepatica is a trematode of herbivorous mammals. Humans are accidentally infected by the ingestion of water or raw aquatic vegetables contaminated with the metacercaria. Radiological findings of six patients with fascioliasis (five hepatic fascioliasis, one biliary fascioliasis) were analyzed. The diagnosis was based on serologic testing and/or histopathologic findings of eosinophilic abscess in five patients and identification of the adult worm in one patient. The characteristic radiological features of hepatic fascioliasis were (1) cluster of microabscesses arranged in tract-like fashion (burrow tract), (2) subcapsular location of the hepatic lesions, and (3) very slow evolution of the lesion on follow-up examinations. In biliary fascioliasis, there were multiple conglomerated filling defects in the common bile duct. The authors believe that the demonstration of these features, together with peripheral eosinophilia or eosinophilic aspirate from the hepatic lesion, is very helpful in making the correct diagnosis.

    Notes: Abstract The computed tomographic (CT) findings of 13 cases of calcified gastric carcinoma were analyzed retrospectively. Eleven cases were confirmed as a mucinous adenocarcinoma by surgery (three cases), or endoscopic biopsy (eight cases). Two cases were diagnosed as adenocarcinoma by endoscopic biopsy. In all cases the calcifications were of the punctate or miliary shape and the size varied from 1–3 mm in diameter. The calcifications were located in the thickened gastric wall in all cases, and were seen in metastatic lesions such as lymph nodes and the liver in two cases. In 10 cases, some tumor portions showed lower attenuation number than that of the muscle on CT scans, and corresponded to mucin pool in tumor portions histologically. Twelve cases were in inoperable advanced stage.

    Notes: Abstract Eighty-six hepatocellular carcinomas (HCCs) in 67 patients were examined by intraoperative sonography. Sensitivity for detecting tumors with intraoperative sonography was compared with sonography, computed tomography (CT), hepatic angiography, and CT after intraarterial injection of iodized poppy-seed oil (Lipiodol-CT). The overall sensitivities were 76% with sonography, 86% with CT, 89% with angiography, 96% with Lipiodol-CT, and 98% with intraoperative sonography. The differences in sensitivity between intraoperative sonography and sonography (p 〈 0.01), CT (p 〈 0.01), and angiography (p 〈 0.05) were significant. In 35 lesions smaller than 2 cm, the sensitivities of Lipiodol-CT and intraoperative sonography were high (91 and 94%, respectively). In operating field, tumors were invisible in 36 (42%) and nonpalpable in 31 of 86 cases (36%). In 35 tumors smaller than 2 cm, invisible tumors were 66% and nonpalpable tumors were 63%. However, 84 of 86 cases (98%) could be localized with intraoperative sonography. These results suggest that intraoperative sonography is the final diagnostic imaging procedure before surgical resection of tumors and in cases of invisible and nonpalpable tumors in the operating field, this procedure is mandatory to improve surgical results.

    Notes: Abstract We encountered 2 cases of guidewire-induced perforation of the gastroduodenal artery after using a hydrophil-coated guidewire. The guidewire penetrated a small side branch of the normal gastroduodenal artery in 1 patient and the weakened gastroduodenal artery due to pancreatic pseudocyst in the other patient. Both complications resulted in serious hemorrhage and were successfully managed by transcatheter embolization using steel coils. In an era of widespread use of hydrophil-coated guidewires for superselective catheterization, it is important to appreciate the possibility of this potentially serious complication.

    Notes: Abstract Purpose The effectiveness of renal transcatheter arterial embolization (TAE) with a mixture of ethanol and iodized oil (Lipiodol) was analyzed retrospectively in 27 patients with unresectable renal cell carcinoma. Methods Symptomatic or permanent palliation by TAE was performed with the intent to achieve total or partial ablation of the kidney with 10–28 ml of embolic material in a 3 ∶ 1 mixture of ethanol and iodized oil. Clinical, angiographic, and computed tomographic (CT) follow-up were analyzed. Cumulative survival was estimated with the Kaplan-Meier method. Results Partial (5) or complete (22) ablation of the embolized kidneys was achieved in all cases. Follow-up CT after 1 month revealed Lipiodol uptake in the tumor area (14/14), decreased size of the tumor (6/14), and gas formation (7/14). Pulmonary metastasis improved significantly in one patient after transcatheter arterial embolization and adjunctive chemotherapy. Overall median survival of the 27 patients was 8.5 months. The median survival was 23 months in 10 patients with stage III, three of whom are still alive, and was 7 months in 15 patients with stage IV tumors. Conclusion The mixture of ethanol and Lipiodol is not only a convenient embolic material to use, but is also effective for selective or nonselective devascularization for unresectable renal cell carcinoma.

    Notes: Abstract Intrahepatic stones are frequently associated with recurrent cholangiohepatitis and complete surgical removal of the stones is almost always difficult because of the large number of stones and the associated bile duct strictures. One hundred and seventy patients with retained intrahepatic stones underwent percutaneous stone removal utilizing pre-shaped angulated catheters, 9-F polyethylene tubes with different angles of distal tip, and Dormia baskets under fluoroscopy with a combination of techniques including irrigation-suction, crushing of large stones, balloon dilatation of strictures and extracorporenal shock wave lithotripsy. The procedure was performed through a mature T-tube tract (168 patients) and a mature transhepatic tube tract (two patients). Stones were exclusively intrahepatic in 125 patients, whereas 45 patients also had stones in the common bile duct. In 84 (49.4%) of 170 patients, the stones were completely removed and in 36 (21.2%) patients, most of the stones were removed. Overall success rate was 70.6%. Angulation deformity, stricture of bile ducts and impacted stones were the factor mosto ften responsible for failure. No significant complications were observed. Fluoroscopically-guided percutaneous interventional procedures with a pre-shaped angulated catheter is a useful complementary procedure to surgery for patients with intrahepatic stones. The major benefits of an individually fitted angulated catheter are its safety and easy access to small peripheral bile ducts.
