    Notes: Aim: To examine the efficacy and safety of human embryonic stem (ES) cell-based therapies, allogeneic transplantation of monkey ES cells would be useful. We transplanted genetically marked monkey ES cells into the allogeneic fetus. Methods and Results: Cynomolgus ES cells were transduced once using a simian immunodeficiency virus-based lentivirus vector encoding the GFP gene driven by the CMV promoter at 1, 10 and 100 transducing units per cell. Five days posttransduction, 60, 80 and 90% of the cells expressed GFP, respectively, and the expression levels were stable for 5 months. GFP expression was still observed after embryoid-body formation. The gene-marked ES cells were transplanted into the cynomolgus fetus in the abdominal cavity (n = 2) or liver (n = 1) after the first trimester. The fetuses were delivered 1 month posttransplantation. Transplanted cell progeny were detected (∼1%) in multiple tissues by quantitative PCR and in situ PCR of the GFP sequence. No teratoma was found in the tissues. Conclusions: Cynomolgus ES cells can be engrafted in the allogeneic fetus. We are now trying to transplant cynomolgus ES cells differentiated to neural or hematopoietic lineage.

    Notes: Background  The majority of skin changes associated with ageing are caused by photoageing and reflect cumulative sun exposure. Although the actinic damage plays a major role in skin pigmentation, it is also important to examine the effects of chronological cellular ageing on the pigmentation. The chief cellular components of the skin other than melanocytes are keratinocytes and fibroblasts, and the influences of age-related changes in those cells on skin pigmentation have not been elucidated.Objective  To clarify the effects of cellular ageing of keratinocytes and fibroblasts on age-related skin pigmentation.Methods  Using ELISA analysis, we measured the level of melanogenic cytokines secreted by cultured keratinocytes and fibroblasts derived from skin of various chronological ages. We also compared the cytokine secretion by cultured keratinocytes between the second and fifth cultures.Results  There was no correlation between age and hepatocyte growth factor (HGF), stem cell factor (SCF), and basic fibroblast growth factor (bFGF) secretion by fibroblasts. On the other hand, a significant positive correlation existed between age and interleukin ((IL)-1α secretion (R2 = 0·50, P = 0·002), and a relatively weak correlation existed between age and endothelin-1 (ET-1) secretion (R2 = 0·17, P = 0·051, not significant). The IL-1α secretion by keratinocytes was significantly increased in the fifth cultures compared with the second cultures (P 〈 0·005).Conclusions  These findings suggest that IL-1α secretion increases as cells grow older, and the increased secretion of IL-1α by aged keratinocytes may stimulate HGF production in dermal fibroblasts paracrinely and ET-1 production in keratinocytes autocrinely, which stimulates melanocyte proliferation and induces an increase of tyrosinase activity in melanocytes. Because IL-1α is a primary mediator that responds to inflammation and injury, the transcription of genes involved in skin inflammation may be persistently induced in the aged skin. Thus the increased secretion of IL-1α by aged keratinocytes in the aged skin may play a role in the accentuated cutaneous pigmentation and other skin ageing.

    Notes: Summary Background The mechanism of the accentuated melanization in café-au-lait macules (CALMs) in patients with neurofibromatosis type 1 (NF1; von Recklinghausen's disease) has not been elucidated. Objectives To clarify the mechanism involved in the hyperpigmentation of CALMs in NF1. Methods Using enzyme-linked immunosorbent assay (ELISA) and reverse transcriptase-polymerase chain reaction (RT-PCR) analysis of cultured cells, we measured the levels of cytokines produced and secreted by keratinocytes and fibroblasts derived from CALMs (group RC: Recklinghausen CALM) skin, compared with cells derived from the skin of normal individuals (group NN: Normal skin of Normal individuals) and cells derived from non-CALM skin of NF1 patients (group RN: Recklinghausen Non-CALM). Results ELISA revealed that the secretion of hepatocyte growth factor (HGF) and stem cell factor (SCF) by cultured fibroblasts was significantly elevated in group RC compared with groups RN and NN. In parallel, semiquantitative real-time RT-PCR of HGF and SCF mRNAs demonstrated increased expression of both types of transcripts by cultured fibroblasts in group RC compared with group NN. In contrast, the secretion of endothelin-1 and granulocyte/macrophage colony-stimulating factor by cultured keratinocytes occurred at a similar level among all three groups, RC, RN and NN. Conclusions These findings suggest that increased secretion of HGF and SCF by dermal fibroblasts may be associated with the accentuated epidermal melanization observed in CALMs in the skin of NF1 patients.

    Notes: Summary Facial palsy is treated by many different operative procedures. In addition, factors such as the varying degree of paralysis, make postoperative comparisons difficult. The limited number of patients treated by free muscle transplantation at any one center also complicates evaluation of alternative treatments. This limitation has been overcome by development of an International Muscle Transplant Registry for collection, storage and analysis of data from different centers. As of June 1, 1988, 137 patients from 6 countries have been entered into the Registry. Significantly more USA/European patients had a congenital deficit and significantly more Japanese patients had a deficit resulting from surgery. According to a modification of the paresis scoring system proposed by Stennert et al. [3], USA/European patients also had a higher degree of nonsurgical facial paresis than Japanese patients. The most frequent preoperative treatment used in Japan was electrostimulation in 24% of cases, only 6% of European and American patients received this treatment. Evaluation of surgical procedures performed by the 2 groups of surgeons showed similar dimensions of muscle grafts used in the face, with a length of about 8.5 cm and a width of 3.4 cm. The extensor digitorum brevis muscle was the most popular, being used in 58% of European and American cases, whereas the Japanese choice was the gracilis muscle in 94% of cases. The length of the nerve graft used by Japanese surgeons was 6 cm, longer than that used by European and American surgeons. At this time, these preliminary data suggest evidence for paresis improvement in 79% of patients overall. Ultimately, the Registry should permit evaluation of the efficacy of different operative techniques and should influence experimental research and clinical application of muscle transplantation.

    Notes: Summary The relationship between the ischemia time of the isolated skin flap and its survival after transfer by microvascular anastomosis was studied in rat. Our result would confirm that warm ischemia of under 9 h will not adversely affect survival of transplanted skin flap while cool ischemia of under 48 h also will not have an adverse effect. Based on our histological findings, the degeneration of vessels and fatty tissue were the most important change in skin flaps resulting from ischemia.
