    Notes: Early investigations (Roberts, Lowe, Guth and Jelinek, 1958) of the ninhydrin positive substances of the whole brain of rats indicated that these substances were unchanged by various drug treatments or physiological stresses. It was mentioned at that time that studies of the whole brain could be misleading. Later work with whole brains of rats and mice (Häkkinen and Kulonen, 1961; Soep and Janssen, 1961; DeRopp and Snedeker, 1961; Kamrin and Kamrin, 1961) revealed that the administration of different drugs does alter the concentration of several of the free amino acids. Although no meaningful pattern of changes was evident, these reports raised the possibility that a meaningful pattern of changes could be found through a more detailed study with more refined techniques.In recent years the development of dual column (Spackman, Stein and Moore, 1958) and single column (Piez and Morris, 1960) amino acid analysers has facilitated the relatively rapid separation and determination of ninhydrin positive substances in biological extracts. The studies of Tallan, Moore and Stein (1954) had already established the large number of ninhydrin positive substances present in the brain of a cat as revealed by column chromatography, including unknown peaks.This paper describes in detail the procedures used in establishing the concentrations of ninhydrin positive substances in four areas of the rat brain. Other body organs and tissues were analysed for comparison to establish the presence of unknown substances peculiar to the brain.

    Notes: Streptomycetes form hydrophobic aerial hyphae that eventually septate into hydrophobic spores. Both aerial hyphae and spores possess a typical surface layer called the rodlet layer. We present here evidence that rodlet formation is conserved in the streptomycetes. The formation of the rodlet layer is the result of the interplay between rodlins and chaplins. A strain of Streptomyces coelicolor in which the rodlin genes rdlA and/or rdlB were deleted no longer formed the rodlet layer. Instead, these surfaces were decorated with fine fibrils. Deletion of all eight chaplin genes (strain ΔchpABCDEFGH) resulted in the absence of the rodlet layer as well as the fibrils at surfaces of aerial hyphae and spores. Apart from coating these surfaces, chaplins are involved in the escape of hyphae into the air, as was shown by the strong reduction in the number of aerial hyphae in the ΔchpABCDEFGH strain. The decrease in the number of aerial hyphae correlated with a lower expression of the rdl genes in the colony. Yet, expression per aerial hypha was similar to that in the wild-type strain, indicating that expression of the rdl genes is initiated after the hypha has sensed that it has grown into the air.

    Notes: Abstract A prospective study comparing open and subcutaneous lateral internal sphincterotomy for chronic anal fissure was conducted. One hundred twelve patients were randomized to open (n=54) or subcutaneous (n=58) sphincterotomy. There was no significant difference in acute complications between the subcutaneous (8.6 percent) and open (7.4 percent) groups. Postoperative length of stay was significantly shorter for the subcutaneous group (1.7±0.2 days) than for the open group (2.3±0.1 days;P 〈0.001). Although the response rate to a pain questionnaire was 〈50 percent, the data suggest a lower level of postoperative pain in the subcutaneous group. Fissure healing was similar between the subcutaneous (96.6 percent) and open (94.4 percent) groups. We conclude that subcutaneous lateral internal sphincterotomy for chronic fissure-in-ano is effective and may result in significantly less postoperative discomfort, shorter postoperative lengths of stay, and a comparable rate of complications compared with the open technique.

    Notes: Die endotracheale Intubation zählt zu den etablierten Methoden zur Sicherung der Atemwege von Notfallpatienten im präklinischen und klinischen Bereich [6]. Im Rettungsdienst erfolgt die Intubation nicht nur bei manifester respiratorischer Insuffizienz, sondern bei einer Vielzahl von Krankheitsbildern. Sie gewährleistet sicheren Schutz vor Aspiration, erleichtert die Korrektur einer vorbestehenden Hypoxie oder Hyperkapnie wie auch das Absaugen des Tracheobronchialsystems und ermöglicht die Applikation kardiovaskulär wirksamer Medikamente. Intubationsschwierigkeiten im Rahmen der präklinischen Notfallmedizin treten in aller Regel unvermittelt auf und können sich rasch zu einer akuten vitalen Bedrohung des Patienten entwickeln, die eine zerebrale Schädigung oder den Tod des Patienten zur Folge hat. Von einer schwierigen Intubation spricht man, wenn die direkte Laryngoskopie, die Plazierung oder das Einführen des Endotrachealtubus durch die Stimmritze problematisch oder unmöglich ist [8]. Der Algorithmus der endotrachealen Intubation sowie Alternativen zur Sicherung der Atemwege müssen allen im Rettungsdienst tätigen Personen vertraut sein (Abb. 1). Hierzu zählen die Oxygenierung des Patienten über eine Gesichtsmaske, die Anwendung der Larynxmaske [4, 7] und des Kombitubus © [12]. Zu den invasiven Zugangsmöglichkeiten zur Sicherung der Atemwege zählen die Koniotomie und Tracheotomie [11].

    Notes: Abstract The aim of the study was to evaluate the long-term outcome of various surgical procedures for lumbar spinal stenosis. Operations were performed on 117 consecutive patients for lumbar spinal stenosis between 1987 and 1992. Pre- and intraoperative data were recorded in a standardized manner. Three treatment groups were distinguished: group I consisting of 39 patients submitted to undercutting decompression; group II, 51 patients, submitted to laminectomy and foraminal decompression alone; and group III, 27 patients, who underwent foraminal decompression and laminectomy with instrumented fusion. Eight years (5–10 years) after surgery a questionnaire was mailed to the patients containing the outcome scales according to Greenough and Fraser [6] and Turner et al. [22] together with questions about residual pain, necessity of treatment and satisfaction with the operative outcome. A total of 72 questionnaires (61.6%) gave enough information for analysis. After a mean follow-up of 8 years, walking capacity had increased significantly in all groups (P〈0.001). Compared to preoperative values, pain had decreased significantly in all groups (P〈0.01). In group I 36% had good-to-excellent outcomes, and 30.8% and 23.8% in groups II and III (P〉0.05). Forty percent of group I patients were unsatisfied with the result, compared to 38.4% and 33.3% in the other groups (P〉0.05). Overall, 25 of 72 patients (34.7%) had severe constant back and/or leg pain requiring daily administration of analgesics. We conclude that the long-term outcome of decompressive surgery of the lumbar spinal canal, without and with instrumented fusion, is less favourable than was previously reported.

    Notes: Zusammenfassung Bei einem Fall von Ind. p. pl. konnte als Grundlage eine Endarteriitis obliterans nachgewiesen werden. Nachdem schon vor mehreren Jahren ein gleicher Fall von mir beschrieben worden ist, dürften diese Befunde über den Wert von Einzelbeobachtungen hinausgehen. Die anatomischen Veränderungen an den betroffenen Arterien gleichen weitgehend denen bei der juvenilen Gangrän und beim Kältebrand. Auf einige ätiologische und klinische Probleme wird kurz eingegangen.
