Résumé La période de demi-vie moyenne de la glycodiazine dans le sang des personnes n'ayant pas de maladie du foie et des reins est de 4.6 h (variation 3.1–5.6 h). Une élimination significativement retardée de la glycodiazine a été trouvée lors du traitement oral par phénylbutazone, phenprocumarole et doxycycline. La période de demi-vie moyenne pendant le traitement à la phénylbutazone est de 12 h (5.8–19.6 h), avec la phenprocumarole, de 7.5 h (4.3–15.7 h) et avec la doxycycline de 7.6 h (4.6–10.6 h). Seulement 3 sur 8 des patients ayant une maladie du foie ont éliminé avec retard la glycodiazine de leur sang. Parmi les malades atteints d'insuffisance rénale, seulement 1 sur 7 présenta une demi-vie prolongée de la glycodiazine dans le sang. Mais chez ces patients l'excrétion des métabolites de glycodiazine dans l'urine est nettement diminuée. On a constaté la présence de carboxy-glycodiazine dans le plasma des malades atteints d'insuffisance rénale. Avec notre méthode, ce métabolite ne pouvait pas être détecté dans le sang de sujets normaux.
Abstract:
Zusammenfassung Die Halbwertszeit von Glycodiazin im Blut leber- und nierengesunder Probanden beträgt im Mittel 4.6 Std (Schwankungsbreite 3.1–5.6 Std). Eine deutlich verzögerte Elimination von Glycodiazin aus dem Blut wurde während einer Behandlung mit Phenylbutazon, Phenprocumarol und Doxycyclin gefunden. Die Halbwertszeit betrug während der Verabreichung von Phenylbutazon im Mittel 12.0 Std (5.8–19.6 Std), von Phenprocumarol 7.5 Std (4.3–15.7 Std) und von Doxycyclin 7.6 Std (4.6–10.6 Std). Nur bei 3 von 8 leberkranken Patienten wurde Glycodiazin aus dem Blut verlangsamt eliminiert. Unter den Patienten mit Niereninsuffizienz zeigte nur 1 von 7 eine verlängerte Halbwertszeit von Glycodiazin im Blut. Die Ausscheidung der Glycodiazinmetaboliten im Urin ist bei diesen Patienten jedoch deutlich vermindert. Im Plasma der niereninsuffizienten Patienten läßt sich 2-Benzolsulfonylamin-5-carboxymethoxypyrimidin (Metabolit II) nachweisen. Dieses Abbauprodukt konnte mit der angewendeten Methode im Plasma nierengesunder Probanden nicht festgestellt werden.

Summary In a group of subjects with normal liver and kidney function, the average half-life of glycodiazine was found to be 4.6 h (range, 3.1–5.6 h). During treatment with phenylbutazone, with phenprocoumarole or with doxycycline, the elimination of glycodiazine from the blood was significantly slower. The half-life of glycodiazine during treatment with phenylbutazone was 12 h (5.8–19.6 h), with phenprocoumarole 7.5 h (4.3–15.7 h) and with doxycycline 7.6 h (4.6–10.6 h). In only 3 out of 8 patients with liver disease, and in 1 out of 7 with renal insufficiency was the elimination of glycodiazine prolonged. However, the excretion of metabolites of glycodiazine in the urine was markedly decreased in patients with renal insufficiency. Carboxyglycodiazine was found in the plasma of these patients; but with the method used, this metabolite could not be detected in the blood of healthy men.
