    Notes: The objective of the study described here was to obtain information on the beliefs of professionals concerning possible indicators of a child having been sexually abused. Data were collected by means of self-administered questionnaires, distributed at meetings on child sexual abuse. The respondents were professionals working in the field of child sexual abuse. Twenty-three social workers, 14 psychologists, 12 nurses, eight medical doctors, including three paediatricians and three psychiatrists, three policemen and two nurses participated. The variables studied were the perceived prevalence of sexual abuse in British children under the age of 10 and the perceived prevalence of various possible signs of sexual abuse in two sub-groups of these children: those who had been sexually abused, and those who had not. The signs investigated were: behavioural problems, somatic complaints, enuresis, chronic urinary infections, fear of toileting, sexualized language, sexualized behaviour, comments suggestive of sexual activity, allegations of sexual abuse, sexually transmitted diseases and reflex anal dilatation.The results indicate that these professionals have widely differing beliefs about signs which might indicate that a child has been sexually abused. We conclude that data on the prevalence of indicators of child sexual abuse are essential to ensure that appropriate informed and consistent decisions can be made.

    Notes: Abstract This paper outlines the approaches of two apparently competing schools of statistics. The criticisms made by supporters of Bayesian statistics about conventional Frequentist statistics are explained, and the Bayesian claim that their method enables research into new treatments without the need for clinical trials is examined in detail. Several further important issues are considered, including: the use of historical controls and data routinely collected on patients; balance in randomised trials; the possibility of giving information to patients; patient choice and patient autonomy; and how widely the results of clinical trials can be used. It is concluded that good statistical techniques in the design and analysis of medical studies are essential, but the statistical school used in developing such techniques is relatively unimportant.

    Notes: Abstract There is an increased awareness that total hysterectomy may be associated with subsequent vesicourethral dysfunction, but although several factors have been implicated in the cause, the precise etiology has yet to be defined. This study was carried out to determine whether damage to the pelvic innervation may be responsible for post-hysterectomy lower urinary tract dysfunction. Urodynamic evaluation and measurement of sacral latencies (SRLs) were performed in 36 women awaiting total hysterectomy. After operation vesicourethral function was altered in 30.6% of these patients, with the same incidence of neuropathy as detected by SRLs. Of the women with postoperative urinary dysfunction, 72.7% had evidence of pelvic neuropathy. The results show that there is an objectively demonstrable neurological abnormality incurred as a result of hysterectomy which is pertinent to the subsequent development of disordered micturition.

    Notes: Abstract A patient's informed consent is required by the Nuremberg code, and its successors, before she can be entered into a clinical trial. However, concern has been expressed by both patients and professionals about the beneficial or detrimental effect on the patient of asking for her consent. We examine advantages and drawbacks of popular variations on consent, which might reduce the stress on patients at the point of illness. Both informed and uninformed responses to particular trials, and trials in general, are discussed. The selection by doctors of patients, to whom entry to trials will be offered, is explored. Alternative forms of consent require restrictions on patients' knowledge, personal responsibility and freedom of choice.
