    Notes: Predictions of deep levels associated with N vacancies in AlxGa1−xN as functions of alloy composition x explain both (i) the dramatic change from naturally n-type to semi-insulating behavior (for x=xc(approximately-equal-to)0.5) in terms of a shallow-deep transition for the vacancy's T2 level, and (ii) the major photoluminescence feature in terms of recombination from the vacancy's A1 deep level. Extrinisic photoluminescence data for Zn-doped AlxGa1−xN are interpreted in terms of a transition from the conduction band to a T2-symmetric deep level in the lower part of the gap. This level is associated with antisite Zn on a N site, ZnN.

    Notes: A simple, analytical, semiempirical model is presented for the optical constants of AlxGa1−xAs including the index of refraction. The model is tailored to the energy range of 1.0–3.0 eV, the operating range for many AlxGa1−xAs-based devices and incorporates the important electronic transitions in the solid which affect light propagation and absorption. The parameters of the model are simple functions of composition allowing for ready computation of any of the optical constants, n, k, α, ε1, or ε2, for any energy E (or wavelength λ) of light between 1.0 and 3.0 eV (400–1200 nm) and any composition x.

    Notes: Abstract  Background: Although, Off-Pump Coronary Artery Bypass (OPCAB) surgery is being increasingly explored and practised in many cardiac units worldwide, there have been only few reports documenting the training of surgeons in this new technique. The purpose of this study was to address the reproducibility of the OPCAB in a unit where this technique is used extensively. Methods: Registry data, notes, and charts of 64 patients who were operated on by four trainee cardiac surgeons over a period of thirteen months at Harefield Hospital, were reviewed retrospectively. These trainees were part of an accredited training program for cardiothoracic training and were trained by a single consultant trainer in a cardiac unit after it had an established recent experience in performing nonselective OPCAB for all in-coming patients. Five (7.8%) patients (with 17 distal anastomoses) consented and underwent early postoperative angiography to check the quality of the grafts and anastomoses. Results: The mean age of the study patients was 65.6 and the mean Parsonnet score was 9.4. There was a mean of 2.9 grafts per patient and circumflex territory anastomoses were performed in 48 (75%) patients. No operation required conversion to Cardiopulmonary Bypass (CPB). Angiography of the five patients revealed 17 satisfactory (100%) distal anastomoses. Conclusion: With appropriate training, it is possible for trainees to learn OPCAB and perform multivessel revascularization in relatively high-risk patients with good results.

    Notes: We have attempted to experimentally detect a nonlinear reflection in mixtures of cholesteric and cholesteric/nematic liquid crystals induced by Nd:YAG laser pulses. At power densities of up to 4 MW/cm2 (532 nm) and 8 MW/cm2 (1064 nm) we have not observed any optically induced pitch dilation or distortion in these media.

    Notes: Microcomputer systems have been installed in the delivery suites of two obstetric units which cater between them for nearly 10 000 deliveries annually. The midwifery and medical staff enter administrative, antenatal, anaesthetic and delivery details during labour and as soon after delivery as possible. The computer prints out the official Birth Notification, the entry for the Midwives Register and the record of labour and delivery for the patient's notes. Subsequently the postnatal doctor and anaesthetist, where relevant, enter details of puerperium and anaesthetic follow-up, before the production by the computer of discharge and anaesthetic summaries for the community and records. In the first 6 months of use at the Bristol Maternity Hospital (July to December 1982) 2229 patientswere delivered and have had obstetric details recorded on the computer. A general obstetric audit of these patients is presented.

    Notes: Objective To define the relation between the size of a CIN 3 lesion (measured histologically) and invasive squamous carcinoma.Design Measurement of CIN 3 by computerized planimetry of histological sections from 39 cone biopsies containing microinvasive cancer. Comparison with previous measurements of CIN lesions, associated with different grades of dyskaryosis.Results Microinvasive carcinoma of the cervix is associated with extensive CIN 3 on the surface and in endocervical crypts. The mean size of CIN 3 lesions showing microinvasion is seven times greater than that for severe dyskaryosis without invasion, (P〈0.0001) and a 100-fold greater than with mild dyskaryosis (P〈0.00001).Conclusion Invasive squamous carcinoma of the cervix arises in large CIN 3 lesions and these are associated usually with severe dyskaryosis. We propose a model for the development of high-grade precancer as a small focus within low grade pre-cancer which then undergoes expansion in size, accompanied by apparent progression in cytological grade.

Amniocnonon ootainea at caesarean section ana vaginal delivery, at 34–36 weeks gestation and at term, have been studied by electron microscopy and immunofluorescence for evidence of amniotic epithelial degenerative changes and the presence of plasminogen. When delivery was by caesarean section between 34–36 weeks, electron microscopy revealed no degenerative changes in four cases, but in two cases they were widespread. All but one membrane obtained at term showed only minimal amniotic epithelial cell degenerative changes, but extensive change was seen when delivery was premature after premature membrane rupture. Plasminogen was seen in amniotic epithelium proportional to the degree of cell degeneration; it was absent from healthy membranes. These findings demonstrate that degenerative changes are extensively present in membranes that rupture prematurely, particularly before the onset of premature labour, and suggest a role for plasminogen in membrane rupture.
