    Notes: Sjögren's syndrome (SS) is an autoimmune rheumatic disorder characterized by chronic lymphocytic infiltration and decreased secretion in the salivary glands (SGs). For some time, apoptosis has been suggested to constitute the major mechanism for acinar epithelial destruction during the effector phases; however, this is still controversial. We have recently demonstrated that despite the expression of Fas and FasL, the incidence of apoptosis is not increased in SS patients compared with control individuals. Our aim was therefore to further evaluate the expression of the pro- and anti-apoptotic Bax and Bcl-2 proteins. CD40 and CD154 expression was also investigated, as CD40 ligation has been suggested to protect cells from Fas-mediated apoptosis. Immunohistochemical staining was performed on SG tissue from primary and secondary SS patients, a group of patients with non-SS-related degenerative changes as well as on healthy control individuals. The frequency of stained cells in the foci of infiltrating mononuclear cells (IMCs) and in the acinar and ductal epithelium was determined. We found the expression of Bcl-2 but rarely Bax in SS SG IMCs. Bcl-2 in epithelial cells was sparse, while Bax expression occurred frequently and with no significant difference between the patient groups. CD40 and CD154 expression was high among SS IMCs, while CD40 levels were slightly decreased in SS epithelium compared with controls. Elevated CD154 expression was found in SS epithelium, being significantly increased in the ducts. In conclusion, our study further supports the hypothesis about apoptosis resistance among SS focal IMCs and suggests a putative protective role of CD40 ligation in SS SG epithelium.

    Notes: Sialadenitis develops spontaneously in MRL/Mp mice bearing a lymphoproliferative gene, lpr (MRL/ Mp-lpr/lpr). Based on recent observations of an oligoclonal expansion of T-cell receptor (TCR) expressing Vβ chain families (Vβ4, Vβ8.1, 2, Vβ10b) in salivary glands of these mice we have initiated selective antibody therapy. Treatment with monoclonal antibodies (MoAb) specific for T cells expressing a mixture of TCR Vβ4, Vβ8.1, 2 and Vβ10b was applied to MRL/lpr mice before and after the spontaneous development of Sialadenitis. The in vivo treatment with Vβ4, Vβ8.1, 2 and Vβ10b MoAb did not prevent the development of Sialadenitis. However, in animals with established Sialadenitis, treatment with the MoAb significantly decreased the inflammation compared with the control groups, Immuno-histochemical staining of cell phenotypes demonstrated a change in the ratio of CD4/CD8 in the animals with established Sialadenitis. Altogether, these findings illustrate that it is possible to modulate Sialadenitis and infiltrate cell phenotypes in vivo in MRL/lpr mice with specific anti-TCR Vβ MoAb treatment.

    Notes: Abstract. The replacement of teeth by means of titanium implants is a safe and predictable procedure in most patients. Some studies show that a small number of patients lose the majority of their implants. Unfortunately, it is impossible to identify these patients prior to surgery. It is important to report such cases so that we may, in the future, be able to identify them before surgery. The present case showed a severe and rapid exfoliation of titanium implants. Out of 8 implants inserted in the anterior lower jaw of this patient, only two maintained osseointegration over a 2-year period.

    Notes: Abstract Biopsy specimens were taken at gingivectomy from 18 adult patients undergoing treatment for chronic marginal periodontitis. They were embedded so that the cut surface of the gingiva was parallel to the top of the block to obtain a comprehensive view in a transversal plane of the inflammatory cell infiltrate near the bottom of the pocket. Sections were stained with HES or with toluidine blue for histological description, and acid α-naphthyl acetate esterase (ANAE) was used to differentially stain T lymphocytes, plasma cells and monocytes/macrophages. Sections stained with HES showed that the density and size of the cell infiltrates varied along the circumference of a tooth over very short distances and on various surfaces on neighbouring teeth. Differential counts of cells stained for ANAE demonstrated great variation in the composition of the cell infiltrates, particularly along the pocket epithelium. The predominating ANAE positive cell type in this area was T lymphocytes, while in the central connective tissue, plasma cells predominated. There was no systematic covariation between the localization of the gingiva (i.e. mesial, facial, etc.) and the composition of the cell infiltrates. The local variation in the composition of the cellular infiltrate most likely reflects local variability in the noxious substances (i.e. plaque composition) within the periodontal pocket, and in the resulting local inflammatory response.

    Notes: Acetone fixed cryostat sections from 25 patients with adult chronic marginal periodontitis were characterized using an indirect immunofluorescence technique with monoclonal antibodies. The amount of B lymphocytes (Leu-12 positive) varied considerably between the specimens and were usually seen in largest numbers in the most apical parts of the cellular infiltrates beneath the pocket epithelium (PE). Varying amounts of T lymphocytes (OKT 3 positive) were demonstrated in all specimens. The amount of T helper cells (OKT 4a positive) exceeded that of T suppressor/cytotoxic cells (OKT 8 positive) in the cellular infiltrates beneath the PE (OKT 4a/ OKT 8 =1.13). There was a more even distribution of these cell types beneath the oral gingival epithelium (OGE). Langerhans cells were observed within and occasionally subjacent to the OGE. Scattered macrophages (Leu-M3 or OK Ia 1 positive) were observed in the inflammatory cell infiltrates and on the connective tissue papillae beneath the OGE. HLA-DR antigen reacting with OK Ia 1 was present on cells corresponding to OKT 6 positive cells in the OGE and subjacent to the OGE as well as in the inflammatory cell infiltrates beneath the PE and in the perivascular infiltrates. In some specimens HLA-DR antigen was also found to be associated with keratinocytes in the outer parts of the OGE. Occasional NK cells (Leu-7 positive) were localized inside and subjacent to the OGE. There was a considerable variation with respect to the number and distribution of the various mononuclear cells between specimens and from section to section from the same specimen.

    Notes: The inflammatory cell infiltrates in human dental periapical granulomas were studied through the demonstration of receptors (R) for fragments C3b and C3d of the complement component C3. C3bR and C3dR were detected in cryostat sections by the closed chamber hemadsorption technique. Sheep erythrocytes sensitized with rabbit IgM antibody and coaled with human C3b or C3d were used as indicator cells. Of the 62 granulomas studied. 23 showed strong or moderate C3bR activity, and weak C3dR activity was detected in 5 out of 25 specimens. The C3bR activity was abolished by treatment with 0.25 mM periodic acid, indicating that carbohydrate is a pan of the C3bR. Using an antiserum to C3R as first layer in indirect immunofluorescence experiments, the C3R was localized to cell membranes. The low C3dR activity indicates that a minor part of the mononuclear cells in the human periapical granulomas were B lymphocytes.

    Notes: In the last three decades, more work has been done on apoptosis and its role in the pathogenesis of many diseases including cancer. In almost all instances of cancer, dysregulation of cell death (apoptosis) and cell proliferation have been found to play a major role in tumourigenesis. A lot of progress has been made on understanding the molecular basis of apoptosis and its regulatory mechanisms. This review focuses on current knowledge on the regulation of apoptosis in oral squamous cell carcinoma, current methodologies and methodological consideration in estimation of cell death in tissue sections and the clinical significance of apoptosis related molecules in progression of oral squamous cell carcinoma.

    Notes: Interstitial fluid pressure (IFF) in normal and inflamed gingiva of rats was measured by sharpened glass capillaries (diameter 2–4 μ) connected to a servocontrolled counterpressure system ad modum Wiederhielm. Gingival inflammation was induced by a steel ligature placed unilaterally around the first maxillary molar. After 8 weeks, the ligature was removed and measurements of IFF in normal and experimentally inflamed gingiva were made by puncture through the intact oral epithelium. Average IFF on the control side was 3.5 mm Hg in free, and 7.4 mm Hg in attached gingiva. Compared to the control side. IFF on the experimental inflamed side was elevated significantly (p〈0.05) both in free and attached gingiva. In the ligated inflamed side IFF in free gingiva averaged 6.7 mm Hg, whereas in attached gingiva it showed a mean value of 10.4 mm Hg. Histological examinations showed a dense infiltrate of inflammatory cells in the connective tissue, while the contralateral control side showed normal features of epithelial and connective tissue. The present finding of increased IFF in the inflamed gingiva may in theory contribute to augmented crevicular fluid flow during inflammation.

    Notes: The binding of particulate (sensitized sheep erythrocytes= EA) and soluble (horse radish peroxidase (HRP)-anti-HRP) IgG immune complexes to cryostat sections of inflamed human gingiva was studied. EA bound preferentially to inflammatory cell infiltrates in all biopsy specimens, and HRP-anti-HRP complexes were found to bind to the surface of mononuclear cells. Binding of EA was abolished by treatment of cryostat sections with 1 mM periodic acid (30 min) or 4% formaldehyde (10 min). Binding was also abolished by exposure of sections to cold ethanol for 5 min. No binding of fluorescein labelled, heat aggregated human IgG to sections of washed and ethanol-fixed biopsies of inflamed human gingiva was observed. Results of inhibition experiments with rabbit and human native or heat aggregated IgG, rabbit and human IgG Fc and IgG F(ab′)2 fragments, human IgG 1 and IgG2, mildly reduced and alkylated human IgG and with rabbit anti-human Clq, indicated that the binding is not due to activated C1 or to the presence of rheumatoid factors. Our findings are consistent with the concept that the binding is due to the presence of IgG Fc receptors (IgGFcR) on mononuclear cells. Ethanol fixation of specimens compromises any conclusions regarding IgGFcR activity in the tissues.
