Summary Prognathism can be considered in three different age groups, there being a special type of treatment for each group: 1. the deciduous dentition group, 2. the mixed dentition group and 3. the permanent dentition group. Early treatment during the deciduous dentition is aimed at impeding any abnormal development of the dentition. During the mixed dentition it is important to impede growth in the mandible and produce a forward movement in the maxilla. In cases of prognathism in the permanent dentition with a small incisal overjet orthodontic therapy with fixed appliances is indicated. Only in those cases of prognathism in the permanent dentition with an reversed overjet and an unfavourable external appearance do we correct by surgical operation. Examples are given of the different forms of treatment.

Résumé Nous appliquons trois modes thérapeutiques différents selon la classe d'âge où l'on traite la prognathie mandibulaire: 1. en denture temporaire, 2. en denture mixte, 3. en denture permanente. En denture temporaire le traitement doit être interceptif. En denture mixte, il est souhaitable de provoquer une inhibition de la croissance longitudinale de la mandibule et de favoriser la croissance longitudinale et transversale du maxillaire. En denture permanente, un profil équilibré et une inversion de l'occlusion incisive légère militent pour des appareillages fixes supérieur et inférieur; un profil disgracieux et un fort décalage incisif inversé mènent à l'opération. Des exemples caractéristiques illustrent les différentes thérapeutiques.

Zusammenfassung Wir unterscheiden drei verschiedene Altersklassen beim Vorliegen der echten Progenie, die jeweils einer speziellen Behandlung bedürfen: 1. die Progenie im Milchgebiß, 2. die Progenie im Wechselgebiß und 3. die Progenie im bleibenden Gebiß. Die Frühbehandlung im Milchgebiß soll ein vorzeitiges Abfangen der abwegigen Gebißentwicklung bewirken. Auch für die Progenie im Wechselgebiß ist eine Hemmung des Unterkieferlängenwachstums und eine Förderung des Breiten- und Längenwachstums im Oberkiefer anzustreben. Bei der Progenie im bleibenden Gebiß mit geringer Stufe und ausgeglichenem Profilverlauf ist eine orthodontische Behandlung mit festsitzenden Apparaturen (=Multiband) im Oberkiefer und Unterkiefer indiziert. Lediglich bei der Progenie im bleibenden Gebiß mit extremer positiver Stufe im Frontzahnbereich und störendem progenen Profilverlauf führen wir die Korrektur operativ durch. Das unterschiedliche therapeutische Vorgehen wurde anhand von charakteristischen Fällen besprochen.
