    Notes: Summary The authors compared the results of a retrospective analysis of two groups of head-injured patients who had coexistent pelvic or lower extremit fractures. One group was treated with early osteosynthesis within the first 12 hours after trauma, simultaneously with neurosurgical treatment, while the second group was treated neurosurgically and osteosynthesis was postponed for 4 to 10 days. The second group revealed a higher mortality, which was due to fat embolism. We conclude that early osteosynthesis is the treatment of choice in patients with coexistent head injury and lower extremity fractures.

    Description / Table of Contents: Zusammenfassung Die Autoren beschreiben hier die Symptome nach SchÄdelhirnverletzungen, die hinsichtlich der Art, Lokalisation oder der Zahl intrakranieller posttraumatischer Komplikationen irreführend sein können. Sie bemühen sich, Ursachen solcher irreführender Symptome aufzuzeigen. Besonders zu beachten sind folgende Faktoren: die Zeit zwischen Trauma und Aufnahme in der Neurochirurgischen Abteilung, der Allgemeinzustand des Kranken und sein Alter, eine sogenannte atypische Lokalisation eines intrakraniellen HÄmatoms und das neurologische Syndrom. Au\erdem ziehen die Autoren in Betracht, da\ durch eine zu mechanische Interpretation der Ergebnisse der Zusatzuntersuchungen (Röntgenübersichtsaufnahmen, Lumbalpunktion und selbst zerebrale Angiographie) Irrtümer verursacht werden können.
    Abstract: Summary The authors present the problem of deceptive signs which could falsely indicate the nature, the site, or the number of intracranial complications developing after an injury. They also seek to uncover those factors which underly the appearance of deceptive elements in the clinical syndrome, or which contribute to a false assessment of the clinical picture. Among these elements the authors consider:—the time which elapses between the injury and the arrival of the patient in a neurosurgical unit, the age and general state of the patient, a so-called “atypical” site of the intracranial haematoma, and the neurological syndrome. The authors also consider deceptive features derived from a “mechanical” interpretation of the ancillary examinations such as plain radiography, lumbar puncture and even cerebral angiography.
    Notes: Résumé Les auteurs présentent le problème des symptomes trompeurs qui peuvent indiquer d'une faÇon décevante la nature, la localisation ou le nombre de complications intra-crâniennes post-traumatiques. Ils s'efforcent également de mettre en évidence les facteurs déterminant l'apparition de ces éléments trompeurs du syndrome clinique ou contribuant à la valeur erronée du tableau clinique. Les auteurs considèrent, parmi ces éléments, les facteurs suivants: le temps dans lequel le malade arrive après le traumatisme au service neuro-chirurgical, l'état général du malade et son âge, une localisation dite «atypique» de l'hématome intracrânien, le syndrome neurologique. Les auteurs considèrent aussi les aspects fallacieux indiqués par une interprétation «mécanique» des résultats des examens complémentaires tels que radiographie standard, ponction lombaire et mÊme angiographie cérébrale.
