    Notes: Objective To compare a one-step (rapid) application of negative pressure (vacuum) with conventional stepwise application for ventouse extraction following a prolonged second stage of labour.Design Randomised controlled study.Setting Teaching Hospital.Participants Forty-seven women were randomised to the rapid vacuum group and 47 to the stepwise group.Main outcome measures Duration of ventouse procedure, effectiveness of methods of application, morbidity of mother and infant.Results There was no significant difference in frequency of detachment of the cup after rapid or stepwise application of vacuum. A reduction in mean duration of the ventouse procedure of 6 min was realised without significant difference in maternal or neonatal morbidity.Conclusion Rapid application of vacuum significantly reduces the duration of a ventouse extraction procedure without compromise to efficiency and safety.

    Notes: Summary Ten patients with impotence due to venous leakage who had undergone resection of the deep dorsal penile vein (DDPV) without showing a good clinical late response underwent a second pharmacocavernometry/ graphy. In two patients we could no longer demonstrate leakage, and in three others we found a decrease in leakage. Eight times we observed persistent leakage through the remaining proximal part of the DDPV. In two patients we succeeded in restoring erectile potency by occluding the remaining proximal part of the DDPV with detachable balloons. Our findings suggest that better results can probably be obtained when the proximal part of the DDPV is occluded as well.

    Notes: Summary The pharmacokinetics of sulfametopyrazine were studied for seven days after a single oral dose of 2 g. in healthy volunteers in order to establish its chemotherapeutic value. — The appearance and disappearance of the drug in the plasma were evaluated both for compounds with a free amino group and for total sulphonamides. The half-life and absorption, distribution, elimination and excretion coefficients were calculated, as well as the concentrations in plasma water and interstitial fluid. The estimated drug concentrations in the urine agreed with those calculated from the excretion coefficients. — In all subjects at the end of the seventh day the concentrations in all body compartments of active compounds exceeded the minimum required for a therapeutic effect. The highest concentrations found in the urine were always significantly lower than the drug's basal solubility at pH 5, thus excluding any risk of crystalluria.
