Abstract Intramedullary (subpial) lipomas not associated with spinal dysraphism are very unusual [1]. A confident preoperative diagnosis is usually possible with MRI. We present a case of an isolated cervical intramedullary lipoma with unusual imaging morphology and imaging characteristics (distinctly low-signal on T2-weighted images) on MRI. An explanation for these features is proposed.
